Complete occlusion of the anterior capsular opening by fibrocellular membrane associated with retained silicone oil: a clinicopathologic correlation.

BACKGROUND: To report the clinicopathologic features of a fibrocellular membrane in a pseudophakic eye with retained silicone oil in the absence of any capsular contraction syndrome, necessitating repeat anterior capsulorhexis. METHODS: Clinicopathologic report of a case. RESULTS: Histopathologic study of the membrane showed absence of any true periodic acid Schiff-positive capsule within the specimen. Clinically, there was no evidence of any anterior capsular contraction. The fibrocellular membrane may have occurred as a result of inflammatory and mechanical effects of silicone oil in the a…

Accumulation of milky fluid: a late complication of cataract surgery.

We describe 3 patients who presented with an accumulation of homogeneous milky fluid in the capsular bag several years after continuous curvilinear capsulorhexis, phacoemulsification, and posterior chamber intraocular lens (IOL) implantation. In each case, the entire edge of the anterior capsule opening was tightly attached to the peripheral IOL optic. The milky fluid was present in the closed chamber between the IOL optic and the posterior capsule. The fluid was sampled in 2 patients, and its concentration of sodium hyaluronate was determined by high-performance liquid chromatography. The con…

Comparison of endothelial cell loss and phacoemulsification energy during endocapsular phacoemulsification surgery.

We counted central corneal endothelial cells and measured corneal thickness in 64 human eyes before and three months after cataract extraction and posterior chamber lens implantation. Cataract surgery was the same in all patients and included capsulorhexis and posterior chamber phacoemulsification of the nucleus. Total phacoemulsification energy and time were recorded. A heparin-surface-modified intraocular lens or a regular poly(methyl methacrylate) lens, both having the same design and a 5.0 mm optic, was implanted in the capsular bag with the aid of sodium hyaluronate (Healon). Mean endothe…

The phacoemulsification learning curve: per-operative complications in the first 3000 cases of an experienced surgeon.

PURPOSE: To assess the per-operative complications occurring during the first 3000 phacoemulsification cases performed by an experienced con … nitial rate of vitreous loss was 4.0% in the first 300 cases falling to 0.7% in the last 300 cases. Capsulorhexis failure was the commonest per-operative complication observed, but the risk of subsequent posterior capsule rupture fell significantly from 9 of 45 (20.0%) in the first 100 cases to 1 of 49 (2.0%) in the next 2000 cases (p = 0.0061, Fisher's exact t …

Correlation of pupil diameter and intraoperative complications in patients with pseudoexfoliation undergoing phacoemulsification cataract surgery.

BACKGROUND AND OBJECTIVE: Study the correlation between pupillary diameter and intraoperative complications in patients with pseudoexfoliati … s 5.94 mm. Most complications occurred during the phacoemulsification phase (6; 66.7%), followed by capsulorhexis (2; 22.2%). Additionally, the presence of zonular weakness was significantly associated with the development of complications (P < .001). CONCLUSIONS: Preoperative identification of a pupil diameter <5.5 mm post dilation may alert the surgeon to po …

A Case Report of the Triple Procedure in a Patient - Penetrating Keratoplasty, Extracapsular Cataract Extraction, and Posterior Chamber Intraocular Lens Implantation.

The triple procedure, which includes penetrating keratoplasty (PKP), extracapsular cataract extraction (ECCE), and posterior chamber lens implantation (PCIOL), is a surgical preference and a powerful remedy for patients with corneal pathologies and cataracts. In the patient, the PCIOL is implanted into the capsular bag, and the usage of a closed anterior chamber technique is recommended. 10-zero nylon sutures are used in the ten to twelve o'clock positions, and the donor corneal button is added. Continuous curvilinear capsulorhexis (CCC) is accomplished with a capsulotomy inserted through the …

Laser-assisted cataract surgery in small pupils using mechanical dilation devices.

PURPOSE: To describe methods for performing laser-assisted cataract surgery in small pupils. METHODS: Iris retractors or a Malyugin Ring (M … orly dilating pupil constitutes a major challenge in cataract surgery, particularly when performing capsulorhexis. Femtosecond laser treatment can be employed with considerable benefit for the patient, even in these difficult cases.

Altered mechanical behavior and properties of the human anterior lens capsule after cataract surgery.

Several studies have quantified the mechanics of the normal lens capsule, motivated in large part by the need to understand better the mecha … the first time, that cataract surgery leads to a significant stiffening of the capsule nearest the capsulorhexis edge. These data promise to be important for developing predictive tools capable of elucidating interactions between the post-surgical capsule and implant.

Induction of cataracts of varying degrees of hardness in human eyes obtained postmortem for cataract surgeon training.

PURPOSE: To report a method of inducing nuclear cataracts of varying degree of hardness using intralenticular injection of the Karnovsky solution. METHODS: Twelve postmortem human eyes were prepared according to the Miyake-Apple posterior video technique. After performing capsulorhexis and hydrodissection/delineation, 0.2 ml of the Karnovsky solution was injected into the lens nucleus of the eyes. Manual extracapular cataract extraction techniques and various two-handed phacoemulsification maneuvers were then practiced at various time intervals after the injection. RESULTS: Uniform nuclear c…

[Combined laser instrumental extraction of congenital cataracts in children].

The authors have developed a new method for removal of congenital cataract in children, by taking into account its clinical forms, namely, combined laser instrumental extraction including YAG-laser anterior capsular rhexis, followed by removal of lens debris by the aspiration-irrigation technique. A differential technology of YAG-laser anterior capsular rhexis has been developed, by taking into account of age-specific features of the lens and eyes in children. The optimum energy modes and techniques have been defined for different clinical forms of congenital forms of congenital cataracts. An

Painting technique for staining the anterior lens capsule.

We describe a technique of capsule staining called capsule painting. Using a Khokhar capsule painting cannula, the dye is spread directly over the anterior capsule but to no other part of the anterior chamber. There is minimal turbulence in the anterior chamber as there is no need to replace the injected viscoelastic material. The use of minimal dye and viscoelastic material reduces the cost of surgery.

Conforming to the anatomy and not the standard: A technique of eccentric capsulorrhexis and intraocular lens haptic amputation for eyes with iris coloboma.

PURPOSE: To describe a novel technique for cataract surgery in patients with iris coloboma. METHODS: The technique involves 1) creation of an inferiorly displaced capsulorrhexis and 2) amputation of one intraocular lens (IOL) haptic, thus allowing for controlled IOL decentration in the direction of an inferior iris defect. RESULTS: We report favorable outcomes in two eyes (one patient) where eccentric capsulorrhexis and haptic amputation were employed during one-piece IOL repositioning in one eye and cataract surgery with three-piece IOL implantation in the contralateral eye. CONCLUSION: In

Anterior haptic flexing and in-the-bag subluxation of an accommodating intraocular lens due to excessive capsular bag contraction.

UNLABELLED: We describe the case of a patient who had cataract surgery with implantation of the hydrophilic acrylic Tetraflex accommodating intraocular lens (IOL), with subsequent development of capsulorhexis phimosis and in-the-bag IOL subluxation. Contraction of the capsular bag secondary to fibrosis resulted in significant anterior flexing of the lens haptic component. Explantation of the IOL-capsular bag complex was required 7 years after implantation. Histopathologic analysis demonstrated multiple areas of thick anterior subcapsular fibrosis. Pseudoexfoliative material was present through…
