The use of specific antidotes as a response to bleeding complications during anticoagulant therapy for venous thromboembolism.

When a bleeding complication occurs during therapy with heparin or vitamin K antagonists, there is an option to give a specific antidote. Several new anticoagulants have been developed that are likely to have some risk of bleeding complications, for which no specific antidotes are available. Interestingly, it is unknown how often the use of an antidote is necessary in clinical practice. We investigated 1877 patients treated for venous thromboembolism included in three large clinical trials, of which 181 (9.6%) had a total of 225 adjudicated bleeding episodes; 46 hemorrhages being designated as…

Effects of cyanide antidotes used with sodium nitroprusside infusions: sodium thiosulphate and hydroxocobalamin given prophylactically to dogs.

Cyanide antidotes were given to dogs before an infusion of sodium nitroprusside 1.5 mg kg-1 for 1 h. Dogs given thiosulphate 75 mg kg-1 had significantly lower plasma and red cell cyanide concentrations while plasma thiocyanate concentrations were significantly increased in comparison with control. These changes were associated with only minimal disturbance of tissue oxygenation. There was no effect on red cell cyanide or thiocyanate concentrations in dogs treated with hydroxocobalamin 1.5 mg kg-1, but plasma cyanide concentrations were significantly greater than in those receiving no antidote…

'This noble method in physic': Peter Shaw's proposal for a controlled trial of an 'antidote' for rabies (1723).

IntroductionPeter Shaw (1694-1763) was an English physician who practised in London and Scarborough, Yorkshire. He was appointed physician to Kings George II and III. Shaw published many medical texts as well as translations of Lord Francis Bacon and Robert Boyle.MethodHistorical examination of Shaw's text A Treatise of Incurable Diseases (1723).ResultsShaw states that 'incurable' conditions offer an opportunity to examine the cause of disease and its treatment. He proposes a model for a comparative trial of 'antidote' for hydrophobia (rabies) in one of two dogs and - if successful - in humans

Tiger nut: Antidote for alcohol-induced testicular toxicity in male Sprague-Dawley rats.

OBJECTIVE: Studies have implicated alcohol consumption as a factor leading to male infertility. Tiger nuts (Cyperus esculentus) on the other hand, have been shown to possess the potential to boost male reproductive indices. This study was carried out to investigate the effect of tiger nuts on alcohol-induced testicular toxicity in male Sprague-Dawley rats. METHODS: Thirty male Sprague-Dawley rats (160g averagely) were divided into six groups (A-F) (n=5). Group A (Control) received distilled water only; Group B (Tiger nut alone) received 1.8 g/kg body weight (BW) tiger nut; Groups C-F received

Intramuscular sodium tetrathionate as an antidote in a clinically relevant swine model of acute cyanide toxicity.

Background: Cyanide is a metabolic poison used in multiple industries and is a high threat chemical agent. Current antidotes require intravenous administration, limiting their usefulness in a mass casualty scenario. Sodium tetrathionate reacts directly with cyanide yielding thiosulfate and the non-toxic compound thiocyanate. Thiosulfate, in turn, neutralizes a second molecule of cyanide, thus, per mole, sodium tetrathionate neutralizes two moles of cyanide. Historical studies examined its efficacy as a cyanide antidote, but it has not been evaluated in a clinically relevant, large animal model…

Cinnamon (Cinnamomum zeylanicum) as an antidote or a protective agent against natural or chemical toxicities: a review.

Cinnamon (Cinnamomum zeylanicum, Lauraceae) is a food additive greatly used for its taste. However, recently this medicinal plant has been brought to attention due to its medical effects. Cinnamon has constituents such as cinnamaldehyde and cinnamic acid that offers some health benefits including antioxidant and free-radical scavenging properties, lowering of blood glucose, anti-cholesterolemic, analgesic, antimicrobial, anti-inflammatory, anti-yeast, anti-secretagogue, and anti-gastric ulcer effects. This review summarizes various in vitro and animal studies on the protective effects of cinna

[Combination of a universal antidote and temporary skin substitute for chemical burns: Extended case report].

