[Current MR imaging of cartilage in the context of knee osteoarthritis (part 2) : Cartilage pathologies and their assessment].

High-quality magnetic resonance (MR) imaging is essential for the precise assessment of the knee joint and plays a key role in the diagnostics, treatment and prognosis. Intact cartilage tissue is characterized by a smooth surface, uniform tissue thickness and an organized zonal structure, which are manifested as depth-dependent signal intensity variations. Cartilage pathologies are identifiable through alterations in signal intensity and morphology and should be communicated based on a precise terminology. Cartilage pathologies can show hyperintense and hypointense signal alterations. Cartilag…

Runx1 contributes to articular cartilage maintenance by enhancement of cartilage matrix production and suppression of hypertrophic differentiation.

Osteoarthritis (OA) results from an imbalance of the dynamic equilibrium between the breakdown and repair of joint tissues. Previously, we reported that Runx1 enhanced chondrogenic differentiation through transcriptional induction of COL2A1, and suppressed hypertrophic differentiation. Here, we investigated the involvement of Runx1 in OA development as well as its potential underlying molecular mechanism. When we analysed OA development in Col2a1-Cre;Runx1fl/fl and Runx1fl/fl mice by surgically inducing joint instability, Cartilage degradation and osteophyte formation of Col2a1-Cre;Runx1fl/fl …

Prospective Clinical Trial for Septic Arthritis: Cartilage Degradation and Inflammation Are Associated with Upregulation of Cartilage Metabolites.

Background. Intra-articular infections can rapidly lead to osteoarthritic degradation. The aim of this clinical biomarker analysis was to investigate the influence of inflammation on cartilage destruction and metabolism. Methods. Patients with acute joint infections were enrolled in a prospective clinical trial and the cytokine composition of effusions (n = 76) was analyzed. Characteristics of epidemiology and disease severity were correlated with levels of cytokines with known roles in cartilage turnover and degradation. Results. Higher synovial IL-1β concentrations were associated with clini…

Diagnostic Approach for Cricoid Cartilage Tumors.

BACKGROUND: Tumors of the cricoid cartilage are rare, with chondroma and chondrosarcoma being the most common histologic types. Clinical presentation is often insidious and symptoms usually appear when the lesion enlarges significantly or invades surrounding structures. Differentiating benign chondroma from low-grade chondrosarcoma remains challenging due to overlapping imaging findings and nonspecific clinical presentation. Histopathological examination following biopsy remains the gold standard for definitive diagnosis. CASE PRESENTATION: We present the case of a middle-aged male patient wi…

Alar Cartilage Hematoma: A Case Report.

In maxillofacial trauma, the most affected structure is the nose due to its prominence on the face. Trauma to the nose can lead to fractures, whether bony or cartilaginous, soft tissue damage, or, less commonly, hematomas. The nasal septum is the typical site for hematoma development and is considered a surgical emergency. Another site for a hematoma is the alar cartilage; however, it is very rare. In this report, we present a rare case of an alar cartilage hematoma in a three-year-old female after facial trauma.

Costal cartilage mimics urate on DECT.

OBJECTIVE: To establish whether the material in costal cartilages isoattenuating to urate on dual-energy CT is, in fact, urate. MATERIALS AND METHODS: Under the Human Tissue Act, costal cartilage specimens from 12 donated embalmed cadavers were examined with DECT before and after storage in air, ethanol, and saline. Cadaveric tophi were examined as positive controls. The storage fluid was analyzed for urate. Raman spectroscopy was performed. The results were compared using Student t-tests and ANOVA. RESULTS: For cadavers 1-6, initial DECT isoattenuating volumes were 0.3-1.91 cm3 (mean 1.1). …

Porcupine inhibition enhances hypertrophic cartilage differentiation.

Porcupine (PORCN) is a membrane-bound protein of the endoplasmic reticulum, which modifies Wnt proteins by adding palmitoleic acid. This mod … use organ cultures. Also, LGK974 treatment of Danio rerio embryos induced expansion of craniofacial cartilage width together with the shortening of the body axis, which was consistent with a phenomenon occurring upon inhibition of non-canonical Wnt signaling. By combining PORCN inhibition with exogenous Wnt proteins activating either canonical/β-catenin (WN …

Cartilage-hair hypoplasia: A comprehensive review.

