CAVE-Onc: Graph-constrained agentic validation for cross-domain contradictions in CDISC oncology submissions.

The CDISC Rules Engine (CORE) provides industry-standard validation for Study Data Tabulation Model (SDTM) submissions, yet its imperative, domain-scoped architecture cannot express cross-domain contradictions-such as a Response Evaluation Criteria in Solid Tumors (RECIST) 1.1 overall response that contradicts the combined target lesion, non-target lesion, and new-lesion status-which pass structural validators undetected. We present CAVE-Onc, a two-layer graph-constrained agentic validation engine that augments CORE with declarative Shapes Constraint Language (SHACL) validation (L1) and a Lang

Study on the impact of stress effects on gas extraction in thick inclined coal seam caving faces.

In order to study the development of overburden fractures and the distribution characteristics of the “three zones” in extra-thick coal seams, a rota-table physical similarity simulation test platform was used to analyze the deformation and failure characteristics of overburden during the mining of extra-thick coal seams. The displacement patterns of key layers were measured under different excavation distances. A statistical analysis of the overburden fracture distribution was conducted, revealing the evolution characteristics of fracture development in the overburden with increasing advancin

Axonal Guillain-Barré Syndrome After Leptospirosis Contracted in a Cave or While Packrafting-A Case Report.

A 64-y-old male went caving and packrafting in Chiapas, Mexico. On return, he developed fever, headaches, and nausea and was treated empirically for leptospirosis with ceftriaxone and doxycycline. Convalescent IgM titers returned positive for leptospirosis. Other work-up was negative. As the symptoms of leptospirosis abated, the patient developed an areflexic quadriparesis. Nerve conduction velocities confirmed acute motor axonal neuropathy consistent with Guillain-Barré syndrome. Serologic work-up disclosed anti-GD1a antibodies. He was treated with intravenous immune globulin for 5 d and then

Primary Circumscribed Meningeal Melanoma Involving the Meckel's Cave: A Report of a Rare Case and Review of Literature.

Primary intracranial meningeal melanomas are rare. Diagnosing primary meningeal melanomas mostly involves comprehensive assessment through clinical and radiological means. This evaluation should encompass a detailed dermal and ophthalmic examination. Any suspicious lesion must be biopsied and examined microscopically. This is crucial not only to differentiate primary intracranial melanoma from other brain tumors but also to rule out metastases from potential sources of primary cutaneous or non-cutaneous melanomas. Surgery is considered the mainstay of treatment. Despite melanomas being general

Comparative Analysis of Surgical Working Corridors for Meckel Cave Trigeminal Schwannomas: A Quantitative Anatomic Study.

BACKGROUND AND OBJECTIVES: Volumetric analysis of the working corridors of the interdural approach to the Meckel cave may lead to a selection of routes which are anatomically more advantageous for trigeminal schwannoma resection. The herein-reported anatomic study quantitively compares the infratrochlear (IT) transcavernous, anteromedial (AM), and anterolateral (AL) corridors, highlighting their feasibility, indications, advantages, and limitations. METHODS: Anatomic boundaries and depth of Meckel cave, porus trigeminus, IT transcavernous, AM, and AL corridors were identified in 20 formalin-f

Neurovascular Structures at Risk During Surgical Access to Trigeminal Ganglion and Meckel's Cave - An Anatomical Study.

BACKGROUND: It is difficult to approach the Trigeminal Ganglion (TG) and Meckel's cave (MC) during surgeries. Therefore, the exact knowledge of the relationship of surgical landmarks to related anatomical structures is vital to reduce the associated postoperative morbidity. The aim of the present study was to enhance the knowledge of the surgical anatomy of structures that are present in the conduit of all surgical approaches to TG and MC, their distances from surrounding neurovascular structures, and their variations. MATERIAL AND METHODS: The study was carried out on 40 embalmed cadavers (E

Early Pleistocene Serotine Bat Eptesicus praeglacialis (Vespertilionidae, Chiroptera) from the Taurida Cave in Crimea.

The cranial and mandibular remains of a large serotine bat Eptesicus praeglacialis Kormos, 1930 are described from the Lower Pleistocene deposits of the Taurida cave in the central Crimea. This is the first finding of the skull material of E. praeglacialis and the first record of the species in Crimea. Judging by the tooth wear stages, the remains of both young and adult specimens are present in the taphocenosis. The small mammal tooth marks on the bones (caused by eating the remnants of soft tissues) in the absence of signs of digestion, characterizing materials from the predatory bird pellet

Distant metastasis of hepatocellular carcinoma to Meckel's cave and cranial nerves: A case report and review of literature.

BACKGROUND: Metastasis occurs as a late event in the natural history of hepatocellular carcinoma (HCC), and most patients die of liver failure attributed to the tumor supplanting the liver. Conversely, the brain is a less common metastatic site. CASE SUMMARY: We describe a rare case of hepatitis C virus-related multiple HCC metastasizing to the cavernous sinus, Meckel's cave, and the petrous bone involving multiple cranial nerves in an 82-year-old woman. At admission imaging studies including Gadolinium-ethoxybenzyl-diethylenetriamine pentaacetic acid-enhanced magnetic resonance imaging (MRI)

First report of paracoccidioidomycosis reactivation as a complication of immunosuppressive therapy for acute severe colitis in a caving enthusiast.

