Dysphonia associated with bulimia has been described in the literature associated with vocal fold edema and polypoid changes. Laryngopharyngeat reflux (LPR) has been documented to cause reflux vocal fold pathology including edema and polypoid changes. We studied eight singers with bulimia and documented vocal fold pathology, including edema, posterior commissure hypertrophy, ventricular obliteration, and telangiectasia. Reflux was demonstrated in all eight. The results of this study showed that LPR may be a contributing factor to vocal disorders in singers with bulimia.

OBJECTIVE: Two treatments for bulimia nervosa have emerged as having established efficacy: cognitive-behavioral therapy and antidepressant medication. This study sought to address 1) how the efficacy of a psychodynamically oriented supportive psychotherapy compared to that of cognitive-behavioral therapy; 2) whether a two-stage medication intervention, in which a second antidepressant (fluoxetine) was employed if the first (desipramine) was either ineffective or poorly tolerated, added to the benefit of psychological treatment; and 3) if the combination of medication and psychological treatmen…

OBJECTIVE: A cerebral function lateralization has been described in bulimic patients in positron emission tomography (PET) studies realized during a specific cognitive task. The purpose of this study was to evaluate, at rest, brain glucose metabolism in patients with bulimia nervosa.
METHOD: PET with (18-F)-fluorodeoxyglucose was used to evaluate cerebral glucose metabolism in 11 normal-weight bulimic girls compared to 11 age- and sex-matched healthy volunteers. Patients were diagnosed following DSM-IV and were off psychoactive medication.
RESULTS: In comparison with control subjects, bulimi…

Adolescents with bulimia nervosa present a challenge to pediatric nurse practitioners. These adolescents have a disorder that is secretive and difficult to diagnose, chronic, multifaceted, and difficult to treat. However, armed with the proper knowledge and skills, pediatric nurse practitioners are the ideal primary health care providers for this population.

The present study reports findings concerning the prevalence of obsessive-compulsive symptoms in eating disorders. Ninety-three women meeting DSM-III-R criteria for anorexia nervosa (AN) or bulimia nervosa (BN) were investigated using the Hamburg Obsession Compulsion Inventory HZI-K (Klepsch et al. 1993). Forty-six patients (49.5%) met the criteria for obsessive-compulsive disorder (OCD). This subgroup showed significant pathologically elevated scores on several scales of the Eating Disorder Inventory (Garner et al. 1983), and two different personality questionnaires (Freiburger Persönlichkeit…

Phosphorus abnormalities among patients suffering from eating disorders, although cited, have received relatively little attention. Studies generally report decreased concentrations of phosphorus for both bulimia and anorexia nervosa. We have recently noted hyperphosphatemia in several consecutive hospitalized bulimic patients who appeared to have normal renal function and calcium levels. Case files of 30 inpatient and outpatient female bulimic patients and 30 sex- and age-matched physically healthy psychiatric inpatients and outpatients were reviewed. Phosphorus and electrolytes related to ph…

Central pontine myelinolysis is classically described in alcoholics or other malnourished individuals after correction of severe hyponatremia. A patient with bulimia developed central pontine myelinolysis after correction of severe hyponatremia and hypokalemia. Early symptoms of abnormal affect and dystonia made initial diagnosis elusive. The spectrum of neurologic signs and possible causes of central pontine myelinolysis are discussed. A conservative approach to treatment of hyponatremia is advocated.

Despite the immense volume of literature on anorexia nervosa and bulimia nervosa, very few issues can be regarded as even near settled. Future research needs to concentrate on controlled population-based studies and replication studies to confirm or reject hypotheses that are supported by interesting preliminary results.

OBJECTIVE: The purpose of this study was to provide an estimate of the rate of relapse in patients with bulimia nervosa and to attempt to identify indexes of functioning before or after treatment that predicted subsequent relapse.
METHOD: The subjects were 48 female patients with bulimia nervosa who achieved symptom control while attending a specialized day hospital treatment program for eating disorders. The subjects were assessed before treatment, after treatment, and at a 2-year follow-up interview with respect to their eating symptoms and psychosocial functioning.
RESULTS: A relapse rate…

A number of compounds having an effect on serotonergic transmission have been shown to have a beneficial therapeutic action in patients with bulimia nervosa. Many previous studies, however, have suffered from methodological shortcomings, in particular small population sizes. This article reviews present data which suggest that fluoxetine is effective in the treatment of bulimia nervosa. Two trials of unprecedented size (n = 387 and 398, respectively) have recently been reported. These two studies show that fluoxetine, 60 mg daily, has a clear beneficial effect on behavioural and other (attitud…

The purpose of this paper is to examine the factors that influence treatment response and outcome in 39 patients with bulimia nervosa who were assessed during the course of 8 weeks of cognitive-behavioral therapy, and after an 8-week and 1-year follow-up period. The patients' progress was assessed using data gathered from clinical examination, structured interviews, and self-rating scales. Patients who had a poor clinical response at the end of treatment had greater pretreatment symptom severity, lower body mass index, and were more likely to have personality disorders. Poor response after 1 y…

Bulimia nervosa is an eating disorder found predominantly in young females and characterized by binge-purge cycles as well as medical and psychological problems. The patient with bulimia may present to the otolaryngologist with an upper aerodigestive complaint. It is of paramount importance that the otolaryngologist be cognizant of the disease in order to refer the patient for further therapy.

Little is known about prognostic factors in the treatment of bulimic patients. In the context of an ongoing study we looked at 1-year symptomatic outcome of 37 outpatients fulfilling DSM-III-R criteria for bulimia nervosa in relation to a variety of measures at first assessment. These included multiple measures of depression, measures of borderline personality disorder, a personality inventory, a symptom checklist and information about the history and the severity of the illness. Results add evidence to the importance of personality variables in predicting outcome: patients with high scores on…

In a review of the records of 454 patients with bulimia nervosa, 87 (19.2%) reported having a history of weight greater than 130% ideal body weight, although they were normal weight at evaluation. As contrasted to patients who had no history of having been overweight, this group of patients had a later onset of bulimia symptoms, weighed more and were more likely to be very dissatisfied with their current weight and to fear becoming overweight. The eating problems were similar in both groups.

Promising research data, relating hypnotizability and the possible presence of a dissociative mechanism in bulimic individuals, stimulated the present authors to incorporate hypnosis in their directive and multidimensional treatment of bulimic patients. Important strategies and how and when they can be applied in the different phases of treatment are described. In many cases, hypnotherapeutic techniques may enhance the effectiveness of cognitive behavioral and interactional strategies in the treatment of bulimic patients. In other instances, the incorporation of hypnosis may help both the ther

Nurses working with adolescents need an understanding of the clinical syndrome of bulimia in order to identify individuals at risk for this disorder and implement preventive and therapeutic interventions. The epidemiology, early precipitants, physiology, and biochemistry of bulimia nervosa are reviewed. From this integrated framework, implications for treatment are explored with an emphasis on holistic nursing care including health education.

Anorexia and bulimia nervosa are common in western civilized countries. They are frequently associated with disorders of the skin. Up to now, no dermatological studies of large numbers of patients with eating disorders have been published. We examined 145 patients with eating disorders (65 anorexia nervosa, 80 bulimia nervosa) regarding skin alterations. Simultaneously we controlled the moisture content and the superficial lipids of the skin. Several symptoms relevant to dermatology were found. In detail we report on: xerosis, hair changes, nail disorders, pathognomonic hand lesions, sialadeno…
