Serotonin Signaling and Vascular Reactivity in Cardiometabolic Disease and Cardiac Surgery.

Serotonin (5-hydroxytryptamine, 5-HT) is a vasoactive amine and important regulator of vascular tone. Aberrant serotonergic activity contributes to vascular pathology in multiple cardiovascular disease states, including hypertension, diabetes, metabolic syndrome and chronic myocardial ischemia. Serotonin also plays a vital role in postoperative vascular dysfunction after cardioplegic arrest with cardiopulmonary bypass (CP/CPB). Perioperative microvascular dysfunction following CP/CPB manifests as an increased propensity for vasospasm or vasoplegia, with different effects in different tissue be

Impact of three-dimensional heart models on surgical planning and outcomes in congenital heart disease: a systematic review and meta-analysis.

BACKGROUND: Congenital heart disease surgery includes intricate structures that are perhaps hard to completely understand by traditional two-dimensional shapes. Individualised, centre-based three-dimensional (3D) heart models may enhance structural imaging and surgical preparation, while data on their medical effect continue to be unpredictable and uneven. This systematic review and meta-analysis assessed the impact of 3D heart simulations on surgical preparation and outcomes in congenital cardiac disease. OBJECTIVE: To assess the successful outcomes of patient-centred 3D cardiac simulations

Risk factors of delirium after gastrointestinal surgery: A meta-analysis.

BACKGROUND: Postoperative delirium is one of the common complications after any major surgery such as gastrointestinal surgery. And it is related to increased mortality and morbidity and other serious surgical outcomes. AIMS: This study aims to identify risk factors for postoperative delirium in patients undergoing gastrointestinal surgery. MATERIALS AND METHODS: Relevant studies published before August 2021 were searched on Pubmed, Embase and Medline. The risk of bias of included studies was assessed by Newcastle-Ottawa Scale (NOS). A random-effects model of DerSimonian-Laird was used to sy

The Reducer device in patients with angina pectoris: mechanisms, indications, and perspectives.

Despite available pharmacological and interventional therapies, refractory angina is a common and disabling clinical condition, and a major public health problem, which affects patients' quality-of-life, and has a significant impact upon health care resources. Persistent angina is common not only in patients who are not good candidates for revascularization, but also in patients following successful revascularization. Clearly, there is a need for additional treatment options for refractory angina beyond currently available pharmacological and interventional therapies. It is of pivotal importan

[Acute Leg Ischemia at Acute Aortic Dissection].

Legs ischemia might be associated with acute aortic dissection, in acute phase or even in subacute phase. In general, immediate central repair for acute aortic dissection is mandatory prior to intervention of leg ischemia because central repair might improve leg ischemia. Depending on condition of leg ischemia, revascularization of occluded artery should be considered. The perfusion to ischemic leg during central repair is useful to diminish leg injury. There are some useful modalities for revascularization such as some route of extra-anatomical bypass, placing of stent graft for closing entry

[Mitral valve replacement for a manifesting carrier of duchenne muscular dystrophy].

Duchenne muscular dystrophy (DMD) is an X-linked disease, but female carriers infrequently have some symptoms, who are called manifesting carriers. Here we report a case of a manifesting carrier of DMD with skeletal muscle weakness and cardiac abnormalities such as deterioration of cardiac function and left ventricular dilatation, who successfully underwent cardiac surgery. A 79-year-old female with acute heart failure for severe mitral regurgitation was admitted to our hospital. Surgical replacement of the mitral valve was performed under general anesthesia with intravenous anesthetics and no

Tricuspidization and reimplantation procedure for unicuspid aortic valve with annuloaortic ectasia.

Unicuspid aortic valve is a rare anomaly. We report 2 cases of successfully treated unicuspid valves with aortic dilatations by using a tricuspidization and reimplantation procedure. Two men, 35 and 39 years old, with severe aortic regurgitation and stenosis received this procedure. The sclerotic portion of a unicuspid valve, including 2 rudimentary commissures, was resected and reconstructed with autologous pericardium to create a tricuspid valve. An aortic root was replaced with a prosthetic graft. Postoperative echocardiogram showed trivial aortic regurgitation and minimal pressure gradient

The evolution of cardiovascular surgery in elderly patient: a review of current options and outcomes.

