Purpuric phototherapy-induced eruption in transfused neonates: relation to transient porphyrinemia.

OBJECTIVE: Blue light phototherapy is commonly administered to neonates as treatment of indirect hyperbilirubinemia, often in conjunction with blood transfusions to treat hemolytic anemia. We observed a distinctive cutaneous complication of phototherapy in six neonates with hyperbilirubinemia. METHODOLOGY: We studied the clinical and histologic characteristics of the eruption, as well as the porphyrin levels in affected neonates. Five of the patients had erythroblastosis fetalis; the other had profound anemia from twin-twin transfusion. All of the neonates developed purpuric patches at sites

Screening, Treatment, and Monitoring of Iron Deficiency Anemia in Pregnancy and Postpartum.

Iron deficiency anemia is the most prevalent form of anemia worldwide. In the United States, clinicians routinely screen for iron deficiency anemia upon initiation of prenatal care, at the start of the third trimester, and prior to birth. Treatment of iron deficiency anemia generally begins with oral supplementation of elemental iron, which is associated with adverse gastrointestinal effects. These adverse effects can decrease adherence, leading to subtherapeutic treatment. Newer evidence highlights the benefits of early screening for iron deficiency before the onset of anemia, as well as the

Erythrokinetic mechanism(s) causing the "late anemia" of hemolytic disease of the fetus and newborn.

A transfusion-requiring "late anemia" can complicate the management of neonates convalescing from hemolytic disease of the fetus and newborn (HDFN). This anemia can occur in any neonate after HDFN but is particularly prominent in those who received intrauterine transfusions and/or double-volume exchange transfusions. Various reports describe this condition as occurring based on ongoing hemolysis, either due to passive transfer of alloantibody through breast milk or persistence of antibody not removed by exchange transfusion. However, other reports describe this condition as the result of inade

Current issues in neonatal transfusions.

Two controversial issues of neonatal transfusion practices, erythrocyte 'booster' transfusions and granulocyte transfusions, are critically reviewed, and current recommendations for transfusion practices are made. Infants should receive erythrocyte transfusions to treat congestive heart failure caused primarily by anemia. It is customary to maintain the hematocrit at greater than 40% in neonates with severe respiratory disease, although the efficacy of this practice has not been firmly established. Erythrocyte transfusions seem to be indicated for infants with anemia plus recurrent apnea, poor

[Onset of Glycogen Storage Disease Type Ⅻ in Two Brothers in the Neonatal Period].

Glycogen storage diseases (GSDs) are a group of autosomal recessive disorders of glucose metabolism.GSDs are caused by congenital deficiency of enzymes in glycogen synthesis or decomposition, which results in glycogen accumulation in organs.According to the types of enzyme deficiency,GSDs can be classified into more than ten types, among which GSD Ⅻ is a super-rare type of GSD.Two brothers with a 5-year age difference presented severe neonatal asphyxia, myasthenia, myocardial damage, anemia, and mental retardation, being GSD Ⅻ homozygous cases with neonatal onset. The results of gene detection showed

Neonatal lupus erythematosus with multisystem organ involvement preceding cutaneous lesions.

Here we report a case of neonatal lupus erythematosus syndrome presenting with multisystem organ involvement, including anemia, thrombocytopenia, purpura, bloody diarrhea, enzymatic liver abnormalities, splenomegaly and pneumonitis. These findings preceded the cutaneous rash that was the clue for the diagnosis. The patient's mother had an undiagnosed subacute cutaneous lupus erythematosus. The various forms of onset of neonatal lupus erythematosus syndrome are emphasized.

Protocol for the Redefining Maternal Anemia in Pregnancy and Postpartum (ReMAPP) study: A multisite, international, population-based cohort study to establish global hemoglobin thresholds for maternal anemia.

BACKGROUND: Anemia affects one in three pregnant women worldwide, with the greatest burden in South Asia and sub-Saharan Africa. During pregnancy, anemia has been linked to an increased risk of adverse maternal and neonatal health outcomes. Despite widespread recognition that anemia can complicate pregnancy, critical gaps persist in our understanding of the specific causes of maternal anemia and the cutoffs used to diagnose anemia in each trimester and in the postpartum period. METHODS AND ANALYSIS: The Redefining Maternal Anemia in Pregnancy and Postpartum (ReMAPP) study is a multisite, pros

Maternal allo anti-M antibody-induced hemolytic disease of newborn.

Hemolytic disease of the fetus and newborn is a syndrome associated with immune destruction of the fetal and newborn red cells by maternal red cell alloantibodies. The detection of anti-M in antenatal screening can be responsible for neonatal red cell aplasia. A 32-h-old full-term neonate admitted with inconsolable cry and mild fever. Laboratory tests revealed progressive anemia and hyperbilirubinemia on day 3. The peripheral blood smear showed evidence of hemolysis and reticulocyte count was reduced. Intensive phototherapy and antibiotics were started after ruling out other causes of hyperbil

Infantile pyknocytosis: A rare but not so rare condition. A new case report.