Treatment for ulcerative colitis often requires the administration of immunosuppressive therapy. Shortly after rescue therapy with infliximab for acute severe colitis, a patient who was also taking corticosteroids, azathioprine and adalimumab became rapidly unwell with atypical pneumonia, which did not respond to conventional antimicrobials. Re-examining the travel history revealed a prior caving trip to Costa Rica. Dimorphic fungal serology was thus tested and a diagnosis of paracoccidioidomycosis was made. After a lengthy intensive care unit admission, the patient made a recovery after the a

Neuronavigation-guided Endoscopic Endonasal Excision of Schwannoma-like Chordoma of Meckel's Cave: A Case Report.

Chordomas are locally aggressive malignant tumors due to their recurrence potential and originate from embryonic notochord remnants. Chordomas can originate anywhere on the axial skeleton. They are extradural and spread by bone destruction. Chordomas are locally aggressive tumors that invade the dura mater, and may also present with secondary intradural growth. The Meckel's cave location of chordomas has been very rarely reported in the literature. Chordomas located in Meckel's cave can be radiologically confused with trigeminal schwannomas. Herein, we report a case of Meckel's cave chordoma t

Trigeminal Neuralgia Caused by Dural Arteriovenous Fistula in Meckel Cave Treated with Gamma Knife Radiosurgery.

BACKGROUND: Classic trigeminal neuralgia is an often debilitating condition that frequently results from vascular compression at the root entry zone of the trigeminal nerve. We report a case of dural arteriovenous fistula surrounding the trigeminal nerve from near the root entry zone extending as far ventral as the Meckel cave treated solely with Gamma Knife radiosurgery. CASE DESCRIPTION: A 71-year-old woman presented with a history of persistent attacks of right facial pain after failed conservative management. Magnetic resonance imaging of the brain demonstrated dilated vasculature in the

Endoscopic endonasal transmaxillary transpterygoid approach to meckel cave: anatomical study and preliminary clinical results.

OBJECTIVE: Tumors involving Meckel cave (MC) always present challenges to neurosurgeons. We performed an investigation of the anatomical characteristics of the endoscopic endonasal transmaxillary transpterygoid approach to MC as an alternative to routine transcranial approaches and further confirmed its efficacy in the clinical setting. METHODS: Five adult fresh head specimens (10 sides) were studied to identify crucial anatomical landmarks and quantify the anatomical structures involved in the endoscopic endonasal transmaxillary transpterygoid approach. Two patients with a tumor involving th

Complex craniofacial changes in blind cave-dwelling fish are mediated by genetically symmetric and asymmetric loci.

The genetic regulators of regressive craniofacial morphologies are poorly understood. To shed light on this problem, we examined the freshwater fish Astyanax mexicanus, a species with surface-dwelling and multiple independent eyeless cave-dwelling forms. Changes affecting the skull in cavefish include morphological alterations to the intramembranous circumorbital bones encircling the eye. Many of these modifications, however, have evolved separately from eye loss, such as fragmentation of the third suborbital bone. To understand the genetic architecture of these eye-independent craniofacial al

Parasitism of prehistoric humans and companion animals from Antelope Cave, Mojave County, northwest Arizona.

Previously, we reported a tick recovered from Antelope Cave in extreme northwest Arizona. Further analyses of coprolites from Antelope Cave revealed additional parasitological data from coprolites of both human and canid origin. A second tick was found. This site is the only archaeological locality where ticks have been recovered. We also discovered an acanthocephalan in association with Enterobius vermicularis eggs in the same coprolite. This association shows that the coprolite was deposited by a human. This discovery expands our knowledge of the range of prehistoric acanthocephalan infectio

Meckel's cave tumors: relation to the meninges and minimally invasive approaches for surgery: anatomic and clinical studies.

BACKGROUND: Literature on tumors originating from Meckel's cave (MC) and their surgical treatment are scarce. OBJECTIVE: We present 37 cases of tumors originating from MC, the largest single-institution series reported thus far, and discuss the ideal surgical methods for each tumor type in relation to the normal and pathological anatomy of MC. METHODS: We studied 37 cases of surgery for tumors in MC (26 schwannomas, 7 meningiomas, 2 epidermoids, 1 hemangiopericytoma, and 1 dermoid) performed at our institution between 1986 and 2008. We excluded cases of large tumors of unknown origin, especi

Trigeminal Neuralgia Due to a Small Meckel's Cave Epidermoid Tumor: Surgery Using an Extradural Corridor.

Tumors at the petrous apex are associated with a variety of symptoms, which most often involve the trigeminal nerve. The authors present a rare case of a small epidermoid tumor in Meckel's cave that caused medically refractory trigeminal neuralgia. The surgical challenge associated with approaches to such lesions is discussed. The skull base tumor was excised completely through a small temporal craniotomy. The practicality of neuronavigation in reaching the petrous apex using a small extradural window is presented.

Recurrent arachnoid cyst of Meckel's cave mimicking a brain stem ischaemia. Report of a rare case.

UNLABELLED: A 44-years old man developed TIA-like symptoms with dysaesthesia around the mouth, vertigo and diplopia. MRI revealed a cystic space-occupying lesion on the right Meckel's cave, which spread out into cerebellopontine angle in a further examination. Therefore surgical exploration was performed using a suboccipital approach. An arachnoidal cyst was found and removed including its wall. About three months later the patient suffered again from dysaesthesias of the right side of the face and a new MRI revealed a recurrence of the lesion, with extension into the cerebellopontine angle, t