Due to the increase in average life expectancy and the higher incidence of cardiovascular disease with advancing age, more elderly patients present for cardiac surgery nowadays. Advances in pre- and postoperative care have led to the possibility that an increasing number of elderly patients can be operated on safely and with a satisfactory outcome. Currently, coronary artery bypass surgery, aortic and mitral valve surgery, and major surgery of the aorta are performed in elderly patients. The data available show that most cardiac surgical procedures can be performed in elderly patients with a s

Pharmacological and non-pharmacological therapy for arrhythmias in the pediatric population: EHRA and AEPC-Arrhythmia Working Group joint consensus statement.

In children with structurally normal hearts, the mechanisms of arrhythmias are usually the same as in the adult patient. Some arrhythmias are particularly associated with young age and very rarely seen in adult patients. Arrhythmias in structural heart disease may be associated either with the underlying abnormality or result from surgical intervention. Chronic haemodynamic stress of congenital heart disease (CHD) might create an electrophysiological and anatomic substrate highly favourable for re-entrant arrhythmias. As a general rule, prescription of antiarrhythmic drugs requires a clear dia

[Surgical treatment of aortic root disease].

We reviewed aortic root disease and operative procedures. Between January 1982 and December 2008, aortic root operation was performed for 58 patients with various aortic root disease. We chose Bentall type operations in extensive root destructive cases and urgent or reoperative cases. Overall in-hospital mortality was 8.6% (5/58). Four patients (7.5% of survivors) died during the period of followup. Actuarial survival at 15 years was 92%. Freedom from cardiovascular event at 10 and 15 years was 81% and 27%, respectively. Of 5 reoperations in 5 patients, only 1 was required due to complications

[Cardiovascular surgery for patients with chronic respiratory failure and respiratory dysfunction].

Chronic obstructive pulmonary disease is one of the major comorbidities in elderly patients with heart disease and thoracic aneurysm because of an overlap of risk factors. Although postoperative ventilator dependency is a major concern, recent study has suggested chronic obstructive pulmonary disease (COPD) was not a risk factor for postoperative prolonged mechanical ventilator support for off-pump coronary artery bypass grafting. Cardiopulmonary bypass may induce acute lung injury, but short-term cardiopulmonary bypass alone does not give adverse effect in clinical practice. Perioperative air

Necessity for evaluation of anomalous pulmonary venous return before percutaneous closure of atrial septal defects--a case report.

About 10%-15% of patients with an atrial septal defect will have some form of anomalous pulmonary venous connection. With the advent of percutaneous closure of atrial septal defects, it is imperative that presence of partial anomalous pulmonary venous return be excluded to prevent unnecessary percutaneous closure in patients who would ultimately require a surgical approach. The authors herein report a 29-year-old man who underwent percutaneous closure of 2 atrial septal defects but was later found to have his left upper pulmonary vein draining into the innominate vein via a vertical vein. He s

Surgical treatment of giant coronary artery aneurysm.

OBJECTIVE: Giant coronary artery aneurysm is an extremely uncommon disease. Most previous reports have involved only single cases. This report describes 6 patients with giant coronary artery aneurysm, examines its causes, and aims to establish the optimal surgical strategies for this exceptional and rare pathology. METHODS: From July 1996 to October 2004, a total of 30,268 patients underwent heart surgery at Fuwai Hospital in Beijing. Among these, 6 patients had giant coronary artery aneurysm diagnosed and underwent operation. Various surgical strategies were used for the operations of these

Double switch with reversed Patrick-McGoon for corrected transposition of the great arteries with double outlet right ventricle.

A 4-year-old boy diagnosed with a double outlet right ventricle with transposition of the great arteries in a discordant atrioventricular connection. The anatomical characteristics of the ventriculoarterial relationships resulting from leftward deviation were quite similar to a mirror image of those in the malformation reported by Patrick and McGoon. Hence, it was most appropriate to perform the double switch operation associated with Patrick-McGoon's intraventricular rerouting and Senning's atrial switch. This intraventricular rerouting could be performed without damaging the conduction syste

Successful implantation of a permanent pacemaker through a persistent left superior vena cava.

We present a case of successful implantation of a permanent pacemaker through an unusual course of a persistent left superior vena cava (PLSVC). A young male patient presented with symptomatic bradycardia and a heart rate of 35 beats per minute. The pacing lead was introduced through the standard left subclavian vein approach and was found to pass through an unusual course that was suspected to be a PLSVC. The lead was further advanced to the right atrium and positioned successfully in the right atrial appendage with excellent short-term and long-term pacing parameters. PLSVC should be suspect
