[Complications in long-term survivors of biliary atresia].

While most long-term survivors of biliary atresia lead lives of good quality, some patients experience late complications. Cholangitis is a common complication even among 20-year survivors including some who develop ongoing liver dysfunction. Portal hypertension is another late complication. Endoscopic treatment for esophageal varices and partial splenic embolization for hypersplenism is effective as long as hepatic functional reserve is preserved. On the other hand, the treatment of secondary intrapulmonary vascular disorders including hepatopulmonary syndrome and portopulmonary hypertension …

Cholangiocyte secretion of chemokines in experimental biliary atresia.

UNLABELLED: Biliary atresia (BA) is a disease of the newborn that results in obstruction of the biliary tree. The cause of BA remains unknown; however, recent studies using the murine model of biliary atresia have found that rotavirus infection of the biliary epithelial cell (cholangiocyte) triggers an inflammatory response. We hypothesized that rotavirus infection of cholangiocytes results in the release of chemokines, important mediators of the host immune response. METHODS: In vivo, Balb/c pups were injected with rhesus rotavirus (RRV) or saline, and, their extrahepatic bile ducts were mic…

Hepatic haemangioendothelioma: a vertical association with biliary atresia?

Haemangioendotheliomas (HAE) are the commonest vascular tumours of the liver in infancy and are characterised by solitary or multifocal lesions often arising in conjunction with cutaneous haemangiomas. The authors report on a 4-month-old infant with massive hepatomegaly and hypothyroidism due to multifocal HAE whose mother had been born with biliary atresia. As the incidence of biliary atresia in the UK is about 1 in 16 000 live births, the association reported here seems more than co-incidental. This report reviews the literature and discusses potential aetiologies to explain this rare associ…

Immunohistochemical study on liver fibrosis in biliary atresia.

BACKGROUND/AIMS: Although advanced liver fibrosis is a critical complication in influencing the outcome of biliary atresia (BA), the mechanism is poorly understood. In adult hepatic disorders, the relationships between sinusoidal liver cells (SLC) such as hepatic stellate cells (HSCs), some growth factors and enzymes concerned with extracellular matrix (ECM) metabolism have been clarified, but are unknown in BA. This study aimed to investigate such relationships in BA. METHODOLOGY: Seventeen liver samples from 14 patients with BA were immunohistochemically examined using primary antibodies su…

Fractures in biliary atresia misinterpreted as child abuse.

Bone fractures in children without a history of injury are highly suspicious for child abuse. Biliary atresia is a disorder associated with metabolic bone disease, and there are numerous reports of osteopenia, rickets, and/or fractures in this population. We report 3 cases of children with biliary atresia who had bony fractures as well as osteopenia whose caretakers were investigated for child abuse. Pediatricians should be aware of an increased incidence of fractures and overall prevalence of bone disease in this population.

Magnetic resonance cholangiography for the diagnosis of biliary atresia.

PURPOSE: The aim of this study was to evaluate the usefulness of magnetic resonance cholangiography (MRC) for the diagnosis of biliary atresia in infantile cholestatic jaundice. METHODS: Forty-seven consecutive infants with cholestatic jaundice underwent single-shot MRC. The diagnosis of biliary atresia was made by MRC based on the nonvisualization of extrahepatic bile ducts and excluded on the basis of the complete visualization of extrahepatic bile ducts. The final diagnosis of biliary atresia (BA group, n = 23) or nonbiliary atresia (NBA group, n = 24) was established by operation or clini…

Is extrahepatic biliary atresia related to viral infections?

Extrahepatic biliary atresia is characterized by obliteration or absence of the extrahepatic bile ducts in very young infants and ultimately lead to liver cirrhosis and failure. The cause of this disease is currently unknown. There are several hypotheses for the development of extrahepatic biliary atresia. Failure of development or canalization of extrahepatic biliary tree is the unconfirmed old theory. Perinatal insults like viral infections have been suspected as possible etiologies including reovirus type 3, cytomegalovirus and rotavirus, although none of them has been concluded as playing …

Diverse morphology of biliary atresia in an animal model.

BACKGROUND/AIMS: Extrahepatic biliary atresia can be simulated in Balb/c-mice which have been infected with rotavirus. Irreversible occlusion of the common bile duct is the result of an inflammatory process of the whole biliary tract. The observations in this animal model are similar to extrahepatic biliary atresia in newborn children. The present study describes the wide range of morphological findings in mice and compares the results with several classifications of extrahepatic biliary atresia in children. METHODS: Newborn Balb/c-mice were infected intraperitoneally with rhesus rotavirus; t…

Progress in developing animal models for biliary atresia.

