In a study of 300 Alaskan Malamutes, dwarfism was shown to be an autosomal recessive inherited disease with complete penetrance that resulted in disturbed endochondral bone formation. Osseous growth disturbance was manifest at the metaphyses of tubular bones. Clinical and radiographic changes were very similar to those of rickets, although appositional bone formation rates were normal. Serum calcium, phosphorus, and alkaline phosphatase were within normal limits. Urinary excretion of calcium, phosphate, and amino acids were normal. Excess matrix was formed in the zone of cartilage cell prolife

Fibrodysplasia (myositis) ossificans progressiva (FOP) is a rare autosomal dominant disorder characterized by symmetrical congenital malformations of the blastemal anlage of the hands and feet and by progressive heterotopic chondrogenesis and ossification of the soft connective tissues. There is neither an established pathogenesis nor an effective treatment for this disabling disorder. We reevaluated the published data on the natural history of FOP and discovered an array of developmental gradients (characteristic patterns of disease expression) similar to developmental anomalies induced by pl

Marshall-Smith Syndrome (MSS) is a rare progressive developmental disorder that severely impairs a patient's intellectual development and physical health. The only known cause for MSS is a mutation in the nuclear factor 1 X (NFIX) gene. This mutation affects neuronal development and protein transcription. Historically, most patients with MSS do not survive beyond 3 years of age. Reports of ocular findings are limited. We report a case of a 9-year-old MSS patient with progressive craniosynostosis, elevated intracranial pressure, and catastrophic ocular complications. A comprehensive PubMed lite

Fibroblast growth factors are structurally related proteins associated with cell growth, differentiation, migration, wound healing, angiogenesis, and oncogenesis. At the cellular level, their function is mediated by transmembrane tyrosinekinase receptors, fibroblast growth factor receptors. Four genes encoding fibroblast growth factor receptors have been identified, and mutations in three of these, FGFR1, FGFR2, and FGFR3, can cause different congenital, autosomal dominant disorders affecting the craniofacial and skeletal development: craniosynostosis and chondrodysplasias. The craniosynostosi

In recent years, the problem of osteoporosis is increasingly raised in children as well as adults both as a primary disorder and as secondary results to various diseases, medications, and lifestyle issues. On the other hand, molecular mechanisms of pathogenesis have been elucidated in several sclerosing disorders contributing to our understanding of basic biology of bone metabolism. In this chapter, we reviewed the recent progress on the method to evaluate BMD in growing period and etiologies of abnormal BMD in children.

In conclusion, the attempt has been made to demonstrate that three major forms of new bone formation exist: reactive, neoplastic, and the newborn or relative skeletal sclerosis in congenital (developmental) disorders. A classification of skeletal disorders has been presented and four major groups have been selected from the nine categories in this classification. These are: congenital-developmental, metabolic and endocrine, benign neoplasms and malignant neoplasms. In all four categories a large group of entities which may present with new bone (sclerosis) are listed and are discussed in some,

Despite the numerous advances in technology and techniques for total hip arthroplasty, this surgery is often not the procedure of choice for all patients or for all hip conditions. Originally described as a treatment for femoral neck nonunions, the intertrochanteric femoral osteotomy retains an enduring role in the treatment of various posttraumatic and developmental hip conditions including femoral neck nonunions, iatrogenic or postsurgical deformities, leg-length inequality, osteonecrosis, slipped capital femoral epiphysis, deformities occurring after Legg-Calvé-Perthes desease, and for cert

Mitochondria are essential regulators of bone health, controlling cell differentiation, cellular energy production, immune function, osteogenesis, and osteoclast activity. Their dysfunction is linked to orthopedic disorders such as osteoporosis, osteoarthritis, and osteomyelitis, contributing to impaired bone homeostasis and increased fracture risk. While mitochondrial research has been more advanced in fields such as cardiology and neurology, emerging therapeutic strategies from these areas are beginning to show potential for translation into orthopedics. These include mitochondrial biogenesi