Cartilage-hair hypoplasia (CHH) is a rare syndromic inborn error of immunity, caused by variants in the noncoding RNA gene RMRP. The effects of RMRP deficiency are pleiotropic, affecting the ribosomal RNA processing, cell cycle, and gene regulation. Typical clinical manifestations of CHH include chondrodysplasia with short stature, hair hypoplasia, combined immunodeficiency, and anemia. In addition, individuals with CHH have increased prevalence of malignancies, Hirschsprung's disease, and autoimmunity. The only curative option for immunodeficiency or severe anemia in CHH remains hematopoietic…

Cartilage Plating in Flail Chest Fixation.

INTRODUCTION: Operative stabilization of flail chest has been shown to have several benefits over nonoperative management. Often, flail chest injuries will involve the anterior ribs and their associated costal cartilage. In certain cases, operative fixation with open reduction and internal fixation (ORIF) of anterior rib fractures involving the costal cartilage may be warranted. Currently, there is scant literature regarding the surgical approach and clinical outcomes of ORIF involving the costal cartilage. The purpose of this study is to describe the surgical approach and first reported clini…

Decorin regulates cartilage pericellular matrix micromechanobiology.

In cartilage tissue engineering, one key challenge is for regenerative tissue to recapitulate the biomechanical functions of native cartilage while maintaining normal mechanosensitive activities of chondrocytes. Thus, it is imperative to discern the micromechanobiological functions of the pericellular matrix, the ~ 2-4 µm-thick domain that is in immediate contact with chondrocytes. In this study, we discovered that decorin, a small leucine-rich proteoglycan, is a key determinant of cartilage pericellular matrix micromechanics and chondrocyte mechanotransduction in vivo. The pericellular matrix…

Mesenchymal stem cells for cartilage regeneration.

Cartilage injuries are typically caused by trauma, chronic overload, and autoimmune diseases. Owing to the avascular structure and low metabolic activities of chondrocytes, cartilage generally does not self-repair following an injury. Currently, clinical interventions for cartilage injuries include chondrocyte implantation, microfracture, and osteochondral transplantation. However, rather than restoring cartilage integrity, these methods only postpone further cartilage deterioration. Stem cell therapies, especially mesenchymal stem cell (MSCs) therapies, were found to be a feasible strategy in…

Orthobiologics for Focal Articular Cartilage Defects.

Focal chondral defects of the knee are extremely common and often result in pain, dysfunction, joint deterioration, and, ultimately, the development of osteoarthritis. Due to the limitations of conventional treatments for focal chondral defects of the knee, orthobiologics have recently become an area of interest. Orthobiologics used for cartilage defects include (but are not limited to) bone marrow aspirate concentrate, adipose-derived mesenchymal stem cells, platelet-rich plasma, and micronized allogeneic cartilage. Each of these products can be applied in the clinical setting, as an isolated…

Cartilage invasion patterns in laryngeal cancer.

The cartilaginous invasion determines the T and is one of the most common sources of mistake in tumor staging. Also it is of great importance when planning any therapeutic alternative. In the latest revision of the TNM classification a clear distinction is made between infiltration of cartilage without going through it, considered a T3 recently and that would be a T4 according to the previous classification, and those going through the cartilage, classified as T4a. While this classification makes the difference in depth of infiltration, it does not emphasize the extent of invasion. This paper …

Biochemical properties of tissue-engineered cartilage.

OBJECTIVE: Microtia is treated with rib cartilage sculpting and staged procedures; though aesthetically pleasing, these constructs lack native ear flexibility. Tissue-engineered (TE) elastic cartilage may bridge this gap; however, TE cartilage implants lead to hypertrophic changes with calcification and loss of flexibility. Retaining flexibility in TE cartilage must focus on increased elastin, maintained collagen II, decreased collagen X, with prevention of calcification. This study compares biochemical properties of human cartilage to TE cartilage from umbilical cord mesenchymal stem cells (U…
