Work-related musculoskeletal complaints and ergonomic risk factors among Egyptian anesthesiologists: a cross-sectional study.

BACKGROUND: Anesthesiologists are vulnerable to work-related musculoskeletal disorders (WMSDs) due to sustained repetitive movements and awkward postures. This study aimed to assess the prevalence of WMSDs among anesthesiologists and to evaluate its association with ergonomic risk factors. METHODS: A convenience sample of 380 Egyptian anesthesiologists were invited to participate in this cross-sectional study through an electronic questionnaire. Data were collected from May to August 2022 and involved questions about participants' demographic, health, and work-related characteristics; the erg…

Use of opioids and opioid alternatives during general anesthesia: a pan-Canadian survey among anesthesiologists.

PURPOSE: While there is limited patient-centred evidence (i.e., evidence that is important for patients and end-users) to inform the use of pharmacologic opioid minimization strategies (i.e., the use of opioid alternatives) for adult surgical patients requiring general anesthesia, such strategies are increasingly being adopted into practice. Our objectives were to describe anesthesiologists' beliefs regarding intraoperative opioid minimizing strategies use and utility, and to explore important clinical decision-making factors. METHODS: We conducted a pan-Canadian web-based survey of anesthesi…

A Rare Case of Kostmann Syndrome Presenting Difficult Airway Challenges and Patient Preparedness for Anesthesiologists.

Severe congenital neutropenia (SCN), commonly known as the Kostmann syndrome, is a rare and complex set of disorders defined by a lack of ne … extension, and gynecomastia. These additional conditions dramatically increased the challenges for anesthesiologists to address the anticipated difficult airway. The anticipated difficult airway challenges were handled by following the protocols of difficult airway guidelines 2022.

A medicolegal analysis of malpractice claims involving anesthesiologists in the gastrointestinal endoscopy suite (2007-2016).

STUDY OBJECTIVE: Gastrointestinal endoscopy cases make up the largest portion of out of operating room malpractice claims involving anesthesiologists. To date, there has been no closed claims analysis specifically focusing on the claims from the endoscopy suite. We aim to identify associated case characteristics and contributing factors. DESIGN: Retrospective review of closed claims. SETTING: Multi-institutional setting of hospitals that submit data to the Controlled Risk Insurance Company (CRICO) Comparative Benchmarking System, a database representing approximately 30% of annual malpractic…

In the Aftermath: Attitudes of Anesthesiologists to Supportive Strategies After an Unexpected Intraoperative Patient Death.

BACKGROUND: Although most anesthesiologists will have 1 catastrophic perioperative event or more during their careers, there has been little research on their attitudes to assistive strategies after the event. There are wide-ranging emotional consequences for anesthesiologists involved in an unexpected intraoperative patient death, particularly if the anesthesiologist made an error. We used a between-groups survey study design to ask whether there are different attitudes to assistive strategies when a hypothetical patient death is caused by a drug error versus not caused by an error. First, we…

Practice guidelines for perioperative transesophageal echocardiography: recommendations of the Indian association of cardiovascular thoracic anesthesiologists.

Transoesophageal Echocardiography (TEE) is now an integral part of practice of cardiac anaesthesiology. Advances in instrumentation and the information that can be obtained from the TEE examination has proceeded at a breath-taking pace since the introduction of this technology in the early 1980s. Recognizing the importance of TEE in the management of surgical patients, the American Societies of Anesthesiologists (ASA) and the Society of Cardiac Anesthesiologists, USA (SCA) published practice guidelines for the clinical application of perioperative TEE in 1996. On a similar pattern, Indian Asso…

Impact of high-intensity work on the physical and mental health of anesthesiologists and intervention strategies.

With the ongoing transformation of the healthcare landscape, anesthesiologists are experiencing increasing occupational stress, which profoundly affects their physical and mental health. A high-intensity work environment adversely affects the physiological health of anesthesiologists, leading to issues such as cardiovascular diseases and sleep disorders. Furthermore, it also leads to an increased risk of mental health problems such as anxiety, depression, and burnout. This article aims to review the current state of research in this field and analyze the characteristics of anesthesiologists' w…

Drug-laced vaping-implications and emerging perioperative challenges for the anesthesiologist: a narrative review.

PURPOSE: Drug-laced vaping (DLV) involves vaping devices used to inhale non-nicotine psychoactive substances (e.g., cannabinoids, stimulants, opioids, benzodiazepines [BZDs]), sometimes without the user's awareness. Because exposure is often undisclosed and may not be detected by routine toxicology screening, DLV poses a hidden perioperative risk. We aimed to summarize current evidence, highlight emerging challenges, and outline implications for anesthesiologists. SOURCE: We performed a structured narrative review of MEDLINE, Embase®, Scopus, and the Web of Science™ (inception to November 202…

Investigation of the attitudes and behaviors of anesthesiologists regarding the prophylaxis and treatment of postoperative nausea and vomiting.

BACKGROUND: Postoperative nausea and vomiting is a prevalent complication during the postoperative phase. Numerous factors affect postoperative nausea and vomiting, which can be categorized into patient-related, anesthesia-related, and surgery-related factors. The management of postoperative nausea and vomiting is multifaceted and consists of both prophylactic and therapeutic approaches. OBJECTIVES: This study aimed to investigate the attitudes and behaviors of anesthesiologists regarding the prophylaxis and treatment of postoperative nausea and vomiting, while also raising awareness of curre…

Point-of-Care Ultrasound (POCUS) Exam Performed by an Anesthesiologist Changes Obstetric Management.

