Sympathetic nerve blocks for persistent pain in adults with inoperable abdominopelvic cancer.

BACKGROUND: Persistent visceral pain is an unpleasant sensation coming from one or more organs within the body. Visceral pain is a common symptom in those with advanced cancer. Interventional procedures, such as neurolytic sympathetic nerve blocks, have been suggested as additional treatments that may play a part in optimising pain management for individuals with this condition. OBJECTIVES: To evaluate the benefits and harms of neurolytic sympathetic nerve blocks for persistent visceral pain in adults with inoperable abdominopelvic cancer compared to standard care or placebo and comparing sin

Effect of cervical sympathetic block on optic nerve sheath diameter measured by ultrasonography.

Optic nerve sheath diameter (ONSD) measurement using ocular ultrasonography was introduced as a non-invasive technique to assess intracranial pressure. We investigated changes in ONSD after cervical sympathetic block (CSB). Ultrasound-guided CSB was performed with a lateral approach at the C6 level in 35 patients. ONSD was measured before CSB and after checking for Horner's syndrome 15 minutes after CSB. The mean ONSD was significantly higher after CSB than before (5.15 ± 0.38 mm vs. 4.75 ± 0.32 mm, p < 0.001). A comparison of ONSDs between the blocked and non-blocked sides revealed that these

Postoperative fall after the use of the 3-in-1 femoral nerve block for knee surgery: a report of four cases.

We present a serious postoperative complication related to the use of femoral nerve block in 4 patients, each of whom fell and sustained further injury. Preoperatively, all patients underwent a 3-in-1 femoral nerve block with 30 to 35 ml of 0.25% levobupivacaine with 1:200,000 epinephrine, with guidance by a nerve stimulator. After the falls, neurological examination of the operated legs revealed reduced 2-point discrimination, pain, and/or light touch sensation. All patients underwent further operation for the fall injury and had delayed full weight bearing. We recommend that, after having a

Sympathetic nerve blocks for posttraumatic stress disorder: an evidentiary review for future clinical trials.

Posttraumatic stress disorder (PTSD) is a chronic disorder resulting from exposure to traumatic events. In recent years, sympathetic nerve blocks have gained interest as an emerging treatment modality for PTSD. They have been shown to reduce autonomic dysfunction associated with PTSD symptoms, particularly in refractory and treatment-resistant patients. However, there is limited evidence regarding the technique's effectiveness in PTSD patients. Therefore, this scoping review was designed to update and summarize the current literature on this topic to inform the design of future clinical trials

Lateral cervical epidural catheter placement for continuous unilateral upper extremity analgesia and sympathetic block.

BACKGROUND AND OBJECTIVES: The use of the laterally directed cervical epidural catheter for the treatment of acute and chronic pain has not been previously described. We have used this technique in a series of 30 patients to produce unilateral upper extremity analgesia and sympathetic block. We present a case report of a patient treated with this technique and a description of the technique and results from the series. METHODS: We placed an epidural catheter at the C6-7 vertebral interspace and directed the needle bevel and catheter to the affected side in a woman with a history of complex re

Bilateral burning foot pain: monitoring of pain, sensation, and autonomic function during successful treatment with sympathetic blockade.

We describe a patient with burning pain in both feet associated with local autonomic disturbances following bilateral traumatic sciatic mononeuropathies. The diagnosis of a sympathetically maintained pain was confirmed through a prompt response to sympathetic blockade. Although a mild alcohol-nutritional neuropathy was found, the clinical findings strongly suggested a diagnosis of bilateral causalgia. Clinical evaluation and quantitative sensory testing were performed prior to and after successive unilateral lumbar sympathetic nerve blocks. After unilateral blockade, bilateral improvement was

Hemodynamics of hyperthyroidism. The effects of autonomic nervous blocking and anti-thyroid drug treatment.

This work was intended to analyze the sympathetic and parasympathetic factors affecting the hemodynamics of hyperthyroidism. Seven patients with hyperthyroidism, diagnosed based on the determinations of BMR, 131I-uptake, T3-resin sponge uptake (T3-RSU), and serum level of thyroxine (T4) were subjected to the study. The hemo-dynamic estimation was done (1) at rest, (2) after vagus blocking by the injection of 0.04 mg/Kg b.wt. atropine and 0.2 mg/Kg b.wt. propranolol before (hyperthyroid state=H) and after (euthyroid state=E) anti-thyroid drug therapy. Cardiac output was measured by the dye-dilu

The blocking action of choline 2:6-xylyl ether bromide on adrenergic nerves.

