Is vaginal delivery of a fetus in breech presentation at an extremely preterm gestational age associated with an increased risk of neonatal death? A comparative study.

BACKGROUND: The effect on neonatal mortality of mode of delivery of a fetus in breech presentation at an extremely preterm gestational age remains controversial. OBJECTIVE: To compare mortality associated with planned vaginal delivery (PVD) of fetuses in breech presentation with that of fetuses in breech presentation with a planned cesarean delivery (PCD). MATERIAL AND METHODS: Retrospective study reviewing records over a 19-year period in a level 3 university referral center of singleton infants born between 25+0 and 27+6 weeks of gestation, alive on arrival in the delivery room, and weighi

The influence of the fetal leg position on the outcome in vaginally intended deliveries out of breech presentation at term - A FRABAT prospective cohort study.

INTRODUCTION: Vaginal delivery out of a breech presentation in pregnancies at term are being re-implemented into clinical practice. Still, recommendations regarding exclusion criteria leading to caesarean sections are based on expert opinions, not on evidence-based guidelines. The difference in perinatal outcome and course of delivery in births with babies in frank breech position and babies in incomplete or complete breech presentation never has been investigated in a large patient cohort. OBJECTIVE: To compare perinatal outcome of vaginally intended breech deliveries between births out of f

The effect of mode of delivery on neonatal mortality in very low birthweight infants born in Victoria, Australia: Caesarean section is associated with increased survival in breech-presenting, but not vertex-presenting, infants.

To determine whether the improved survival of very low birthweight (VLBW) infants (< 1500 g) born in Australia can be attributed to currently high rates of Caesarean section, we examined the associations between neonatal mortality and Caesarean section in singleton liveborn VLBW infants (500-1499 g) born during 1986-93 in Victoria, Australia, using data from the Victorian Perinatal Collection Unit. The infants included in this study had completed > 23 weeks of gestation, had no life-threatening malformations and had not been delivered by a repeat Caesarean without a trial of labour (n = 2763).

A CARE-compliant article successful salvage of bilateral perinatal testicular torsion after delivery out of breech presentation: A case report and review of the literature.

RATIONALE: Neonatal testicular torsion is a rare condition. There is no consensus about the optimal surgical treatment of neonatal testicular torsions. The salvage rate reported in literature remains poor. Discrimination of the onset of a neonatal testicular torsion seems to have an important impact of the salvage rate. Deliveries out of breech presentation have a risk for neonatal complications including neonatal genital birthtrauma. PATIENT CONCERNS: A newborn boy, who was delivered at 40 weeks of gestation out of breech presentation, showed shortly after birth, a painful discoloration and

The usefulness of universal ultrasound before hospital discharge for early diagnosing hip dysplasia in Korean neonates with breech presentation in a single center: A retrospective study.

Developmental dysplasia of the hip (DDH) is a broad-spectrum disorder. Early diagnosis and treatment are important for improved prognosis and a lower risk of long-term complications. Selecting high-risk infants is important for the early diagnosis of DDH using ultrasonography; however, there are no standard international guidelines. This study aimed to identify the usefulness of universal ultrasound before hospital discharge in breech-born neonates and proposes selective ultrasound for high-risk patients. A retrospective chart review was conducted to identify breech-born neonates who underwent

Association between planned mode of delivery and severe maternal morbidity in women with breech presentations: A secondary analysis of the PREMODA prospective general population study.

BACKGROUND: Neonatal morbidity among fetuses in breech presentation is not associated with planned mode of delivery in France. Data about consequences to these mothers are sparse. METHODS: The prospective PREMODA study took place in France and Belgium (2001-2002) in 138 maternity units and included all women with a singleton fetus in breech presentation ≥ 37 weeks of gestation (n=8105). We excluded women with more than one previous cesarean delivery, an in utero fetal death, or for whom cesarean delivery or induction of labor was planned due to maternal disease. The composite variable "severe

[Low birth weight is a negative factor in the mother-infant relationship. (A study of 122 women following primary Caesarean section for breech presentation or repeat Caesarean section.) (author's transl)].

