We report severe organophosphate poisoning complicated by hypotension and ischemic sequelae in two patients with pre-existing vascular disease. Both patients had a low total peripheral resistance and high cardiac output that were significantly reversed by doses of atropine in excess of those required to control other muscarinic symptoms. Cerebral infarcts and gangrene requiring a below knee amputation were complications of the poisonings. In an attempt to estimate the in vivo acetylcholine level in rat iris-ciliary body, animals were sacrificed by microwave irradiation which rapidly inactivates metabolic enzymes. Acetylcholine levels in the iris-ciliary body were determined by high-performance liquid chromatography with electrochemical detection. We undertook a retrospective analysis of 35 victims of red scorpion (Mesobuthus tamulus) sting, admitted to our hospital in Mahad, Maharashtra State, India. Increase in duration as well as severity of clinical signs, including myocardial injury and poor response to conventional treatment, were observed in 17 victims treated with atropine compared to another 18 victims who were not so treated. We do not advocate routine use of atropine to block transient cholinergic effects of scorpion envenoming.

We studied the Oddi sphincter motility by endoscopic manometry in 10 consecutive patients randomized in a double-blind fashion, after i.v. administration of two anticholinergic compounds (0.5 mg atropine sulfate and 10 mg pirenzepine). Pirenzepine significantly decreased the basal sphincteric pressure, as well as the amplitude and frequency of the phasic contractions. Thirty infants with birth weights from 580 to 3450 gm (25 to 40 weeks gestation) were prospectively studied during nasotracheal intubation. Fifteen subjects with chronic bronchitis were treated with inhaled atropine sulfate, isoproterenol hydrochloride, or placebo in a double blind crossover trial.
