To evaluate the incidence of low cardiac output in preterm infants with respiratory distress syndrome (RDS), we measured cardiac output, stroke volume, heart rate, mean arterial blood pressure, and systemic vascular resistance at 8-48 hours of age in 30 preterm infants with RDS who were dependent on inotropic support. We then compared them to 23 normotensive preterm infants with RDS and 27 preterm infants without RDS. RDS infants had a higher cardiac output and lower systemic vascular resistance and blood pressure than infants without RDS. Infants treated with dopamine and dobutamine had a hig

Patients coming to open heart surgery with advanced cardiac dysfunction may require mechanical cardiac support to avoid life-threatening low cardiac output in the postoperative period. 15 patients who could not be withdrawn from cardiopulmonary bypass because of low cardiac output were supported with a left heart bypass system (left atrium to ascending aorta). Ten were ultimately separated from the device, 6 were dismissed from the hospital and 4 remain well (the longest 2.6 years postoperative). A major asset of the device is that thoracic reentry is not required when support is discontinued.

The purpose of our study was to quantitate left ventricular outflow obstruction in the infant of the diabetic mother (IDM) through estimation of cardiac output by pulsed Doppler ultrasound. We evaluated 42 IDMs (White class B, C, or D) and compared them with two control groups, one similar in birth weight and the other similar in gestational age. Ventricular septal hypertrophy was found in 18 (43%) of 42 of the IDMs, but in none of the control infants. Morbidity increased with advancing septal thickness. None had hyaline membrane disease. Twenty-nine IDMs were asymptomatic, 11 of whom had sept

BACKGROUND: To test the hypothesis that low end-tidal carbon dioxide tension encountered during anhepatic phase in liver transplantation is related to hemodynamic status rather than ventilatory status, and can be used to predict the change in cardiac output during anhepatic phase.
METHODS: We retrospectively analyzed and compared data, included end-tidal carbon dioxide tension (ETCO2), arterial blood pressure, heart rate, central venous pressure, cardiac output, cardiac index, and stroke volume, before and after inferior vena cava clamping, and 0, 5, 10, 30 minutes during the anhepatic, and 5

Pediatric long-term ventricular support with paracorporeal assist devices is performed in only a few institutions. We report on our experience with two pediatric paracorporeal devices, which have been implanted in neonates, infants, and small children. Seven children with ages ranging from 2 weeks to 6 years and a body weight of 3 to 19 kg were provided with either a Medos or a BerlinHeart System. The underlying heart diseases included dilative cardiomyopathy (n = 3), endocardial fibroelastosis (n = 2), Ebstein anomaly, and status post redo aortic valve replacement (n = 1). All children were i

A 71-year-old male patient with end-stage heart failure, atrial fibrillation, congestive and low output symptoms, underwent biventricular pacing and atrioventricular junction ablation while anuric and hypotensive. Following atrioventricular junction ablation blood pressure increased by 20 mmHg during biventricular but not during right ventricular apical pacing. A rapid clinical improvement was observed and the patient was discharged from the hospital in NYHA functional class III.

Delayed sternal closure is an established method to overcome circulatory instability, especially in pediatric cardiac surgery. We describe the management of complications in a seven-year-old boy in whom staged chest closure augmented by sternal stenting was used twice within three weeks, once for right heart and once for left heart failure.

Hypoglycemic coma occurring 5 h after cardiac surgery in a nondiabetic adult patient was primarily triggered by: low cardiac output leading to severe hepatic functional derangement; metabolic acidosis, probably secondary to peripheral hypoperfusion; and possible inhibition of the adrenergic response due to anesthesia with fentanyl. Glucose infused at 8 g/h progressively increased the blood glucose level and paralleled hemodynamic improvement.

