Pressure Points Technique for Traumatic Proximal Axillary Artery Hemorrhage: A Case Report.

INTRODUCTION: While the pressure points technique for proximal hemorrhage control is long known, it is not recommended in standard prehospital guidelines based on a study showing the inability to maintain occlusion for over two minutes. MAIN SYMPTOM: This report details a gunshot wound to the left axillary area with complete transection of the axillary artery, leading to profuse junctional hemorrhage and profound hemorrhagic shock. THERAPEUTIC INTERVENTION: Proximal pressure of the subclavian artery was applied against the first rib (the pressure points technique) and maintained for 28 minut…

Surgical anatomy of the axillary artery: clinical implications for open shoulder surgery.

BACKGROUND: Axillary artery injury is a devastating complication related to anterior shoulder surgery and can result in significant morbidity and/or mortality. The purpose of our study was to evaluate the course of the axillary artery in relation to bony landmarks of the shoulder and identify variations in artery position with humeral external rotation. MATERIALS AND METHODS: Dissection of 18 shoulders (9 fresh whole-body cadavers) with simulated vessel perfusion using radiopaque dye was performed. The axillary artery position was measured from multiple points including 2 points on the coraco…

Ruptured axillary artery pseudoaneurysm following an anterior glenohumeral dislocation: A case report.

Vascular injuries following anterior shoulder dislocations are rare, with an estimated incidence of 1-2%. The formation of an axillary artery pseudoaneurysm secondary to vascular trauma is a possible late complication and frequently underdiagnosed since it may remain asymptomatic for many years. A rupture of a pseudoaneurysm may occur either from the dislocation itself or after forceful reduction attempts. A ruptured pseudoaneurysm of the axillary artery is a medical emergency and may result in significative upper-limb morbidity or even patient mortality. Nowadays, endovascular techniques have…

Endovascular repair of acute ascending aortic disruption via the right axillary artery.

Endovascular repair of emergent syndromes involving the ascending aorta is uncommon. We describe an acute disruption of the ascending aorta during stenting of the pulmonary artery, resulting in an acute aortopulmonary artery defect and severe pulmonary edema. The disruption was treated successfully using an endovascular approach, with rapid resolution of the patient's pulmonary edema.

Blunt trauma of thorax with subclavian and axillary artery lesion--case report.

We report a rare case of blunt trauma of the axillary artery in a 20-year-old man who was injured as a motorcycle rider and received severe body injuries. Injuries included severe trauma of the left lower leg with contusion and extensive soft tissue and bone trauma of these regions with poor general condition and with the presence of clinical signs of traumatic shock. Upon arrival, we found that in addition to earlier clearly visible trauma to the leg, there was a hematoma of the medial side of the left supraclavicular region and the absence of the radial artery pulse with paralysis of the lef…

Unusual clinic presentation of polyarteritis nodosa: involvement of axillary and brachial artery.

Polyarteritis nodosa (PAN) is a necrotizing inflammatory vasculitis of small- and medium-sized muscular arteries. Multiple organs, especially the kidney, skin, peripheric nerves, striated muscles, and intestine, are affected. In this case report, a patient with renal and hepatic involvement and axillary and brachial artery aneurysm resulting in shoulder hematoma and pain due to underlying PAN is presented.

Experience using the thoracodorsal artery perforator flap in axillary hidradentitis suppurativa cases.

Hidradenitis suppurativa is a chronic and recurrent inflammatory disease of the apocrine glands characterized by recurrent abscesses, draining sinus tracts, and scarring that can be located in the groin, axilla, perineal, and perianal areas and less frequently in the scalp area. The chronic and relapsing nature of hidradenitis suppurativa leads to physical and psychological damage because it frequently causes disabling pain, diminished range of motion, and social isolation. Surgical removal of all apocrine glands in the affected region is the definitive treatment because conservative treatment

[Posterior circumflex humeral artery perforator flap (PCHAP-flap) in axillary scar release].

Scar contracture after burn injury is frequent in the axillary region. If conservative treatment of scars limiting the range of motion of the shoulder joint fails, surgical correction is needed. The authors performed an early reconstruction in a case with unstable scar formation and limited range of motion after burn injury to the axillary region, using the posterior circumflex humeral artery perforator flap. In their opinion, the flap (beside its conventional use as a free flap) may be useful as an island flap in the reconstruction of the axillary region, e. g. for axillary scar release.

