The normal prenatal development of the human pituitary gland and the gland-supporting sella turcica has recently been investigated. OBJECTIVE: To study the clinical course and perinatal outcome of twin pregnancies discordant for anencephaly, without selective termination.
METHODS: We conducted a descriptive retrospective study of 14 cases of dichorionic twin pregnancies discordant for anencephaly, which were managed expectantly in five Israeli perinatal centers.

The rare case of a severe amniotic band syndrome associated with anencephaly, hypoplasia of adrenal glands and limb deficiencies is presented and discussed with regard to its probable pathogenesis. Impressive photo- and sonographic material can be shown. Early rupture of amnion seems to be the most probable explanation for this combination of anomalies.

Serum and hair zinc concentrations were measured in samples taken from 29 mothers with neural tube defects (NTD) (mainly anencephaly) at delivery. The control group consisted of 20 healthy mothers with normal offspring and 40 nonpregnant women 18 to 34 years of age, from middle income backgrounds. Quantitative studies of pulmonary endocrine cells in 8 male autopsy cases of anencephaly were performed. The densities of pulmonary endocrine cells were expressed as the number of the total argyrophil cells/100 bronchiolar cell nuclei and as the number of the total argyrophil cells/mm bronchiole. Keratinous material, in the form of epithelial squames often with foreign body reaction and dense gliosis, was demonstrated in all of ten cases of anencephaly by a sensitive and specific immunohistochemical stain. In some cases keratin was not recognized on routine histological stains.

Cells were plated at confluent densities and incubated with 65ZnCl2 for 2, 6, and 24 hours, or at subconfluent densities for 1-7 days. Radioactivity and DNA were determined in whole cell sonicates. A histological microradiographical analysis was carried out on the cranial base removed at autopsy from three infants--two with microcephalia, one with anencephalia. The histological findings were compared with those from normal individuals of the same age. It was shown that a reduced size of the brain was accompanied by a change in the remodeling pattern of the internal as well as the external surface of the cranial base, leading to a flattening of the cranial base. The findings further suggest that the development of the brain influences the ossification of the chondrocranium.

We report the successful use of a new method described by Gosden and Brock (1977) in two cases of anencephaly; according to this method 'rapidly adhering cells' are identified as neural cells of a specific morphology. Although the original method described adherence of cells during the first 20 h, we were able to identify a considerable number of such cells after a delay of three days due to mailing of the amniotic fluid.

The marriage-to-conception interval in 151 pregnancies producing infants with anencephaly or spina bifida was not significantly different from that in 218 pregnancies resulting in normal infants. Significantly more miscarriages occurred before than after the birth of 285 infants with anencephaly or spina bifida, but in 133 controls no before-after difference was observed. These observations seem to favour the idea that miscarriages are a manifestation rather than a cause of anencephaly and spina bifida.

In two retrospective Liverpool surveys the domestic, family, dietary, and medical environments of the mothers of children affected by anencephaly or spina bifida, or both (ASB) were examined. None seemed to be of more than minor importance and the findings of others were therefore examined to see if they suggested a major factor.