In this article we describe our experiences in the treatment of chemical burns with Diphoterine(®) solution and Suprathel(®) as a temporary skin substitute material, a treatment which in the past was not commonly used for this pattern of injuries. In the study period from October 2012 to December 2013 we treated five patients (four male and one female including two children and three adults) with chemical burns by decontamination with Diphoterine(®) and wound covering with Suprathel(®). The control group included five patients with similar injury patterns who were treated with Diphoterine(®) a

Legalon® SIL: the antidote of choice in patients with acute hepatotoxicity from amatoxin poisoning.

More than 90% of all fatal mushroom poisonings worldwide are due to amatoxin containing species that grow abundantly in Europe, South Asia, and the Indian subcontinent. Many cases have also been reported in North America. Initial symptoms of abdominal cramps, vomiting, and a severe cholera-like diarrhea generally do not manifest until at least six to eight hours following ingestion and can be followed by renal and hepatic failure. Outcomes range from complete recovery to fulminant organ failure and death which can sometimes be averted by liver transplant. There are no controlled clinical studi

Additive effect of DL-penicillamine plus Prussian blue for the antidotal treatment of thallotoxicosis in rats.

DL-penicillamine (DL-P) and Prussian blue (PB) given alone or in combination were tested as possible treatments against acute thallium toxicity. Rats were intoxicated by i.p. injection of thallium (I) acetate at LD(50) (32 mg/kg). A day later, pharmacological treatment was administered until day 4 as follows: (1) vehicles, (2) PB 50mg/kg, by oral route, twice a day, (3) DL-P 25mg/kg i.p. route, twice daily and (4) PB+DL-P. The Estimated Probability Survival (EPS) was recorded during the experiment for each treatment. DL-P alone did not show a significant effect on survival. However, when it wa

[Antidote against local anaesthetic intoxication: new use of lipid emulsion for intravenous administration].

Local anaesthetics are routinely used for several indications, but despite local administration their use may lead to systemic toxicity. The symptoms include numbness of the tongue, dizziness, tinnitus, visual disturbances, muscle spasms, convulsions, coma, and respiratory and cardiac arrest. Recently, an intravenous lipid emulsion was reported to act as a novel potential antidote for systemic toxicity due to local anaesthetics. We describe the application of this lipid emulsion in a 27-year-old patient with generalized seizures and coma due to local anaesthetic toxicity. She recovered quickly

[Histamine blockers increase efficacy of specific antidote prophylaxis of intoxication by cholinesterase inhibitors in mice].

Dimedrol and pipolphen, but not loratadine, increase efficacy of the atropine and dipiroxime treatment of mice--an antidote prophylaxis against their intoxication with phosphacol and aminostigmine. The results of experiments confirm a hypothesis that histamine participates in the formation of secondary toxicity reactions in the case of heavy poisoning with anticholinesterase agents.

Antidotal effect of glucoside extracted from Astragalus membranaceus on dimethoate intoxication in guinea pigs.

Therapeutic effect of the glucoside extracted from the root of Astragalus Membranaceus (AM) was studied in guinea pigs intoxicated with 600 mg/kg doses of Dimethoate (D). Four groups of guinea pigs, each consisting of two males and two females, were treated with (a) D, (b) D + Atropine, and (c) D + AM, (d) D + Atropine + AM. The survival time of the animals increased from an average of 70 minutes without AM to an average of 235 minutes with AM. Severe changes in ECG were observed prior to respiratory distress in groups without AM, and Atropine did not modify such changes. Differently, severe E

Antidotal use of methemoglobin forming cyanide antagonists in concurrent carbon monoxide/cyanide intoxication.

An estimated 35% of all fire victims in the United States have toxicologically significant blood levels of CO and CN. However, the treatment of concurrent CO/CN intoxication has been paid scant attention. The suggestion has been made that these victims should be treated for CN poisoning. The current therapeutic management of CN poisoning in this country includes the utilization of two methemoglobin formers: amyl nitrite and sodium nitrite. This study was undertaken to determine if the administration of methemoglobin formers is advisable, as the victim is already suffering from O2 deprivation d
