MRSA in lower limb amputation and the role of antibiotic prophylaxis.

AIM: Methicillin Resistant Staphylococcus Aureus (MRSA) colonisation is reported in 3-20% of vascular patients. Many develop infective complications. MRSA is associated with poor prognosis. Aim of the study is to assess MRSA in lower limb amputation and efficacy of antibiotic prophylaxis. METHODS: Prospective study of lower limb amputation. MRSA screen and wound swabs were taken at operation. Antibiotic prophylaxis included teicoplanin (400 mg) 1 dose at operation. RESULTS: Twenty-five patients underwent 33 primary amputations. At operation 15 legs (45%) were colonised with MRSA and 18 legs

[Critical ischemia of hip stump. Incidence, clinical significance and diagnosis].

AIM: To study the incidence of hip stump ischemia, risk factors for 5-year survival after amputation, clinical features and diagnosis of this complication. MATERIAL AND METHODS: 1287 patients with peripheral artery disease followed by hip amputation were examined within 1997-2015. 50 of them had stump ischemia. RESULTS: It was found that hip stump ischemia occurs in 5.2% of cases and significantly aggravates 5-year survival. Transcutaneous oxygen tension less than 20 mm Hg is reliable hemodynamic criterion of ischemia.

Free-flap free flap.

We present a patient who sustained bilateral below-knee amputations that were treated with skin grafts as initial coverage. A latissimus dorsi free flap was later used as definitive coverage of one stump. Then at a subsequent operation a portion of the same latissimus dorsi free flap was reharvested, again as a free flap, and transplanted to cover the contralateral stump. Thus one latissimus dorsi free flap was used twice as a free flap (free-flap free flap) to cover bilateral amputation stumps in sequential operations.

Pedicled Sensate Composite Calcaneal Flap in Children With Congenital Tibial Pseudoarthrosis.

BACKGROUND: The preservation and functionality of a limb affected by a malformation (such as congenital pseudoarthrosis of the tibia) or a severely mangled lower limb in children, despite modern reconstructive techniques, remains challenging, often eventually requiring amputation to achieve a better outcome. The classical Syme and Boyd procedures are functionally better than transtibial (TT) amputation, but are not feasible for congenital tibial pseudoarthrosis. TT amputation delivers an excellent, effective, and functional stump that usually leads, after prosthetization, to a functional gait.

Use of lower limb prosthesis among elderly amputees.

Forty-seven elderly patients (21 males and 26 females) with lower limb amputation, and with prostheses fitted were studied. Their ages ranged from 65 years to 91 years old, with an average of 75.4 years. The three most common contributory causes for amputation were diabetes mellitus, vascular occlusion and carcinoma of the skin. Among the lower limb amputations, below knee amputation was the most common (93.6%). About 30% of the patients developed complications from prostheses, the 3 most common in order of frequency were stump pain, pressure ulcers and stump infection. Male patients tended to

Endoscopic revision of an infected Syme amputation. Case report and review of the literature.

Arthroscopic revision of intra-articular infection is more and more accepted. To our knowledge, there is no description of endoscopic management of an infected amputation stump. This paper presents the case of a 54-year-old man after a Syme amputation of his left leg because of diabetic gangrene of the foot. A local infection of the stump was treated successfully with two endoscopic debridements and lavage. Additionally, the patient received local antibiotics by the application of antibiotic collagenous fleece and chains. This therapeutic management combined with early weight-bearing produced

[Forefoot and midfoot amputations].

OBJECTIVE: Partial foot amputations are feasible regardless of the causal condition, including peripheral vascular disease with a few exceptions. Compared to higher amputation levels, a good foot stump permits full end bearing and enables the patient, even with a hindfoot stump, to walk without the need for a prosthesis. The more peripheral the amputation level selected, the greater the need for gentle tissue handling and meticulous postoperative care, but also the greater the risk of a breakdown requiring stump revision surgery. In the forefoot, partial amputation of the metatarsals preserves

Ultrasound in the detection and treatment of a painful stump neuroma.