Infantile pyknocytosis is a rare cause of neonatal jaundice associated with transient hemolytic and usually severe anemia, with a favorable outcome. Its etiology remains undetermined. The diagnosis is mainly based on the presence of abnormal erithrocytes (pyknocytes) on the peripheral blood smears. We report the description of a new case recently occurring in a 12-day old caucasian male. The newborn was submitted, together with his parents, to a complete diagnostic process including erithrocytes tests for osmotic resistance, membrane fragility and lysis, and the study of hemoglobin stucture. Anti-e alloimmunization: a rare cause of severe fetal hemolytic disease resulting in pregnancy loss.

We report a case of severe intrauterine hemolysis caused by sole anti-E alloimmunization. A 36-year-old multipara woman presented with hydrops fetalis at 27 weeks of gestation. She had a history of previous neonatal death. In this pregnancy, she was found to have very high titer of anti-E antibody. Ultrasonography detected marked skin edema, cardiomegaly, hepatosplenomegaly, pleural effusion, ascites, placentomegaly, and polyhydramnios. The Doppler peak systolic velocity in the middle cerebral artery was 0.8 m/s, indicating severe fetal anemia. Multiple intrauterine transfusions for the anemic

Clinical Significance of Maternal Anti-D Antibody Titers in Severe RhD Alloimmunization Requiring Intrauterine Transfusion: A Tertiary Referral Center Experience.

Background: Maternal anti-D antibody titers are routinely used for risk stratification in RhD alloimmunization before the development of fetal anemia. However, their association with disease severity and perinatal outcomes among pregnancies that have already progressed to severe fetal anemia requiring intrauterine transfusion (IUT) remains unclear. This study aimed to investigate the association between maternal anti-D antibody titers, disease severity, and perinatal outcomes in pregnancies complicated by severe RhD alloimmunization requiring IUT. Methods: This retrospective cohort study inclu

Managing the difficult case of fetal anemia.

OBJECTIVES: To describe a series of complex fetal anemia cases, detail the appropriate investigations and management, and review the literature. METHODS: Four cases of non-red cell alloimmunization or infective cases of fetal anemia are presented. RESULTS: Of the four cases presented, one was a neonatal death, one pregnancy was terminated, one case was diagnosed with Diamond Blackfan anemia, and one case was due to recurrent feto-maternal hemorrhages despite negative Kleihauer tests. CONCLUSIONS: Non-alloimmune causes of fetal anemia can be difficult to manage. Some cases require repeated a

Development of a Risk Assessment Model for Predicting Red Blood Cell Transfusion in Neonatal Patients.

BACKGROUND: The goal was to develop a risk assessment model for predicting red blood cell (RBC) transfusion in neonatal patients to assist hospital blood supply departments in providing small portions of RBCs to those requiring RBC transfusion on time. METHODS: Clinical information was collected from 1,201 children admitted to the neonatal unit. Clinical factors associated with predicting RBC transfusion were screened, and prediction models were developed using stepwise and multifactorial logistic regression analyses, followed by the evaluation of prediction models using receiver operating ch

Non-Anemic Iron Deficiency in Pregnancy: A Cross-Sectional Study of Prevalence and Predictors.

Iron deficiency (ID) is the most common micronutrient deficiency worldwide and the leading cause of anemia in pregnancy. However, non-anemic iron deficiency (NAID) often precedes anemia and is associated with adverse maternal and neonatal outcomes. This cross-sectional study screened pregnant individuals at their first obstetric visit for iron deficiency using both laboratory measures and a standardized risk factor questionnaire. Among 201 participants, 175 completed laboratory testing. The prevalence of ID was 59% (54% NAID 54%, 5% iron deficiency anemia [IDA]), highlighting a substantial bur

Pearson syndrome: unique endocrine manifestations including neonatal diabetes and adrenal insufficiency.

PURPOSE: Pearson syndrome is a very rare metabolic disorder that is usually present in infancy with transfusion dependent macrocytic anemia and multiorgan involvement including exocrine pancreas, liver and renal tubular defects. The disease is secondary to a mitochondrial DNA deletion that is variable in size and location. Endocrine abnormalities can develop, but are usually not part of the initial presentation. We report two patients who presented with unusual endocrine manifestations, neonatal diabetes and adrenal insufficiency, who were both later diagnosed with Pearson syndrome. METHODS:

Tailing growth, neonatal jaundice and anaemia.

At medical school, there is a phrase to help us remember that common things are common: 'If you hear hooves think horses, not zebras'. However, zebras do exist, and from time to time in general paediatric and neonatal practice, we will encounter these rare diagnoses, more of which we can now accurately diagnose through the ever-expanding field of genomics. Our case demonstrates how a rare diagnosis can present with common features of growth restriction, jaundice and anaemia. Paediatricians therefore require a high index of suspicion and increasing knowledge of the logistics of genetic testing.

Anaemia of prematurity: pathophysiology and treatment.

Most infants with birth weight <1.0 kg are given multiple red blood cell (RBC) transfusions within the first few weeks of life. The anaemia of prematurity is caused by untimely birth occurring before placental iron transport and fetal erythropoiesis are complete, by phlebotomy blood losses taken for laboratory testing, by low plasma levels of erythropoietin due to both diminished production and accelerated catabolism, by rapid body growth and need for commensurate increase in red cell volume/mass, and by disorders causing RBC losses due to bleeding and/or hemolysis. RBC transfusions are the ma
