Novel technique for resolving a stuck rotational atherectomy device in a peripheral artery: a case report.

Rotational atherectomy device entrapment is a rare but challenging complication of peripheral vascular interventions. This case report details a novel dual-access endovascular technique for retrieving an entrapped atherectomy burr from the posterior tibial artery of a 70-year-old man with severe peripheral arterial and chronic kidney disease. When conventional retrieval methods failed, retrograde posterior tibial access was established as an adjunct to the existing femoral access, enabling sequential balloon angioplasty with progressively larger balloons (1.5-2.5 mm) around the entrapped devic…

Practice patterns in utilization of atherectomy and embolic protection devices in inpatient and outpatient treatment settings.

OBJECTIVE: The frequency of atherectomy in lower extremity arterial disease has increased substantially over the past several years, specifically in the office-based laboratory (OBL) setting, yet the efficacy compared with other interventions and the consequences of distal embolization remain unknown. Embolic protection devices (EPDs) have been used at varying rates depending on physician and practice setting. Previous studies have described lesion characteristics to consider when weighing the benefits and drawbacks associated with device use. Our study focuses on the use of atherectomy and EP…

National trends and variability of atherectomy use for peripheral vascular interventions from 2010 to 2019.

OBJECTIVE: Small, older studies have suggested that the use of atherectomy devices has become common in peripheral vascular interventions (PVIs) despite the paucity of strong clinical guidelines. We analyzed the 10-year trends in the use of atherectomy for PVIs across the United States and identified the main predictors of atherectomy use. METHODS: Using the Vascular Quality Initiative registry, we identified all patients who had undergone endovascular PVIs for occlusive lower extremity arterial disease from 2010 to 2019. Procedures in which an atherectomy device had been used as the primary …

Impact of Angioscopic Evaluation for Femoropopliteal In-Stent Restenosis Before and After Excimer Laser Atherectomy.

In-stent restenosis (ISR) is a prevalent problem following stenting of femoropopliteal lesions. A potential novel treatment modality for ISR including excimer laser atherectomy (ELA) has become available. We performed ELA for in-stent chronic total occlusion (CTO) of femoropopliteal lesions and evaluated lesion morphology before and after ELA by angioscopy in 2 patients. The angioscopic findings clearly showed removal of in-stent thrombi after ELA. Thus, ELA may be effective for in-stent CTO of femoropopliteal lesions. This is the first report describing the direct visualization of ELA effect …

Incidence of Bradycardia and Outcomes of Patients Who Underwent Orbital Atherectomy Without a Temporary Pacemaker.

OBJECTIVE: We analyzed the incidence of bradycardia and the safety of patients with severely calcified coronary lesions who underwent orbital atherectomy without the insertion of a temporary pacemaker. BACKGROUND: The presence of severely calcified coronary lesions can increase the complexity of percutaneous coronary intervention due to the difficulty in advancing and optimally expanding the stent. High-pressure inflations to predilate calcified lesions may cause angiographic complications like perforation and dissection. Suboptimal stent expansion is associated with stent thrombosis and rest…

Alterations of Blood Flow Through Arteries Following Atherectomy and the Impact on Pressure Variation and Velocity.

Simulations were made of the pressure and velocity fields throughout an artery before and after removal of plaque using orbital atherectomy plus adjunctive balloon angioplasty or stenting. The calculations were carried out with an unsteady computational fluid dynamic solver that allows the fluid to naturally transition to turbulence. The results of the atherectomy procedure leads to an increased flow through the stenotic zone with a coincident decrease in pressure drop across the stenosis. The measured effect of atherectomy and adjunctive treatment showed decrease the systolic pressure drop by…

Successful Use of Orbital Atherectomy as an Adjunct in Treating Extensively Calcified Mesenteric Artery Lesions.

BACKGROUND: To describe the use of orbital technique of atherectomy as an adjunct to successful angioplasty and stent placement of the superior mesenteric artery (SMA). CASE REPORT: The technique is demonstrated in a 68-year-old man with critical SMA stenosis. The SMA was cannulated with 0.014-in wire, but the lesion was highly stenotic and densely calcified and prevented the passage of even the smallest 1.5-mm balloon. Orbital atherectomy was thus performed with a 1.25-mm CSI crown. Balloon angioplasty was then possible with a 4 mm × 2 cm balloon followed by placement of a 7 mm × 22 mm ballo…

A novel approach using atherectomy for chronic total occlusion of the brachial artery: a case report.

Chronic upper limb ischemia is uncommon, and is most often secondary to subclavian artery stenosis. Percutaneous intervention for subclavian stenosis usually includes balloon angioplasty and stent implantation. Brachial artery stenosis is even more scarce and accounts for approximately 12% of symptomatic upper extremity ischemia. Because it is rare, the optimal treatment strategy for brachial artery stenosis remains uncertain. We describe a case of chronic brachial artery ischemia due to atherosclerotic disease treated with the SilverHawk plaque excision system and review the available data re

Atherectomy with Angioplasty versus Angioplasty Alone for Critical Limb-Threatening Ischemia with Isolated Tibial Disease.

BACKGROUND: Critical limb-threatening ischemia (CLTI) causes significant morbidity and mortality worldwide. Although simple balloon angioplasty has been around for decades, the use of atherectomy devices for CLTI treatment has expanded exponentially in recent years. There remains a gap in research on the comparative outcomes of adjunct atherectomy versus stand-alone angioplasty in patients with critical limb ischemia, particularly for patients with isolated tibial disease. METHODS: Using TriNetX Data Network, a global federated database of over 250 million patients, we conducted a retrospecti…

A Novel Guidewire Bias Modification Method for Atherectomy of Severely Calcified Femoropopliteal Lesion: JET BIAS.

BACKGROUND: The Jetstream atherectomy catheter is an effective option for treating severely calcified femoropopliteal lesions. However, unfavorable guidewire bias may result in inadequate plaque debulking. CASE SUMMARY: A 55-year-old man with a severely calcified popliteal artery lesion underwent endovascular treatment. Initial atherectomy failed to achieve sufficient luminal gain because of unfavorable guidewire bias. During the second procedure, intentional modification of the guidewire bias using our novel technique, JETstream using the Bidirectional and Intentional Atherectomy System (JET…

Procedural and Clinical Outcomes After Orbital Atherectomy Versus Intravascular Lithotripsy in Patients With Calcified Nodules.

BACKGROUND: Calcified nodules (CNs) remain a major challenge in percutaneous coronary intervention (PCI). We sought to compare procedural and clinical outcomes after orbital atherectomy (OA) versus intravascular lithotripsy (IVL)-facilitated PCI in patients with CNs. METHODS: We identified patients with optical coherence tomography (OCT)-defined CNs who underwent PCI with either OA or IVL between 2012 and 2022 and had both pre- and post-PCI OCT images available. The imaging end point was the minimal stent area on final post-PCI OCT. The clinical end point was 2-year target-lesion failure, a c…

Luminal Gain after Atherectomy Measured by Intravascular Ultrasound (IVUS) and Impact on Mid-Term Results.

BACKGROUND: While atherectomy is commonly used in peripheral vascular interventions, multiplanar data regarding the degree of luminal gain from the atherectomy procedure itself is lacking. We assessed immediate percent luminal gain, measured by intravascular ultrasound (IVUS) in different atherectomy devices. We also evaluated short-term patency measured by duplex ultrasound (DUS) and mid-term clinical follow-up. METHODS: We prospectively collected data on 156 consecutive lower extremity revascularizations for 145 unique patients (11 bilateral staged interventions). Atherectomy was always fol…