Ultrasound is an established modality for the evaluation of neuromas, particularly Morton's neuromas. It has also had success in the assessment of amputation stump neuromas. While ultrasound localization for steroid injection has proven useful in the conservative treatment of Morton's neuromas, it has not yet been established as a localization technique for steroid injection to successfully treat amputation stump neuromas.

A novel technique for transtibial amputation in chronic occlusive arterial disease: Modified Burgess procedure.

The aim of this study was to compare results of transtibial amputations performed using the Burgess or Modified Burgess techniques with those performed using the Bruckner technique. Transtibial amputation (TTA) was performed in 117 patients with Fontaine phase IV chronic arterial occlusion. Fifty-six patients with a mean age of 51.4 years were amputated by the Burgess technique between March 2000 and January 2003; and 61 patients with a mean age of 47.8 years were amputated by Modified Burgess technique between February 2003 and March 2006. In the Burgess technique, all muscles in the amputati

[Transtibial amputation].

OBJECTIVE: To make a transtibial stump as long as possible, free from local and phantom pain with a maximum of terminal load bearing. In order to preserve the knee joint, an ultra-short tibial stump of 5-6 cm may be indicated. INDICATIONS: A hindfoot amputation level is not possible to achieve. CONTRAINDICATIONS: In amputations for peripheral vascular diseases, amputations through the distal third of the tibia are not recommended. If they still heal, the level selection might have been too proximal. SURGICAL TECHNIQUE: According to Verduyn and Burgess, a long posterior muscular flap coverin

Aortobifemoral bypass for the critically ischaemic limb--is it worth while?

Little is written of the place of aortobifemoral bypass as a limb or below-knee-level amputation stump salvage procedure in patients presenting with critical ischaemia with threat of limb loss. Over a 4-year period 151 patients referred to the Vascular Service of the University of Natal Hospitals with aorto-iliac occlusive disease and a threatened limb were studied. All were submitted to aortobifemoral bypass. Patients were divided into two subgroups: group 1 patients presented with rest pain or focal necrosis and were submitted to aortobifemoral bypass with concomitant digital or transmetatar

Epidemiologic data of trauma-related lower limb amputees: A single center 10-year experience.

OBJECTIVE: The aim of this study is three fold: 1) to introduce epidemiologic data of patients with trauma-related amputations as a 10-year experience of a rehabitation center; 2) to determine comorbidities and secondary conditions of lower limb loss; 3) to determine the rehospitalization reasons for lower limb amputee patients. MATERIALS AND METHODS: This retrospective study was conducted in a tertiary rehabilitation center in Turkey. Clinical and demographic data of amputees including sex, age, employment status, time since amputation, time after amputation to first hospitalization, length

Microvascular free on-top plasty in multidigital amputations.

Bidigital amputations proximal to the PIP joints severely impair function and cosmesis of the hand. The author presents a clinical series of five patients in which a three-fingered hand was reconstructed through microvascular transfer of one of the stumps on top of the other. Five male patients with amputation of two fingers proximal to the PIP joint were treated surgically. One of the stumps was transferred as a local free composite flap on top of the other stump. All five transfers were successful. The mean operative time was 2 h 15 min. The subjective improvement on hand function and cosmes

Amputation for reflex sympathetic dystrophy.

We have reviewed 28 patients with reflex sympathetic dystrophy (RSD) who had 34 amputations in 31 limbs. The amputations had been performed for untenable pain (5), recurrent infection (14) or to improve residual function (15). Only two patients were relieved of pain by amputation, and this could not be predicted. Ten of 14 patients were cured of infection and 9 of 15 patients had improvement of residual function. In 28 of the amputations, RSD recurred in the stump, especially after amputation at a level which was not free from symptoms. Because of recurrence of RSD in the stump or severe hyper

A case series featuring extremely short below-knee stumps.

BACKGROUND: Lower limb amputations should be evaluated carefully, especially with regard to the possibility of preserving the knee joint to enable a more physiological gait and lower energy consumption. Below-knee amputations were performed immediately below the tibial tuberosity with maintenance of the insertion of the patellar tendon, resulting in very short, but functional stumps. This case study examined whether very short below-knee stumps allow a more functional gait, as compared to more proximal amputations. CASE DESCRIPTION AND METHODS: Between June 2010 and June 2011, four patients h