The mother-infant relationship was studied in the first days post-partum in 122 women following Caesarean section for breech presentation and following repeat Caesarean section. 1. Birth weight With low birth weights women had more rarely a good relationship to the infant in the early days post-partum. In only 20% of women with children under 3000 grams the early relationship was good compared to a good relationship in 61% with infants over 3500 grams birth weights. 2. Parity Less primiparae had good contact to the children in the first post-partum days (35%) than secondi and multiparae (54%).

OptiBreech collaborative care versus standard care for women with a breech-presenting fetus at term: A pilot parallel group randomised trial to evaluate the feasibility of a randomised trial nested within a cohort.

OptiBreech collaborative care is a multi-disciplinary care pathway for breech presentation at term, with continuity from a breech specialist midwife, including where chosen, for vaginal breech birth (VBB). Pilot randomised trial using unblinded 1:1 parallel group allocation to OptiBreech versus standard care, within a cohort. Participants were women with a breech-presenting fetus > 33 weeks, at four sites in England, January-June 2022. A two-stage consent process was used. Participants consented to undergo random selection to be offered a 'new care process', with a choice to accept it, or not.

Media Representations of Breech Birth: A Prospective Analysis of Web-Based News Reports.

INTRODUCTION: Recent research has demonstrated that the media presentation of childbirth is highly medicalized, often portraying birth as risky and dramatic. Media representation of breech presentation and birth is unexplored in this context. This study aimed to explore the content and tone of news media reports relating to breech presentation and breech birth. METHODS: Google alerts were created using the terms breech and breech birth in online English-language news sites over a 3-year period from January 1, 2013, to December 31, 2015. Alerts were received daily and filed for analysis, and d

Physical theory as a basis for successful rotation of fetal malpositions and conversion of fetal malpresentations.

Maternal posturing, in which a pregnant or laboring woman assumes specific postures with the intention of altering the position and/or presentation of the fetus within the uterus, is a safe alternative to operative procedures recommended in cases of fetal malpositions and malpresentations. Clinicians and researchers are more likely to help pregnant women understand and perform posturing correctly if they comprehend the theoretical underpinnings of the technique. In maternal posturing, the forces of gravity and buoyancy combine to form a couple that overcomes frictional forces and/or obstructio

[Prolapse of umbilical cord - new aspects (author's transl)].

In an evaluation of more than 20,000 births, 39 recorded cases of prolapse of the umbilical cord were investigated in greater detail for the risk of perinatal mortality and morbidity. Particular attention was given to several variants of breech presentation in comparison to vertex presentations as well as to the time of rupture. The interval between diagnosis of prolapse of the umbilical cord and termination of birth was also in the focus of interest.-The authors concluded that the overall definition of "breech presentation" was quite inadequate and should be replaced, at least, by differentia

[Breech childbirth--obstetrical and antepartum ultrasonography findings].

The clinical and sonographic findings in 662 cases with birth from breech presentation were compared with the obstetric and sonographic data of a control group (births from vertex presentation). There were significantly more primiparae aged over 30 (16%) in the breech birth group investigated than in the control group (p less than 0.001). Vaginal bleeding and/or premature labor were found more frequently in gravidae with breech presentation than in those with vertex presentation (p less than 0.001). Also, premature births occurred significantly more frequently (22%) in the former group than in

Breech position, delivery route and congenital hip dislocation.

The purpose of this study was to investigate whether the mode of delivery for fetuses in breech presentation in any way influenced the frequency of congenital hip dislocation. In 13,559 singleton births 583 fetuses were in breech position, and the cesarean section rate was 39.1%. Eighty-three infants were born with congenital hip dislocation, 11 of whom had been in breech position. Of these 11, cesarean section was required in 5 cases. There was no difference in frequency of congenital hip dislocation between fetuses in breech presentation delivered by cesarean section vs. by the vaginal route

Long Bone Fractures Associated with Abdominal Breech Deliveries: A Report of Two Cases.

BACKGROUND: Birth-related injuries are a well-known complication of vaginal breech deliveries but are not anticipated during cesarean breech deliveries. However, long bone fractures in the neonate can result from delivery of the breech presentation by cesarean section. CASES: We report 2 cases of long bone fractures that occurred during cesarean section for breech presentation. No underlying bone pathology or systemic illness could be identified in either case. There were no other predisposing risk factors for fractures except breech presentation. Both infants were managed with cast applicatio