Right axillary artery cannulation for surgical management of the hostile ascending aorta.

Extensive aortic disease, such as atherosclerosis with aneurysms or dissections that involve the ascending aorta, can complicate the choice of a cannulation site for cardiopulmonary bypass. To date, the standard peripheral arterial cannulation site has been the common femoral artery; however, this approach carries the risk of atheroembolism due to retrograde aortic perfusion, or it is undesirable because of severe iliofemoral disease. Arterial perfusion through the axillary artery provides sufficient antegrade aortic flow, is more likely to perfuse the true lumen in the event of dissection, an…

Axillary artery cannulation improves operative results for acute type a aortic dissection.

BACKGROUND: This study was undertaken to identify preoperative and postoperative predictors of hospital death of patients with acute type A aortic dissection. METHODS: Between May 1,1992, and July 31, 2004, 106 consecutive patients (59 male and 47 female, mean age 62.2 +/- 12.1 years) with acute type A aortic dissection underwent surgery with open technique and cerebral protection by antegrade selective cerebral perfusion. The external iliac artery or femoral artery alone was used for arterial cannulation in 37 patients; however, the right axillary artery was cannulated in 69 patients. Univar…

Axillary artery injury secondary to anterior shoulder dislocation: report of two cases.

Vascular injuries secondary to isolated shoulder dislocation are rare. Unawareness for closed axillary artery trauma by many physicians treating shoulder dislocations, counts often for missed or delayed diagnosis. The authors describe two cases that presented with an anterior shoulder dislocation, complicated by a disruption of the axillary artery with subsequent thrombosis. The various pathogenic mechanisms are discussed. The pathognomic triad consists of anterior shoulder dislocation, absent or diminished distal pulse and an axillary protruding hematoma. Prompt surgical arterial repair is ma…

Transbrachial endovascular exclusion of an axillary artery pseudoaneurysm with PTFE-covered stents.

PURPOSE: Endovascular exclusion of arterial injuries associated with arteriovenous fistulas and pseudoaneurysms has only recently been described using various stent-graft prostheses. This report details a transbrachial technique used to exclude an axillary artery pseudoaneurysm developing at the axillary anastomosis of an axillofemoral graft. METHODS AND RESULTS: Thin-walled polytetrafluoroethylene was expanded with an angioplasty balloon catheter and used to cover standard Palmaz stents. Two covered stents were delivered under fluoroscopic guidance via open brachial artery access to the site…

[Profuse hemorrhage from the axillary artery in electric burn: prevention and treatment].

A total of 53 cases of electric burns were admitted to our unit during the 1977-1985 period. In 19 cases an upper limb was involved, accounting 35.8 percent of the total. In 15 cases disarticulation of the shoulder was done, and in 3 of them a profuse hemorrhage from the axillary artery occurred as a result of infection of the amputation stumps. The second segment of the axillary artery was ligated during the amputations in the other 4 cases, no hemorrhage occurred in them.

Retrograde blood flow in the brachial and axillary arteries during routine radial arterial catheter flushing.

BACKGROUND: Flushing of radial arterial catheters may be associated with retrograde embolization of air or thrombus into the cerebral circul … ction through the radial arterial catheter was related to the probability of retrograde flow in the axillary artery (P < 0.001). Reversed arterial flow was noted in the majority of subjects (33 of 51) at a manual flush rate of less than 9 s and in no subjects (0 of 48) at a rate 9 s or greater. Retrograde flow was observed less frequently during flush valve openi …

Axillary artery: an alternative site of arterial cannulation for patients with extensive aortic and peripheral vascular disease.

The increasing number of patients with extensive aortic and peripheral vascular atherosclerosis or aneurysms who are undergoing cardiac operations present difficult decisions as to the optimal site of arterial cannulation for cardiopulmonary bypass. Femoral artery cannulation is the most common alternative to ascending aortic cannulation, but severe iliofemoral disease or the danger of atheroemboli caused by retrograde perfusion through an atherosclerotic or aneurysmal descending aorta may make this approach impossible or undesirable. We have used axillary artery cannulation for cardiac operat…
