Molecular imaging of TTR amyloidosis: New data.

Transthyretin cardiac amyloidosis (ATTR CM) is a recognized cause of heart failure that results from age related or hereditary transthyretin misfolding and systemic amyloid deposition. It is characterized by clinical heterogeneity and may affect the heart among peripheral and autonomous nervous system, gastrointestinal tract, kidneys and eyes. Technetium 99m bone avid scintigraphy with standardized planar and SPECT/CT protocols, interpreted using Perugini grading and heart to contralateral lung ratios after exclusion of light chain amyloidosis, enables highly sensitive, non invasive diagnosis

Biomarkers in Subclinical Transthyretin Cardiac Amyloidosis.

PURPOSE OF REVIEW: The most common type of cardiac amyloidosis is transthyretin amyloidosis (ATTR-CM). Early forms of the disease can often go undetected. Effective pharmacological treatments are available for ATTR-CM. However, current treatment options may be more effective when used earlier in the disease, making early detection paramount. Below, we discuss updates with regards to the role that blood-based biomarkers play in detecting subclinical cardiac amyloidosis. RECENT FINDINGS: Carriers of amyloidogenic mutations in the TTR gene are at a heightened risk of developing heart failure and

From pathophysiology to treatment in transthyretin cardiac amyloidosis.

Amyloid transthyretin (ATTR) amyloidosis is a progressive and multifaceted disorder characterized by extracellular deposition of misfolded transthyretin (TTR) in tissues, leading to irreversible damage. The rising prevalence of this disease underscores the urgent need for a deeper understanding of its complex pathology to improve diagnosis and treatment strategies. This Review explores the cellular and molecular mechanisms underlying ATTR cardiac amyloidosis (ATTR-CA), delving into the processes driving disease progression, exploring the recent progress in the experimental models used to study

A review of Eplontersen use in hereditary transthyretin amyloidosis.

Hereditary transthyretin amyloidosis is a progressive life-threatening disease characterized by deposition of abnormally folded amyloid fibrils into tissues leading to polyneuropathy and cardiomyopathy. Eplontersen, an antisense oligonucleotide has been FDA approved for the treatment of hereditary transthyretin amyloidosis with polyneuropathy in the United States. Eplontersen inhibits the translation of both variant and wildtype transthyretin in the liver, thereby preventing deposition in tissues. In the pivotal Phase III NEURO-TTRansform trial, Eplontersen significantly lowered serum transthy

Diagnostic & therapeutic challenges of presymptomatic hereditary transthyretin amyloidosis.

Hereditary transthyretin amyloidosis (ATTRv) is a rare, lethal, autosomal dominant adult-onset genetic disorder provoked by mutations in the TTR gene. Until recently, therapeutic options were limited to liver transplantation and TTR stabilizers. Antisense oligonucleotides (ASO) and small interfering RNA (siRNA) treatments, now commercially available, have dramatically improved ATTRv neurological outcome, and affected patients' relatives are increasingly being identified at the presymptomatic stage. Guidelines for monitoring presymptomatic patients have been established by different groups. ATT

[Acoramidis (Beyonttra®) : a new era in the treatment of transthyretin amyloid cardiomyopathy].

Transthyretin amyloid cardiomyopathy (ATTR-CM), a rare infiltrative disease associated with the accumulation of transthyretin-derived amyloid deposits, exists in an age-related wild-type form (ATTRwt) and a hereditary form (ATTRv). Myocardial infiltration leads to ventricular stiffness, diastolic heart failure, and a poor prognosis in the absence of disease-specific treatment. After tafamidis, acoramidis, a high-affinity tetrameric stabilizer, represents a major advance. In the ATTRibute-CM trial, it demonstrated a significant reduction in mortality, cardiovascular hospitalizations, and functi

Cognitive manifestations and brain integrity in hereditary transthyretin amyloidosis: a systematic review.

BACKGROUND: Central Nervous System involvement in hereditary transthyretin amyloidosis (ATTRv) is present in liver transplanted patients with longstanding ATTRV30M amyloidosis, and in some rarer variants. The pathophysiology of brain involvement and its relationship with cognitive disturbances is unknown. This systematic review summarized the literature on brain and cognitive involvement in ATTRv amyloidosis and aimed to elucidate the reasons for such involvement. METHODS: The literature search was performed using the following databases: Medline/PubMed, Embase via Elsevier, Scopus, and Web o

Specific Therapy in Transthyretin Amyloid Cardiomyopathy: Future Perspectives Beyond Tafamidis.

Transthyretin amyloid cardiomyopathy (ATTR-CM) is a relatively prevalent cause of morbidity and mortality. Over the recent years, development of disease-modifying treatments has enabled stabilization of the circulating transthyretin tetramer and suppression of its hepatic production, resulting in a remarkable improvement in survival of patients with ATTR-CM. Second-generation drugs for silencing are currently under investigation in randomized clinical trials. In vivo gene editing of transthyretin has been achieving unanticipated suppression of hepatic production in ATTR-CM. Trials of antibodie

Long-term survival in people with transthyretin amyloid cardiomyopathy who took tafamidis: A Plain Language Summary.

WHAT IS THIS PLAIN LANGUAGE SUMMARY ABOUT?: This summary presents the results from an ongoing, long-term extension study that followed an earlier study called ATTR-ACT. People who took part in this extension study and ATTR-ACT have a type of heart disease known as transthyretin amyloid cardiomyopathy (ATTR-CM for short), which causes heart failure and death. In ATTR-ACT, people took either a medicine called tafamidis or a placebo (a pill that looks like the study drug but does not contain any active ingredients) for up to 2½ years. So far, in the long-term extension study, people have continue

Data-independent acquisition mass spectrometry reveals comprehensive plasma protein profiles in the natural history of patients with hereditary transthyretin amyloidosis (ATTRv).

OBJECTIVES: Hereditary transthyretin amyloidosis (ATTRv) is a rare, fatal, autosomal dominant disease with more than 140 mutations discovered. Three phenotypes of amyloid infiltration are neuropathy (ATTRv-PN), cardiopathy (ATTRv-CM), and neuropathy + cardiopathy (ATTRv-MIX). The lack of ATTR-specific biomarkers, difficulties in biopsy evidence, and limited known pathogenic mechanisms have made diagnosis difficult. Newly emerging noninvasive measures for monitoring progression and disease-modifying therapies have improved early diagnosis and patient management. METHODS: Our research applies t

Health impact of tafamidis in transthyretin amyloid cardiomyopathy patients: an analysis from the Tafamidis in Transthyretin Cardiomyopathy Clinical Trial (ATTR-ACT) and the open-label long-term extension studies.

AIM: The Tafamidis in Transthyretin Cardiomyopathy Clinical Trial (ATTR-ACT) showed that tafamidis reduced all-cause mortality and cardiovascular-related hospitalizations in patients with transthyretin amyloid cardiomyopathy (ATTR-CM). This study aimed to estimate the impact of tafamidis on survival and quality-adjusted life-years (QALYs). METHODS AND RESULTS: A multi-state, cohort, Markov model was developed to simulate the disease course of ATTR-CM throughout a lifetime. For survival extrapolation, survival curves were fitted by treatment arm and New York Heart Association (NYHA) Class I/II