Point-of-care ultrasound (POCUS) exams have been shown to impact perioperative patient care. Involvement in antepartum obstetric patients has not been previously described. We report a case of a parturient with chronic hypertension in new-onset respiratory distress. The anesthesiologist performed cardiac and lung POCUS exams that identified new-onset systolic heart failure and contributed to her diagnosis of preeclampsia with severe features. These findings guided the initiation of inotropic therapy and diuresis. The patient ultimately required transfer to a center able to perform more advance

Beyond Clots: The Role of Cardiac Anesthesiologists in a Case of ECMO for Massive Pulmonary Embolism.

We present a case of a 35-year-old male who presented with sudden-onset severe chest pain and breathlessness. CTPA revealed massive bilateral pulmonary embolism. The patient underwent catheter-directed mechanical thrombectomy (CDMT) under general anesthesia, complicated by cardiogenic shock necessitating cardiopulmonary resuscitation (CPR). Post ROSC, venoarterial ECMO was initiated, and catheter-directed thrombolysis was performed. CDMT was reattempted but not fully successful, and the patient developed right heart failure with pulmonary hypertension. ECMO continued for 5 days and gradually w

Anesthesia management for patients with congenital epidermolysis bullosa: A challenge case report for the anesthesiologist.

INTRODUCTION AND IMPORTANCE: Congenital epidermolysis bullosa (EB) is a group of rare genetic conditions characterized by fragile skin that blisters easily. Anaesthesia management in these patients is complex due to mucocutaneous fragility, which can complicate surgical procedures and airway management. Our study aimed to report a case of successful general anaesthesia in two patients with congenital epidermolysis bullosa undergoing oesophagal dilatation. CASE PRESENTATION: The case involved a brother and sister, aged 21 and 15 respectively, both of whom have congenital epidermolysis bullosa

[Fetal and pediatric cardiovascular physiology : Things you should know as an (pediatric) anesthesiologist].

Immediately after birth the physiology of the cardiovascular system of the neonate undergoes some significant changes. The first breaths in life and the inflation of the lungs lead to a considerable drop in pulmonary arterial resistance. This results in the closure of the foramen ovale and ductus arteriosus; however, during the first weeks of life a sharp rise in pulmonary vascular resistance caused by hypoxia, hypercapnia and excessive positive pressure ventilation can lead to the reopening of the ductus arteriosus. This may result in subsequent strain of the left heart. In order to anticipat

Association of Anesthesiologist Handovers With Short-term Outcomes for Patients Undergoing Cardiac Surgery.

BACKGROUND: Complete handover of anesthesia care to a second anesthesiologist has been demonstrated to be associated with worse short-term adverse outcomes among cardiac surgery patients, but little information from multi-institutional studies is available. METHODS: New York's cardiac surgery registry was used to identify patients who underwent cardiac surgery in New York between 2010 and 2016 with and without complete handovers of anesthesia care. A retrospective observational study with inverse probability treatment weighting (IPTW) based on the propensity score was used to adjust for diffe

Impact of the Addition of Examples to the American Society of Anesthesiologists Physical Status Classification System.

Examples of comorbidities for the widely used American Society of Anesthesiologists physical status (ASA-PS) classification system were developed and approved in 2014. We conducted a retrospective cohort study of patients with 4 comorbidities included in the examples as warranting a specific minimum ASA-PS class. For each comorbidity subgroup, we used interrupted time-series models to compare ASA-PS underclassification for the periods before (2011-2014) and after (2015-2017) the introduction of examples. Rates of underclassification ranged from 4.8% to 38.7%. We observed no evidence of a signi…

An Anesthesiologist's Perspective on the History of Basic Airway Management: The "Modern" Era, 1960 to Present.

This fourth and last installment of my history of basic airway management discusses the current (i.e., "modern") era of anesthesia and resuscitation, from 1960 to the present. These years were notable for the implementation of intermittent positive pressure ventilation inside and outside the operating room. Basic airway management in cardiopulmonary resuscitation (i.e., expired air ventilation) was de-emphasized, as the "A-B-C" (airway-breathing-circulation) protocol was replaced with the "C-A-B" (circulation-airway-breathing) intervention sequence. Basic airway management in the operating roo

Facilitating noncardiac surgery for the patient with left ventricular assist device: A guide for the anesthesiologist.

The introduction of left ventricular assist device (LVAD) has improved survival rates for patients with end-stage heart failure. Two categories of VADs exist: one generates pulsatile flow and the other produces nonpulsatile continuous flow. Survival is better for patients with continuous-flow LVADs. With improved survival, more of such patients now present for noncardiac surgery (NCS). This review, written for the general anesthesiologists, addresses the perioperative considerations when the patient undergoes NCS. For best outcomes, a multidisciplinary approach is essential in perioperative ma…

A primer on nerve agents: what the emergency responder, anesthesiologist, and intensivist needs to know.

PURPOSE: The purpose of this review article is to familiarize first responders, anesthesiologists, and intensivists with the medical management of patients exposed to nerve agents. SOURCE: This review is based on the current medical literature available to the general medical community. PRINCIPAL FINDINGS: Nerve agents are some of the deadliest substances known to humanity. Though they kill primarily via muscle paralysis, which leads to respiratory arrest, these agents affect virtually every organ system in the body. Their primary mechanism of action is the body-wide inhibition of cholineste…
