Pathological Distal Tibial and Fibular Fracture in a Paediatric Patient: A Case Report.

Paediatric distal tibial and fibular fractures are seen quite regularly in orthopaedic trauma practice. Most patients are managed conservatively with closed reduction or casting while only a selected few required surgical treatment. Surgical options include plating, percutaneous Kirschner wires, rigid intramedullary nails, and flexible intramedullary nailing. This is dependent upon the patient's age, fracture site, comminution, and concomitant injuries. Here, we present an interesting case of a patient with an unusual lesion seen at the fracture site. This lesion was curetted out during surger

BMP2 and GDF5 for Compartmentalized Regeneration of the Scapholunate Ligament.

Background  Chronic injuries to the scapholunate ligament (SLIL) alter carpal kinematics and may progress to early degenerative osteoarthritis. To date, there is no consensus for the best method for SLIL reconstruction. This study aims to assess the use of growth factors (bone morphogenetic protein [BMP]2 and growth and differentiation factor 5 [GDF5]) for compartmentalized regeneration of bone and ligament in this multiphasic scaffold in a rabbit knee model. Case Description  A total of 100 µg of BMP2 and 30 µg of GDF5 were encapsulated into a heparinized gelatin-hyaluronic acid hydrogel and

Intra-medullary, at fracture site introduction of K-wires for metacarpal fracture fixation (in-site technique). A new fixation technique and a case series.

PURPOSE: Metacarpal bone fractures represent one-fifth of upper extremity fractures. The most commonly affected bone was the fifth metacarpal also known as boxer's fracture with a range 9.7-50%. Different techniques have been described for fracture fixation with K-wires being the most described technique. In this paper, we present a new technique utilizing k-wires to reduce and fix metacarpal fractures in an easy less traumatic way aiming to optimize the clinical outcomes in regards of range of motion and stiffness. METHODOLOGY: Retrospective chart review including all the patients that were

Penetration injury of the hindfoot following intramedullary nail fixation of a tibial fracture.

Technical errors during intramedullary nail insertion are not uncommon. We report a case of tibial guide wire penetration into the distal tibial articular surface, the talus and the calcaneus during insertion of the nail with the ankle dorsiflexed. This has not been reported in the past. Computerized tomogram was a useful tool in the diagnosis. This complication was associated with long-standing ankle pain, which however eventually settled. We advise frequent use of biplanar C-arm image during the insertion of the guide wire, the reamer and tibial nail into the medullary canal of the tibia or

Adding a protective K-wire during opening high tibial osteotomy increases lateral hinge resistance to fracture.

PURPOSE: It was hypothesized in this in-vitro study that positioning a K-wire intersecting the cutting plane at the theoretical lateral hinge location would limit the cut depth and help preserve the lateral hinge during the opening of the osteotomy. Objectives were (1) to compare the mechanical resistance of the hinge and the protective effect of leaving the K-wire during the opening procedure (2) to check if the K-wire would limit the depth of the osteotomy. METHODS: An ex-vivo mechanical study, testing 5 pairs of fresh-frozen tibias, was designed. CT-scan based Patient-specific cutting guid

Second Metatarsal Fracture During Lapidus Hallux Valgus Correction: A Case Report.

CASE: A 67-year-old woman with rheumatoid arthritis underwent Lapidus hallux valgus correction. Initial realignment was unsatisfactory, so the K-wire through the first and second metatarsals was removed. As the clamp was repositioned, the second metatarsal fractured through the K-wire hole. The Lapidus procedure was completed, and then, the fracture was fixed with a percutaneous K-wire. The tarsometatarsal fusion and second metatarsal fracture healed. CONCLUSION: Because second metatarsal structural integrity is imperative for use of the clamp, bone quality and K-wire holes from re-reduction

A Technical Innovation in Midface Advancement With the External Frame Distraction Device.

BACKGROUND: Maxillary advancement with a rigid external distractor (RED) II is a commonly performed procedure for correcting midface hypoplasia. While there are various methods of anchoring the osteotomized maxillary segment to the halo device, the looped transpyriform wire is the simplest and most cost-effective. However, a common complication with this is the cutting through of the stainless steel traction wire across the pyriform buttress during distraction. The patient with cleft undergoing midface distraction is particularly vulnerable since the force needed for distraction is higher due

[Rehabilitation treatment of inveterate meta-epiphyseal deformities of the radius].

Unilateral separate anchor--wiring osteosynthesis of the forearm bones has essential advantages over the accepted methods for osteosynthesis of metaepiphyseal fractures of the radius. For adaptation to modern requirements an apparatus is suggested with external fixation of the radial diaphysis and the second metacarpal bone by means of bundles of short Kirschner wires with Y-shaped insertion of the pointed ends. The apparatus provides for longitudinal and transverse movement with fixation of the fragments at any level of the forearm and maintenance of rotational movements and movements at the

[CT characteristics of solitary long bone cysts before and after treatment].

Computed tomography was used to study the X-ray morphological characteristics of femoral and humoral bone cysts and the course of a reparative process after surgery in 23 patients with varying activity of solitary bone cysts. Treatment using transosseous osteosynthesis, intraosseous armoring, and therapeutic and diagnostic punctures in patients with solitary bone cysts could yield the most active osteoproductive effect and cause a considerable increase in the density of cyst contents in active and activity-losing stages, which was most evident around the intraosseous wires. Patients with late-

The addition of cerclage wiring does not improve proximal bicortical fixation of locking plates for Type C periprosthetic fractures in synthetic humeri.

BACKGROUND: Treatment of proximal humerus periprosthetic fractures is challenging. It remains difficult to achieve robust fixation of the proximal fragment to the locking plate using cerclage wiring and/or unicortical screws. Use of polyaxial tangentially directed bicortical locking screws increases screw purchase, but it is unclear if this option provides robust fixation. This biomechanical study compares fixation of constructs using cerclage wires, bicortical locking screws, and a hybrid method utilizing both methods. METHODS: Uncemented humeral stems were implanted into synthetic humeri an

Internal fixation of scaphoid non-union: a comparative study of three methods.

In this study we compared the results of three methods of fixation for scaphoid non-union. The implants used were the AO 2 mm mini-fragment screw, the Herbert screw and the Kirschner (K) wires. Between 1990 and 1999, 132 patients underwent surgery for scaphoid fractures. We used the modified Filan and Herbert classification. Patients with acute fractures and patients requiring vascularised bone grafts were excluded. Twenty-six non-unions were fixed with an AO mini-fragment screw, 58 with a Herbert screw, and nine with K-wires. Radiological union was achieved in 85% of cases using the AO screw,

[Fractures of the patella].

Fractures of the patella account for approximately 0.5% to 1.5% of all skeletal injuries. The diagnosis is made by means of the mechanism of injury, physical and radiological findings. The kind of treatment of patella fractures depends on the type of fracture, the size of the fragments, the integrity of the extensor mechanism and the congruity of the articular surface. Independent of the kind of treatment an early rehabilitation is recommended. Modified tension band wiring is the most commonly used surgical treatment for patella fractures and can be used for almost every type of fracture. Due
