BACKGROUND: A major contributor to postkeratoplasty astigmatism may be donor/recipient disparity. Deficient or excess cornea at the wound is thought to influence the directions of the steep and flat meridians. Seven eyes of six patients with anterior polar congenital cataract were examined. All patients presented with corneal astigmatism greater than 1.50 diopters. The combination of these two anomalies has often been responsible for amblyopia and occasionally for squint. The association of corneal astigmatism with this type of congenital cataract is possibly due to a delayed separation of the lens from the surface ectoderm during fetal development and thus may be considered a new syndrome.

High degrees of surgically induced astigmatism following cataract surgery may limit the desired visual rehabilitation. The induced astigmatism is a reflection of the cataract incision, its closure, and subsequent wound healing. Excessively tight sutures induce central corneal steepening. To find out if postoperative astigmatism decays less if Mersilene rather than nylon sutures are used, 25 patients had a standard wound closure with 10-0 nylon in one eye and 11-0 Mersilene in the other. There was no difference in the astigmatism decay over a 6-month period.
