[Results of laparoscopic implantation of peritoneal catheters]].

The advantages of miniinvasive surgical techniques were proved in a number of surgical operations. In the submitted article the authors evaluate five years' experience with laparoscopic implantation of Tenckhoff's catheter for peritoneal dialysis. The authors implanted in 1995-1999 catheters by the laparoscopic route in 34 patients. The most frequent complication was early leak of the dialysate along the catheter (41%). After modification of the surgical technique early leak was observed in 11% of the patients. Escape of the catheter from the lesser pelvis was observed in 14.8% patients. The a

Atrial thrombus and central venous dialysis catheters.

A native arteriovenous fistula is the first choice for hemodialysis access. Despite improved catheter designs and the use of internal jugular veins, thrombotic complications still occur when tunneled central venous catheters are used as an alternative. Although right atrial thrombus (RAT) is a well-characterized complication of long-term central venous cannulation, particularly when used for parenteral nutrition and chemotherapy in pediatric practice, only 9 reported cases previously have been associated with the long-term use of central venous catheters for hemodialysis. We report five cases

Femoral positioning of Tesio catheters for hemodialysis.

Vascular access may be of crucial importance in long-term dialyzed patients when traditional blood access fails. Long-term central vascular access devices are usually inserted in the internal jugular or subclavian veins but thrombosis may be the major factor limiting their long-term use. To solve this problem the Tesio caheter is one of the most commonly recommended tools for long-term use in RD patients, and is normally placed in the neck veins. In this study the femoral vein is indicated as an alternative site for positioning the Tesio catheter. The 'high' exit (abdominal) reported here pres

Outcome and complications of temporary haemodialysis catheters.

BACKGROUND: The use of temporary haemodialysis catheters is often complicated by mechanical or infectious complications. Risk factors for these complications and optimal management to reduce their incidence are largely unknown. METHODS: We conducted a prospective study of 105 haemodialysis catheters (79 subclavian, 26 jugular) inserted in 52 patients in order to identify patient outcomes and to analyse the effect of patient and catheter factors on the incidence of infectious complications by multivariate analysis. RESULTS: Fifty-nine per cent of catheters were removed for a suspected complic

Bacteremia associated with tunneled, cuffed hemodialysis catheters.

Bacteremia is a frequent complication associated with tunneled, cuffed, permanent catheters (PCs). The incidence, spectrum of infecting organisms, and optimal treatment for catheter-associated bacteremia (CAB) have not been clearly established. In this study, 101 chronic hemodialysis (HD) patients with PCs for blood access were prospectively monitored for infection during a 24-month period. Data recorded for each patient included the number of catheter-days, episodes of suspected bacteremia, blood culture results, method of treatment, complications, and outcomes. All patients with CAB were tre

Latex protein levels in external condom catheters.

The Food and Drug Administration (FDA) allows for labeling medical products that have reduced levels of total water-extractable latex protein. The standard test method of the American Society for Testing and Materials (ASTM) for analysis of latex protein in natural rubber and its products is a colorimetric assay with a precipitation step called the modified Lowry assay. In an analysis of latex external condom catheters, we have documented a significant difference in protein levels between two brands of external condoms. The modified Lowry assay is a significantly less specific method than is a

Peripherally inserted central catheters in general medicine.

OBJECTIVE: To report the success rate and complications associated with peripherally inserted central venous catheters (PICCs) and to compare costs between PICCs and centrally inserted central catheters. MATERIAL AND METHODS: We undertook a cohort study of the first 1,000 patients referred to the PICC service of a large tertiary-care, university-affiliated, community hospital. The data were analyzed for insertion success rate, insertion mode, complication rate, successful completion, insertion costs, and applicability of PICCs in "high-risk" groups (transplant, human immunodeficiency virus-in

Midline and midclavicular catheters. Intravenous Nurses Society.

The Intravenous Nurses Society (INS) recognizes the need for uniform terminology for midline and midclavicular catheters to encourage standardization for indications, care, and maintenance strategies for these devices. It also recognizes the need for recommendations regarding the choice, use, management, and discontinuation of midline and midclavicular catheters to promote positive patient outcomes and enhance patient comfort, safety, and satisfaction.

Spontaneous migration of subcutaneous central venous catheters.

Along with the increasing use of central venous catheters have come an increasing number of complications. Although many are discovered at the time of insertion, others can occur at a later time. If unrecognized, problems may ensue. We describe two cases of spontaneous migration of subcutaneous venous access catheters to illustrate the importance of early recognition and treatment. In one case, the patient was asymptomatic at the time the migration was discovered, and the catheter was removed. However, in the second case, the patient required hospitalization for sepsis following clot formation

Intraperitoneal catheters: percutaneous placement with fluoroscopic guidance.

PURPOSE: The authors reviewed their experience with percutaneous placement of catheters into the peritoneal cavity for the administration of intraperitoneal chemotherapy to determine if their approach resulted in a lower complication rate than the reported 12%-16% rate and to demonstrate the technical advantages over surgically placed catheters. PATIENTS AND METHODS: Seventy-six patients with gastrointestinal or gynecologic malignancies underwent 152 procedures during a 20-month period. The catheters were used to deliver antineoplastic agents and, in some patients, to drain ascites. Catheter

[Placing of Hickman catheters under ultrasonic guidance].

We describe a new technique using ultrasound guided puncture of the subclavian vein and fluoroscopic control of the guide wire and the catheter position. Using this technique we performed 70 catheter introductions in 54 patients at the radiological department of the University Hospital of Rotterdam. Long term administration of chemotherapy was the most frequent indication for introduction of the Hickman catheter. The puncture related complication rate was compared with the complications of the 'blind' percutaneous puncture method, as mentioned in literature. All the ultrasound guided punctures

Intranasal balloon catheters: how do they work?

Despite there being a wide range of different epistaxis balloons their mode of action is unknown. Manufacturers' drawings depict balloons neatly filling the nasal cavity or precisely occluding the anterior and posterior nasal apertures. In view of the complexity of nasal internal anatomy these explanations seem unlikely. A better understanding of the configuration of these devices within the human nasal cavity may explain their mode of action and complications. This study aimed to delineate radiographically the configuration of 3 commonly used balloons within the noses of 4 human cadavers and

Positioning central venous catheters--a prospective survey.

This paper reports the results of a prospective survey of 266 attempted central venous catheterisations by various routes, evaluating their success rate and incidence of immediate complications and attempts to demonstrate a relationship between patient height in centimetres (H) and ideal catheter length. The overall rate of intrathoracic placement was 230 from 239 catheterisations (96%) after 266 attempts (86%). Of these 230 catheters, 54 terminated in the right atrium (24%). To avoid right atrial placement with its well documented risk of cardiac tamponade, it is recommended that right infrac

Double-J ureteral catheters in gynecologic surgery.

When ureteral injury is identified and properly managed, the gynecologic surgeon can expect good surgical and functional results with little or no patient discomfort or morbidity. The double-J silicone ureteral catheter is a useful surgical tool in this situation. The diameter of the ureteral catheter should be 7 or 8 French in most gynecologic surgical settings. It is better to select a length that is several centimeters too long. Intraoperative and postoperative care, including extraperitoneal drainage of the operative site, antibiotics, transurethral bladder drainage, and selection of the d
