Ablation strategies for atrial fibrillation.

Atrial fibrillation remains the most common arrhythmia in the USA and is associated with an increased risk for stroke, congestive heart failure and overall mortality. There has been a tremendous advance in the field of catheter ablation of atrial fibrillation that has resulted in better outcomes for patients. The approach for ablation of atrial fibrillation can be different depending on patients' presentation of paroxysmal or persistent atrial fibrillation. Pulmonary vein isolation remains the cornerstone of any ablation strategy for atrial fibrillation; however, further ablation, end points o…

Atrial fibrillation in primary aldosteronism.

INTRODUCTION: Primary aldosteronism (PA) is caused by autonomous hypersecretion of aldosterone from the adrenal cortex, classically from an … ay also be cardiotoxic. PATIENTS: In this report, we describe five patients with PA, who developed atrial fibrillation (AF) in the absence of structural cardiac lesions and in one case despite good control of blood pressure and electrolytes. CONCLUSION: In patients with hypertension and AF, who have no evidence of coronary disease or any other underlying cause of A …

Atrial fibrillation and Brugada syndrome.

Brugada syndrome is characterized by right bundle branch block pattern with ST-segment elevation in leads V(1) to V(3) and a propensity for sudden cardiac death due to ventricular arrhythmias. The arrhythmogenic substrate in Brugada syndrome may not be restricted to the ventricles, and atrial arrhythmias are being increasingly reported. Incidences of spontaneous atrial arrhythmias vary from 6% to 38% and those of inducible atrial arrhythmias from 3% to 100%. Atrial fibrillation (AF) is the most common atrial arrhythmia found in Brugada syndrome. Enhanced duration of atrial action potential and…

Atrial fibrillation from thyroid storm.

BACKGROUND: Thyroid storm is an often-discussed but rare presentation to emergency departments (EDs). The clinical presentation of a thyroid storm is the result of a hyperthyroid state that may result in significant morbidity or disability, or even death. Typically, patients are aware of their hyperthyroid condition, and may be able to recognize an episode of thyroid storm. However, the first presentation of hyperthyroidism could, in fact, be from thyrotoxic crisis. OBJECTIVES: To review the presentation of thyroid storm, including tachycardia, hyperpyrexia, agitation, and altered mental stat

Atrial fibrillation and pulmonary embolism.

Atrial fibrillation is a rare diagnosis in children and adolescents. We report a case of a 17-year-old previously healthy boy who presented to the emergency department with a 3-week history of shortness of breath, chest pain, and 1 episode of hemoptysis. Symptoms persisted despite a course of antibiotics and anti-inflammatory agents. The patient developed palpitations and was found to be in atrial fibrillation at the time of hospitalization. An extensive workup led to the diagnosis of pulmonary embolism. This case highlights the importance of evaluating for thromboembolic events in pediatric p…

[Atrial fibrillation: current therapeutic options].

The treatment of atrial fibrillation still remains challenging in everyday practice. Even after the introduction of catheter ablation, the decision making about the type of therapy became more complex. The recently published guidelines of the American Heart Association, the American College of Cardiology, and the European Society of Cardiology clearly show therapeutic approaches for different types of atrial fibrillation. The fear of thromboembolism still forces the physician to restore sinus rhythms and perform a perfect anticoagulation. Based on large studies, we are able to decide for each …

[Catheter ablation of atrial fibrillation].

In patients with drug-refractory atrial fibrillation, left-atrial catheter ablation represents a new curative therapeutic option. Segmental ostial or circumferential pulmonary vein isolation can achieve stable sinus rhythm in some 70% of patients with paroxysmal atrial fibrillation but no severe structural heart disease. In patients with chronic atrial fibrillation, complex left-atrial linear, or substrate-oriented ablation strategies may additionally be applied. In patients with cardiac insufficiency or more severe systolic left-ventricular dysfunction, restoration of a stable sinus rhythm th…

Medical management of atrial fibrillation.

Atrial fibrillation (AF) is the most common sustained tachyarrhythmia encountered in clinical practice. The management of AF remains associated with many challenges that make its treatment a vexing problem. However, in recent years extraordinary progress has been made in the understanding and management of AF. In general, antiarrhythmic drugs for rhythm control have been disappointing. We have a better understanding of pharmacologic cardioversion and this has led to newer strategies, such as intermittent therapy. Several studies have been completed comparing pharmacologic rhythm control with p…

[Pharmacological treatment of atrial fibrillation].

Although anticoagulant treatment of atrial fibrillation is now well codified, the medical treatment of the fibrillation remains controversial. Two types of medication can be proposed: drugs to slow the rhythm (digitalis, betablockers and calcium inhibitors) and anti-arrhythmic mainly Class I or Class III drugs. Some doubt was raised in the 1990's about the pertinence of antiarrhythmic therapy and four recent trials (AFFIRM, RACE, PIAF and STAF) compared the two attitudes of "rhythm control" or "rate control" in atrial fibrillation. The four trials all showed that the results of these two optio…

Radiofrequency ablation of atrial fibrillation.