This reports concerns a 9 years old boy, who had situs solitus and atrioventricular concordance with double outlet left ventricle, pulmonary stenosis, tricuspid stenosis, ventricular septal defect, secundum atrial septal defects and hypoplastic right ventricle. Previous Blalock-Taussig shunt was created at the age of 11 months. Total correction consisted of closure of atrial septal defect and tricuspid valve orifice, and direct anastomosis of right atrial appendage to pulmonary trunk. Though atrial pacing was required for the immediate post operative period, the patient regained sinus rhythm 3

Our patient is a 65-year-old man with a history of hypertension, aortic stenosis, and end-stage renal disease on hemodialysis who presented with worsening dyspnea. On examination, he exhibited signs of volume overload and had a radiocephalic arteriovenous fistula (AVF) with a significantly palpable thrill. Coronary angiogram showed normal coronary arteries. Cardiac catheterization revealed a cardiac output of 10.6 L/min by thermodilution. Ultrasound of the AVF access demonstrated an abnormally high velocity with flow >5 L/min. The patient was diagnosed with high-output heart failure (HOHF) sec

BACKGROUND: High-flow hemodialysis arteriovenous fistulas (AVFs) can augment measured cardiac output and confound transplant hemodynamic assessment.
CASE SUMMARY: A 58-year-old man with end-stage heart failure and end-stage renal disease awaiting simultaneous heart-kidney transplantation had recurrent presyncope, volume intolerance, and ventricular ectopy despite preserved cardiac output on previous right heart catheterization with AVF occlusion using a blood pressure cuff. Repeat catheterization with duplex-confirmed manual AVF occlusion reduced AVF flow from approximately 2.8 L/min to <300

Chronic and recurrent obstruction of the inferior vena cava by the uterus during the second half of pregnancy are theorized to contribute to the causation of preeclampsia, fetal growth restriction, preterm birth, dysfunctional labor and postpartum uterine atony. Such obstruction is hypothesized to be asymptomatic for the mother but can be detected and minimized by non-invasive continuous trending of maternal cardiac output, because positional decreases in cardiac output can serve as a warning signal of obstruction of venous return. Injury may be caused by 1) decreased cardiac output, 2) increa

In 850 patients with acute myocardial infarction, cardiac output has been checked daily with acid-base-balance determinations, respiratory alcalosis being a sign of decreased cardiac output. In 84 patients without spontaneous increase of low cardiac output, treatment with cardiac glycosides has been started with the aim to prevent delayed cardiogenic shock: only 1 patient died in delayed cardiogenic shock and 3 patients in delayed heart failure. In the remaining 766 patients, only 1 patient died in delayed heart failure. With this successful prevention, the overall hospital lethality has dropp

PURPOSE: The clinical benefits, adverse effects, pharmacokinetics, and recommendations for the appropriate use of the aldosterone antagonists spironolactone and eplerenone in patients with heart failure are reviewed.
SUMMARY: Heart failure is a clinical syndrome characterized by the functional inability of the ventricle to meet the metabolic demands of the body. Renal hypoperfusion occurs as a result of reduced cardiac output, resulting in the activation of the renin-angiotensin-aldosterone system, which compensates for the hypoperfusion. However, this contributes to the pathology of the dise

Aldosterone exerts cardiovascular effects by influencing epithelial fluid and electrolyte excretion, and thus blood volume and pressure. Mineralocorticoid receptors (MR) are found in epithelial and non-epithelial tissues (vessel walls, heart, brain), with high affinity for aldosterone and physiological glucocorticoids cortisol and corticosterone. MR blockade by spironolactone or eplerenone favorably affects cardiovascular outcomes. In some situations (primary aldosteronism, experimental mineralocorticoid administration) activation of cardiovascular MR reflects aldosterone levels inappropriate

This review focuses on the role of the myocardial force-frequency relation (FFR) in human ventricular performance and how changes in the FFR can reduce cardiac output and, ultimately, can contribute to altering the stability of the in-vivo cardiovascular system in a way that contributes to the progression of heart failure. Changes in the amplitude, shape, and position of the myocardial FFR occurring in various forms of heart failure are characterized in terms of maximal isometric twitch tension, slope of the ascending limb (myocardial reserve), and position of the peak of the FFR on the freque
