Bracing and rehabilitation for wrist and hand injuries in collegiate athletes.

Athletic injuries of the hand and wrist are common. The key to management of these injuries is prevention. Certain sports require athletes to participate in positions that pose a higher risk of injury to the fingers and wrists. Once healing of the injured digit and wrist has occurred, rehabilitation of the injury is important. This requires close communication between the therapist and the strength and conditioning coach to allow strengthening exercises but limit traction to the involved injury in order to limit the risk of reinjury. Finally, once the injury has been rehabilitated, protective

Vertical leg stiffness following ankle taping and bracing.

This study sought to determine if prophylactic ankle taping and bracing influenced vertical leg stiffness during hopping. Twenty healthy and physically active participants completed testing under three ankle stabilizer conditions, ankle tape, semi-rigid ankle brace or control (no stabilizer) during three separate sessions. Immediately following stabilizer application, participants completed an exercise bout prior to completing the hopping. Vertical ground reaction forces were collected during the hopping and were used to calculate vertical leg stiffness. No significant changes in vertical leg

Ankle bracing and the neuromuscular factors influencing joint stiffness.

CONTEXT: Health care professionals commonly prescribe external stabilization to decrease the incidence and severity of ankle sprains. The mechanism for this decrease is not clearly understood. Examining the effects of ankle bracing on biomechanical stability and influencing factors may provide important information regarding the neuromuscular effects of bracing. OBJECTIVE: To study the effects of 2 different ankle braces on the neuromuscular factors influencing ankle stiffness. DESIGN: Mixed-model repeated-measures design. SETTING: Research laboratory. PATIENTS OR OTHER PARTICIPANTS: Twent…

Bracing in external rotation for traumatic anterior dislocation of the shoulder.

We undertook a prospective study in 51 male patients aged between 17 and 27 years to ascertain whether immobilisation after primary traumatic anterior dislocation of the shoulder in external rotation was more effective than immobilisation in internal rotation in preventing recurrent dislocation in a physically active population. Of the 51 patients, 24 were randomised to be treated by a traditional brace in internal rotation and 27 were immobilised in external rotation of 15 degrees to 20 degrees. After immobilisation, the patients undertook a standard regime of physiotherapy and were then asse

[The role of functional bracing in the treatment of humeral shaft fractures].

OBJECTIVES: We evaluated clinical, radiographic, and functional results of patients treated with functional bracing for humeral shaft fractures. METHODS: Humeral shaft fractures of 30 patients (19 males, 11 females; mean age 34 years; range 18 to 64 years) were treated with functional bracing. Fractures were on the right in 18 patients, on the left in 12 patients. All were closed fractures, being spiral in 10, comminuted in nine, transverse in six, and oblique in five patients. Humeral fractures were in the upper third, middle third, and distal third in 16, 8, and 6 patients, respectively. Fu

The effect of wearing a restrictive neck brace on driver performance.

OBJECTIVE: Thousands of Americans are prescribed cervical orthoses each year. These orthoses restrict motion, which may influence the patient's driving performance. No legal restrictions exist that prohibit patients from wearing cervical orthoses while driving. No study addressing this issue has been published to date. Thus, we sought to assess the effects of wearing a restrictive neck brace on driver performance on the open road. METHODS: We conducted a prospective, randomized block design study in 23 volunteers. Twenty-three adult licensed drivers from the state of Iowa were recruited. Eval

The duPont kyphosis brace for the treatment of adolescent Scheuermann kyphosis.

The purpose of this study was to evaluate the effectiveness of a newly designed brace in the treatment of adolescent Scheuermann thoracic kyphosis. Twenty-two children who met the roentgenographic criteria of Scheuermann kyphosis and were compliant with treatment were followed until skeletal maturity. Sixteen patients (73%) showed nonprogression of their kyphosis (nine patients demonstrated an improvement, seven patients remained unchanged), and had a mean improvement of 9 degrees (64 degrees to 55 degrees). Six patients (27%) demonstrated progression of the kyphosis and had a mean increase in

Functional bracing in the treatment of delayed union and nonunion of the tibia.