Animal models for extrahepatic biliary atresia (EHBA) have failed to simulate the course of the disease. Until now only a few aspects of the entity could be investigated and no model was helpful in discovering the etiology of EHBA. Following the suspicion of a viral and hepatotropic infection, investigations in an infectious mouse model were continued. The results of previous and topical studies are summarized here. Infection of newborn Balb/c-mice with rhesus rotavirus (RRV) leads to cholestasis in 85% of the animals followed by a lethality of 90%. Preparation and histomorphological investiga…

Immunologic reaction and genetic factors in biliary atresia.

The characteristic histopathological features seen in the livers of patients with biliary atresia (BA) are very similar to those of primary biliary cirrhosis, which is an autoimmune disease. To clarify whether BA liver possess an immunological response similar to that in primary biliary cirrhosis, we studied HLA-DR expression in liver tissue of BA patients, using a HLA-DR staining method, and determined the frequency of HLA types in BA patients and their families. HLA-DR was expressed by the bile duct epithelium in 11 of 16 liver specimens obtained from 13 BA patients. By contrast, HLA-DR was …

[How to improve the results of treatment of biliary atresia].

In the present article, strategies on how to improve the clinical results of patients with biliary atresia are discussed. Our policies include 1) meticulous and accurate dissection and enteric anastomosis at the porta hepatic with the use of hepatic mobilization, 2) use of a complete external biliary fistula (Sawaguchi method), 3) use of an intussusception-type anti-reflux valve in the Roux-en-Y limb, 4) prompt reoperation if necessary, 5) prolonged use of antibiotics and choleretics, 6) in-patient care for as long as 3 months after hepatic portoenterostomy, etc. 19 of 21 patients, treated at …

[Endoscopic retrograde cholangiopancreatography in diagnosis of biliary atresia].

The differential diagnosis between neonatal hepatitis and biliary atresia in the newborn is difficult and has therapeutic implications. Despite important advances in diagnostic tools, 10-20% of newborns with jaundice remain without definitive diagnosis. In recent years ERCP has played a decisive role in achieving definitive anatomic diagnosis, thus avoiding unnecessary exploratory laparotomy. We present our experience with ERCP using a pediatric duodenoscope in 18 newborns with inconclusive diagnoses of neonatal cholestasis.

Coincidence of congenital toxoplasmosis and biliary atresia in an infant.

A 4-week-old infant presenting with neonatal cholestasis was found to have congenital toxoplasmosis and biliary atresia. This is the first patient in which their coincidence is reported. Because biliary atresia can coexist with either congenital infection or inborn errors of metabolism, evaluation for an obstructive etiology of jaundice in infants with a recognized cause of intrahepatic cholestasis is necessary.

Cholangitis after the Kasai operation for biliary atresia.

One hundred seventy-nine episodes of cholangitis in 28 consecutive patients having a Kasai operation for biliary atresia during the past 3 1/2 years were analyzed. The diagnosis was made primarily on the basis of unexplained fever (greater than 38.0 degrees C). An increase in serum bilirubin or a decrease in bile volume and in bile bilirubin concentration were often confirmatory, but other laboratory data including serum hepatic enzymes and blood and bile culture data were of little or inconsistent value. All patients were treated with systemic antibiotics. The best results were obtained with …

Requirement of parenteral fat in infants with biliary atresia.

In a prospective study on infants with biliary atresia (BA) we measured the composition of the plasma fatty acid to determine the requirement of fat emulsion. In 14 infants who underwent initial operation for treatment of BA, a trend toward essential fatty acid (EFA) deficiency was evident before the surgery. Fourteen infants given parenteral nutrition (PN) after surgery were grouped into three according to the dose of fat emulsion, fat free in 4, 10% of the total calories in 5 and 20% of the total calories in 5. After operation, EFA deficiency gradually progressed when on fat free PN. Improve…

Extrahepatic biliary atresia: a review of current management.

Macroscopic examination of the extrahepatic bile ducts in infants with biliary atresia reveals fibrous occlusion of variable extent from an inflammatory process of unknown aetiology. Histological studies have shown that bile duct remnants at the porta hepatis frequently contain small epithelium-lined channels which communicate with intrahepatic ducts and through which effective bile drainage may be established by the operation of portoenterostomy. The 4-year survival rate in untreated cases is 2 per cent, but surgical treatment can improve the outlook and recent reports suggest that a 5-year s…

Liver transplantation in biliary atresia with concomitant hepatoma.

Two cases are reported in which the very infrequently reported association was found of liver cell carcinoma and biliary cirrhosis secondary to congenital biliary atresia. A search of the literature revealed 4 previous reports of cases with similar pathology. Our 2 patients were both operated upon within the first few months of life, at which time congenital biliary atresia was documented, and in 1 instance temporarily corrected. They ran a progressive downhill course until they both received replacement livers, one at 4 years of age and the other at 12, at which times hepatoma was found in th…