Choline 2:6-xylyl ether bromide (TM 10), given systemically to cats in doses of 5 to 15 mg./kg., abolishes the effects of adrenergic nerve stimulation whilst leaving the reactions of the effector organs to adrenaline unimpaired. The effects of a single dose may take up to one hour to become fully established and last for more than twenty-four hours. Apart from transitory ganglionic blockade, cholinergic autonomic nerves are unaffected even by large doses of TM 10. Doses of TM 10 which produce effective blockade do not impair conduction along adrenergic nerve trunks; the drug must, therefore, a

Atrial fibrillation electrical remodelling via ablation of the epicardial neural networks and suprathreshold stimulation of vagosympathetic nerve.

BACKGROUND: Numerous studies have shown that the cardiac autonomic nervous system (CANS) is involved in the occurrence and persistence of atrial fibrillation (AF). The CANS is commonly considered to consist of the extrinsic and intrinsic autonomic nerves. The influence of exogenous and endogenous nerve stimulation plexus ablation on pulmonary vein sleeves and atrial myocardium provides important information in understanding the occurrence and persistence of AF. Vagosympathetic nerve stimulation and epicardial neural networks are important participants in atrial electrical remodelling (AER). El

The painful stump neuroma and its treatment.

Successful clinical management of symptomatic neuromas continues to present a challenge to the responsible surgeon. It is unexplained why some patients with neuromas are completely asymptomatic while others exhibit debilitating symptoms. Prevention of neuromas is paramount with precise attention to severed nerves following amputations and other surgical procedures. Once established, treatment of neuromas consists of careful patient counseling, local massage, and desensitization procedures. Sympathetic blockade with guanethidine may be beneficial in some patients. When necessary, surgical excis

Differential nerve block by bupivacaine and 2-chloroprocaine. An experimental study.

The differential blocking effect of bupivacaine and 2-chloroprocaine was studied on isolated rabbit cervical sympathetic trunks (B and C fibres) and phrenic nerves (A fibres). The B fibres (myelinated, preganglionic) were more sensitive than C fibres (unmyelinated, postganglionic) to nerve block by bupivacaine and 2-chloroprocaine, as assessed by changes in action potential amplitude and latency. The blocking action of both bupivacaine and 2-chloroprocaine caused a decrease in amplitude in the B fibres approximately twice as great as that produced in the C fibres. In similar experimental condi

The human vagal complex: from gross anatomy to single neurons, from brainstem to abdomen.

The vagus nerve (VN) connects the brainstem to the viscera to regulate many physiological functions. Mapping the VN with state-of-the-art high-resolution imaging modalities has tremendous potential to improve the safety and efficacy of existing autonomic neuromodulation therapies and to inform design of new therapies. We developed the most comprehensive mapping pipeline of the VN to date, from the perspectives of sample size, lengths of nerve, and range of imaging modalities. Our pipeline maps cadaveric human vagal anatomy with complementary imaging modalities applied serially to the same tiss

Method for measuring cervical vagal nerve activity in conscious rats.

The current study aimed to propose a method to directly measure right cervical vagal nerve activity (cVNA) alongside renal sympathetic nerve activity (RSNA) in conscious rats. The right cervical vagus nerve was surgically exposed and fitted with a bipolar electrode to record cVNA. A microcatheter was used to administer levobupivacaine to selectively block afferent cVNA. Upon levobupivacaine administration, cVNA was reduced by 84%, enabling the exclusive assessment of efferent cVNA. Intravenous and intraperitoneal administration of cholecystokinin-8 (CCK-8) demonstrated that peripherally acting

Stellate Ganglion Block for Post-traumatic Stress Disorder: A Comprehensive Review of Evidence, Technique Considerations and Symptom Outcomes in Military and Non-Military Patients.

PURPOSE OF REVIEW: Post-traumatic stress disorder (PTSD) is a psychiatric disorder which is defined by four symptom clusters of intrusive re-experiencing, avoidance, negative mood/cognitive changes, and hyperarousal. This pathway is mediated by a dysregulation in the amygdala-hippocampus-medial prefrontal cortex circuit resulting in an abnormal threat processing and other symptoms. Stellate ganglion blocks (SGB) are largely supported in a review of the current literature. The objective of this review is to synthesize available evidence and extrapolate findings to identify patient and symptom p

Intra-Cardiac Arrest Use of Stellate Ganglion Block for Refractory Ventricular Tachycardia.

BACKGROUND: Refractory ventricular dysrhythmia, or electrical storm, is a cardiac condition consisting of three or more episodes of ventricular dysrhythmia resistant to treatment within a 24-hour period. These dysrhythmias carry high morbidity and mortality if not diagnosed and abated promptly. When traditional resuscitative algorithms fail to return a patient to a perfusing rhythm, providers need to consider other, more novel techniques to terminate these dangerous dysrhythmias. One approach is the use of a stellate ganglion block, which has been documented in the literature only a handful of