We treated 16 delayed unions and 57 nonunions of the tibial diaphysis with a below-the-knee functional brace. In 48 cases, bracing was preceded by fibular ostectomy, and ten patients had an additional bone graft. We were able to follow 67 patients, of whom six (8.7%) failed to respond to treatment. In patients with nonunion, bony healing occurred with a median of 4 months. There was no difference in the speed of healing according to the level of the defect. Shortening of the limb following ostectomy of the fibula had a mean of 3 mm in the delayed-union group and 5 mm in the nonunion group.

Spondylolysis: returning the athlete to sports participation with brace treatment.

Between 1988 and 1995, 73 adolescent athletes treated with the Boston Overlap Brace for spondylolysis were reviewed to evaluate improvement in pain score and activity level. A favorable clinical outcome was achieved in 80%. Girls and boys who participated in high-risk sports were five times more likely to have an unfavorable clinical outcome than those who participated in low-risk sports (odds ratio = 5, 95% confidence interval = 2.4-7.5, P = .003). In addition, acute onset of pain and hamstring tightness were associated with a worse outcome. Athletes with symptomatic spondylolysis treated wit

Spinal deformity in familial dysautonomia. Prevalence, and results of bracing.

BACKGROUND: Familial dysautonomia (Riley-Day syndrome) is an autosomal recessive disorder primarily affecting individuals of Ashkenazi Jewish extraction. It affects the autonomic, central, and peripheral nervous systems. Spinal deformity (mainly scoliosis) is the most common orthopaedic problem in patients with familial dysautonomia. The objectives of our study were to document the prevalence of spinal deformity in a referral center for familial dysautonomia and to determine the effectiveness of bracing. METHODS: We performed a retrospective radiographic and clinical study of 123 patients wit

In normal subjects bracing impairs the function of the inspiratory muscles.

Normal subjects can increase their capacity to sustain hyperpnoea by bracing their arms on fixed objects, a procedure which is also known to reduce dyspnoea in patients with chronic obstructive pulmonary disease (COPD). In the present study, it was tested whether bracing per se could improve the function of the diaphragm. The effect of bracing on diaphragm function was studied in six normal subjects by recording changes in oesophageal (delta Poes) and transdiaphragmatic (delta Pdi) pressure during inspiratory capacity (IC) manoeuvres in the seated and upright postures, and in the seated postur

Biological factors and predictability of bracing in adolescent idiopathic scoliosis.

Biological factors that could have a predictive value in treating idiopathic scoliosis by brace were studied in 107 patients (102 girls and 5 boys). The mean age of the patients was 14 years 6 months, and the mean Cobb angle of the curves was 36 degrees at start of treatment. The Boston brace was used for an average of 1 year 6 months, and mean follow-up time after weaning was 3 years. Scoliosis with an apex of Th 10-12 proved to be the most favorable for the final result, with a mean correction of 2 degrees. All other curves remained unchanged. Patients who had a period of rapid growth just b

Functional bracing of fractures of the second through fifth metacarpals.

A simple new brace aids in the reduction and immobilization of fractures of the second, third, fourth, and fifth metacarpals. This brace incorporates an adjustable strap and pads made of radiolucent materials. It provides 3-point fixation while allowing full range of motion of the wrist and digits. It has been preferred by patients to an ulnar gutter splint, and its use resulted in less residual angulation of the fracture than occurred with other methods.

An instrumented brace for study of Legg-Calve-Perthes disease.

The accepted clinical method for treating children with Legg-Calve-Perthes disease is by means of abduction bracing. To evaluate the effectiveness of this method, one major necessary step is solving the three-dimensional inverse dynamics problem for the intersegmental loads at the hip joint. In order to provide part of the external loading data on the lower limb required to solve this problem, an instrumented abduction brace has been designed. The instrumented brace duplicates the mechanical behavior of the Scottish Rite Atlanta treatment brace, currently a clinically popular device. Instrumen

[Indications and technic of functional fracture treatment with the Sarmiento brace].

Fracture bracing is a way of conservative fracture treatment on the extremities, which avoids fracture disease by long-term immobilization. Indication is given in humerus shaft-, ulna shaft- and tibia or lower leg fractures, if there is no contraindication like angulation, shortening, muscle distraction or nerve injury. If these and other contraindications are taken into account, functional fracture bracing can be expected to be successful.
