Many pediatric patients requiring anticoagulation during interventional heart catheterizations have antithrombin (AT) deficiency. AT is a necessary cofactor for heparin inhibition of thrombin. Without sufficient AT, these patients are at increased risk for thrombotic complications. This case describes the successful use of bivalirudin, a direct thrombin inhibitor, as an anticoagulant in a 2-month old infant with AT deficiency undergoing stent placement.

It is possible that women with triplet pregnancies are more likely to exhibit pregnancy-induced antithrombin deficiency, gestational thrombocytopenia, and perinatal elevation in serum aspartate aminotransferase (AST) than women with twin pregnancies. We retrospectively reviewed changes in antithrombin activity, platelet count, and blood chemistry in 23 twin and seven triplet pregnancies in which the mothers received antenatal care and gave birth in our hospital during 1999 and 2001. Both antithrombin activity and platelet counts gradually decreased until delivery, then promptly increased after

UNLABELLED: Intrauterine growth retardation (IUGR) and prematurity are often correlated with higher mortality and morbidity in the first days of life especially due to complications such as: hypoglycemia, polycythemia, necrotizing enterocolitis, meconium aspiration syndrome. Disturbances in the haemostatic system could be responsible for poor outcome of these complications.
AIM: To determine the activity of main inhibitor of coagulation-antithrombin, level of protein C, concentration of thrombin-antithrombin (TAT) complexes and fibrinogen in the blood of premature infants with intrauterine gr

Hereditary and acquired deficiencies of protein C, protein S, and antithrombin III are important risk factors of thrombosis, especially in peripheral veins. In a prospective study, concentrations of these factors were measured to determine the prevalence of deficiencies of these proteins in ocular vascular occlusive disease. A total of 167 patients with acute retinal arterial (28%) or venous occlusion (55%) or ischemic optic neuropathy (17%) were included. Exclusion criteria were anticoagulant therapy, renal insufficiency, and hepatic disease. The mean values obtained for all patients were in

To evaluate the occurrence of hypercoagulability during treatment with L-asparaginase (L-ase), thrombin-antithrombin complex (TAT) and D-dimer levels in plasma were serially measured in 15 consecutive adult patients with acute lymphoblastic leukaemia or lymphoblastic lymphoma who had recently completed a chemotherapy cycle with cytosine arabinoside and methotrexate. The first eight patients (group A) received i.v. L-ase alone (20,000 U/m2 on alternate days over 10 d); the last seven patients (group B) received, in addition to L-ase, bolus injection of antithrombin concentrate (2000 U) on alter

A 43-year-old man presented a pulmonary embolism. The unusual circumstances of apparition, the age and the increased heparin requirements suggested an antithrombin III (AT III) deficiency. AT III activity was low in the propositus and seven other members of his family (mean 55%), but immunologic levels were normal (mean 110%). Crossed immunoelectrophoresis in absence of heparin showed a normal pattern, but in presence of heparin showed an abnormal peak as compared with controls. Kinetics experiments showed a normal inhibition of thrombin and Xa in absence of heparin, but abnormal in presence o

Severe pulmonary embolism with thrombosis of the inferior vena cava was observed in a 16 year old girl with no risk factors and treated successfully by fibrinolytic therapy. Secondarily, despite heparino-therapy, upper limb venous thrombosis occurred. Investigation of the clotting factors in the patient and her family revealed a hereditary deficit of antithrombin III. The features of the haemotological diagnosis of this rare condition and the therapeutic implications are discussed.

At III levels measured with six methods in 36 people: 10 healthy controls, 10 women taking a progestagen Lynestrenol and 16 women taking a combined oestrogen-progestagen contraceptive pill. The reproducibility and the sensitivity of these methods as well as the correlation between methods were studied. The Hensen and Loeliger technique had a poor reproducibility and the results obtained with the Howie technique were not in good correlation with those obtained with other methods, especially in the group taking oral contraceptives. The two amidolytic methods using chromogenic substrates were fou

BACKGROUND: Heart transplant recipients often face the clinical dilemma of coexisting bleeding and thrombotic risks postoperatively. Conventional anticoagulation monitoring parameters (e.g., platelet count, fibrinogen, D‑dimer, and anti‑Xa activity) frequently exhibit limitations in evaluating complex postoperative patients, including delayed response, poor sensitivity, and lack of clear target ranges. This case report describes the use of thrombin-antithrombin complex (TAT) to guide anticoagulation management for complex thrombotic events after heart transplantation and explores its potential