Bone marrow is a highly cellular tissue undergoing significant developmental and physiologic changes with age. Indeed, with maturation from pediatric to the adult age there is a progressive, centrifugal conversion from red to yellow bone marrow. Histological characteristics of bone marrow are reflected in MR image signal. MR is therefore extremely sensitive in detecting pathological changes which are mostly characterized by increased free water causing high signal intensity on T2. Among the numerous diseases causing bone marrow edema in children chronic nonbacterial osteomyelitis (CNO) certain

Primary bone tumours are rare but account for a significant proportion of cancers occurring in childhood and adolescence. Malignant bone tumours need to be distinguished not only from their benign counterparts but also from tumour-like lesions, many of which are developmental or reactive in nature and are found commonly in the paediatric population. Taking note of the age of the patient and the site of the lesion within bone (aided by several imaging techniques including plain radiographs, ultrasound, computed tomography, bone scintigraphy and magnetic resonance imaging) is essential for patho

INTRODUCTION: Angular deformity in the growing skeleton of animals, especially in the radius and ulna, is occasionally seen in clinical practice. The mechanism of spontaneous correction of these angular deformities however remains to be elucidated. The purpose of our experiment was to explore the ability of a growth plate to correct an induced valgus deformity, and to study the mechanism of correction.
METHODS: Before beginning the study, valgus deformity of the distal radius had been induced in lambs by the application of a device that causes asymmetrical compression of the growth plate. The

This current study describes how human genetic approaches are used to identify and understand the roles of genes and their protein products, during skeletal development, growth, and homeostasis. Searches for the genes responsible for brachydactyly, symphalangism, spondyloepiphyseal dysplasia, and camptodactyly-arthropathy syndrome are presented to exemplify these approaches. The important role of the orthopaedic surgeon in this process is discussed.

We describe two female fetuses conceived by a nonconsanguineous couple. The pregnancies were interrupted at 31 and 26 weeks of gestation, respectively, because of severe microcephaly. Postmortem X-ray and autopsy studies showed in both fetuses: 1) severe intrauterine growth retardation; 2) facial anomalies characterized by severe microcephaly, sloping forehead, low set and posteriorly angulated ears, prominent eyes, down-slanting palpebral fissures, large nose, small mouth with full lips, and mild microretrognathia; 3) severe brain hypoplasia that was more pronounced in the second fetus; 4) se

To evaluate the developmental status of children and adolescents, a bone-age chart based on the radiograph of hand and wrist has been used in many countries. Bone-age reflects not only the functional status of various hormones but also the influence of chronic disease and it has been used more widely than other indices such as the height-weight-age table. As a standard bone-age chart has not been established in Korea, a foreign bone-age chart has been used in clinics. To make a Korean standard bone-age chart, we took the radiographs of the left hand of about 5,400 children covering the whole c

Stress-induced upregulation of interleukin-6 (IL-6) signaling and its downstream pathophysiological consequences have garnered considerable attention in recent years. However, no comprehensive review has specifically examined the association between stress-induced IL-6 and its implications for bone health in children. During childhood, linear growth and peak bone mass acquisition are tightly regulated processes. Hence, this review aims to investigate current evidence on stress-induced IL-6 upregulation and its detrimental effects on pediatric bone health. Findings from animal models, knockout

Dysplasia epiphysealis hemimelica (DEH), also known as Trevor disease, is a rare pathologic proliferation of cartilage with unknown etiology creating cartilaginous osteochondroma exostoses intra-articularly or juxta-articularly. Herein, we reviewed the literature about acetabular osteochondroma in children and report a case of a 9-year-old boy who presented to the orthopaedic clinic with complaints of gait disturbance, right hip discomfort, and with increasing severity and frequency of hip subluxation episodes over the course of a year. Imaging studies revealed dysplasia of the right hip with