UNLABELLED: Twenty-five patients (16 males, mean age 46 years) underwent radiofrequency ablation because of either paroxysmal (13 patients) or persistent atrial fibrillation (12 patients). Ablation aimed at earliest activation of spontaneous and catheter-induced repetitive ectopy in left and right atria and appendages, and pulmonary veins. Catheter-induced repetitive ectopy was defined as acute onset of a burst of rapid atrial premature beats on touching the wall, sustained irritability while at the spot and acute termination of rapid activity upon release of the catheter. Post-ablation patien…

Atrial fibrillation and cardiac surgery.

PURPOSE OF REVIEW: To discuss the pathophysiology, risk factors, and treatments for atrial fibrillation occurring after cardiac surgery. RECENT FINDINGS: Atrial fibrillation occurs frequently after cardiac surgery and it may lead to patient morbidity. Many variables have been suggested to be associated with this arrhythmia, but only advanced patient age can consistently identify risk for this complication. Immediate electrical cardioversion is indicated when the arrhythmia leads to hemodynamic instability or myocardial ischemia. Otherwise treatment is aimed at heart rate control, elective car…

Atrial fibrillation and cardioembolic stroke.

The most disabling consequence of atrial fibrillation (AF) is stroke. In the elderly, AF is the single most important cause of stroke. The risk of stroke is increased at least 6-fold in subjects with AF. Strokes in patients with AF are in general severe, associated with higher risk of fatality and prone to early and long-term recurrence. The cardiac origin of stroke can be strongly suspected by anamnesis, clinical examination and findings on neuroimaging. Paroxysmal AF is an important cause of brain embolism, that is often difficult to document. Risk factors for stroke in AF include: previous …

[Radiofrequency ablation of atrial fibrillation].

The possibility of curing patients suffering from paroxysmal atrial fibrillation using a radiofrequency ablation treatment is a major change in the management of this arrhythmia. Pulmonary vein disconnection is efficient and safe after a learning curve of the operator. This pulmonary vein isolation is the first and mandatory step allowing disappearance of atrial fibrillation in 70% of the patients. Modification in fibrillatory substrate using linear lesions increases the rate success to 75% in chronic atrial fibrillation and to 82% in paroxysmal atrial fibrillation. The radiofrequency ablation…

Nicotine gum-induced atrial fibrillation.

A 39-year-old man with no prior history of atrial fibrillation was hospitalized with atrial fibrillation and a rapid ventricular rate. For the 7 months before presentation, he had been chewing nicotine polacrilex gum on his own. The week he first developed palpitations, he was chewing more than 1 piece of nicotine Polacrilex gum per hour during work. His diagnostic work-up during hospitalization found no cause for atrial fibrillation. He was cardioverted to sinus rhythm. At 6-month follow-up, he had not renewed chewing nicotine polacrilex gum, was in sinus rhythm, and had no history of palpita…

Etiological pattern of atrial fibrillation.

An attempt to find out the causes of atrial fibrillation was made in this study; although this does not represent total picture of whole population as the number of cases was limited and taken from a particular area for a limited period. Among the causes in our country, rheumatic mitral valvular disease topped the list followed by rheumatic multiple valvular disease IHD, HHD; lone atrial fibrillation came in the aetiology sequentially. This information is valuable in regard to management as rheumatic heart disease, the prime cause of atrial fibrillation in our country.

Radiofrequency Ablation for Atrial Fibrillation.

Catheter ablation has emerged as an excellent treatment option for atrial fibrillation. In appropriate patients, particularly those with paroxysmal atrial fibrillation in the absence of structural heart disease, catheter-based pulmonary vein isolation can eliminate or dramatically reduce the arrhythmia burden. However, with currently available tools, the procedure is complex and labor intensive. Simpler ablation procedures may emerge as newer ablation systems are developed.

[Ablative methods for atrial fibrillation].

Surgery and catheter-based ablation are the only curative approaches of atrial fibrillation. Since 1994, several ablative techniques based on 2 different concepts have been developed: modification of the substrate with long linear lesions to prevent fibrillation maintenance; and foci ablation to prevent fibrillation initiations. The latter is more frequently used now as the clustering of foci in pulmonary veins allows their isolation. The presence of numerous foci in some patients is responsible for more complex procedures and limits the success rate to 70%. In spite of this limit and because …

Subclinical hyperthyroidism and atrial fibrillation.

Subclinical hyperthyroidism is characterized by a clearly low serum concentration of thyrotropin (TSH) and the absence of obvious symptoms of hyperthyroidism. Whether or not all persons with a low value for serum TSH can be considered subclinically hyperthyroid is uncertain, but the low serum TSH per se is a risk factor for atrial fibrillation and perhaps other cardiovascular disease. Screening all persons, even in the older age groups, for subclinical hyperthyroidism may not be justified in and of itself but such persons would likely be screened for subclinical hypothyroidism and so those wit…