Marine sulfated polysaccharides (SPs) are promising candidates for new anticoagulant drug development, necessitating efficient method to evaluate their anticoagulant activity at the molecular level. In this work, a heparin microarray-based competitive strategy was developed to investigate the specific interactions between SPs and antithrombin Ⅲ (AT). The strategy is based on the principle that SPs competitively bind to the active domain of AT, reducing the signal of AT binding to immobilized heparin on microarrays. Validation was performed using established anticoagulants heparin and enoxapari

The pathogenesis of hyperuricemic nephropathy (HN) is increasingly linked to a self-perpetuating cycle of thromboinflammation. This process begins with hyperuricemia-induced endothelial dysfunction, which activates both the coagulation cascade and inflammatory pathways, notably the pyrin domain-containing protein 3 inflammasome. This creates a vicious cycle of which inflammation promotes coagulation and coagulation products further amplify inflammation, ultimately leading to renal microvascular thrombosis, ischemia, and progressive fibrosis. This review critically examines the therapeutic pote

INTRODUCTION: We identified a novel variant of the antithrombin (AT) gene (SERPINC1), c.96 T>G, p.Cys32Trp (C32W), located at the signal peptide cleavage site in a patient with congenital AT deficiency. The impact of signal peptide variants on the intracellular trafficking and secretion of AT proteins has not been previously studied. Thus, we analyzed the intracellular dynamics and signal peptide cleavage of the C32W variant of AT (AT-C32W).
MATERIALS AND METHODS: Wild-type AT (AT-WT) and AT-C32W expression vectors were transfected into HEK293 cells. Functional analyses were performed using w

Hemophilia is a coagulation disorder caused by deficiencies in clotting factors and is primarily classified into hemophilia A (factor VIII deficiency) and hemophilia B (factor IX deficiency). Current treatment relies predominantly on replacement therapy, where patients receive clotting factor concentrates either episodically or prophylactically to achieve hemostasis or mitigate bleeding risks. Despite its efficacy, this approach presents limitations, including suboptimal treatment accessibility due to high costs, reduced patient compliance from frequent intravenous administration, and therapeu

BACKGROUND: Antithrombin deficiency (ATD) in pregnant patients significantly increases the risk of venous thromboembolism (VTE), but guidelines for managing anticoagulation during pregnancy, labour, and postpartum in patients with ATD are limited.
CASE PRESENTATION: A pregnant woman with ATD suffered recurrent VTE in the 20th week of pregnancy despite therapeutic doses of low-molecular-weight heparin (LMWH). The acute VTE was treated with argatroban and then with warfarin until delivery. LMWH with antithrombin (AT) concentrate was introduced before and shortly after delivery, followed by warf

3-O-sulfation of heparan sulfate (HS) is the key determinant for binding and activation of antithrombin III (AT). This interaction is the basis of heparin treatment to prevent thrombotic events and excess coagulation. Antithrombin-binding HS (HSAT) is expressed in human tissues but is thought to be expressed in the subendothelial space, mast cells, and follicular fluid. Here, we show that HSAT is ubiquitously expressed in the basement membranes of epithelial cells in multiple tissues. In the pancreas, HSAT is expressed by healthy ductal cells, and its expression is increased in premalignant pa

Deficiencies in coagulation factor VIII (FVIII, F8) result in the bleeding disorder hemophilia A. An emerging novel therapeutic strategy for bleeding disorders is to enhance hemostasis by limiting natural anticoagulants, such as antithrombin (AT3). To study pro/anticoagulant hemostatic balance in an in vivo model, we used genome editing to create null alleles for f8 and von Willebrand factor (vwf) in zebrafish, a model organism with a high degree of homology to the mammalian hemostatic system and unique attributes, including external development and optical transparency. f8 homozygous mutant l

Extracorporeal membrane oxygenation (ECMO) support of critically ill pediatric patients is associated with increased risk of thromboembolic events, and unfractionated heparin is used commonly for anticoagulation. Given reports of acquired antithrombin (AT) deficiency in this patient population and associated concern for heparin resistance, AT activity measurement and off-label AT replacement have become common in pediatric ECMO centers despite limited optimal dosing regimens. We conducted a retrospective cohort study of pediatric ECMO patients (0 to <18 years) at a single academic center to ch
