Rebleeding of cerebral aneurysms has been previously reported during diagnostic angiography. However, capturing the exact moment of active rebleeding of a cerebral aneurysm during angiography is extremely rare. Here, a case of a rebleeding middle cerebral artery aneurysm during diagnostic digital subtraction angiography (DSA) was illustrated, accompanied with a video demonstration of the incident which is the only one in the literature. During the acquisition of lateral projection DSA images, active extravasation of the contrast medium was witnessed, indicating rebleeding. Simultaneously, ther

BACKGROUND: Contrast-induced encephalopathy (CIE) is a syndrome that may be clinically unrecognized and misdiagnosed as cerebral edema.
METHODS: Case report and review.
RESULTS: A 72-year-old woman was admitted for elective endovascular embolization of a 10-mm left anterior communicating artery aneurysm. One hour post-procedure, she acutely developed global aphasia. Emergent head computed tomography (CT) and computed tomography-angiography (CTA) showed high attenuation of the left hemispheric subarachnoid spaces interpreted as hemispheric edema; emergent magnetic resonance imaging revealed l

Transient global amnesia is considered a very rare complication of diagnostic cerebral angiography, and has been reported only in a limited number of case reports more than 15 years ago. We describe a patient experiencing transient global amnesia following cerebral digital subtraction angiography. While the condition by definition is self-limiting, its differential diagnoses may cause severe morbidity and/or mortality if left untreated. It is therefore important to build and maintain awareness of transient global amnesia as a possible complication of cerebral angiography.

Anastomosis of the carotid-anterior cerebral artery (ACA) is a rare anomalous vessel that arises from the internal carotid artery (ICA) at the level of the ophthalmic artery and takes an infraoptic and prechiasmatic path to anastomose with the ACA. It has known right-sided predominance. We report the case of an 83-year-old man with a left carotid-ACA anastomosis that was diagnosed by magnetic resonance (MR) angiography from the neck to the head during investigation of transient left hemiplegia. The right ICA was occluded at its origin. Our literature search revealed only six reported cases of

INTRODUCTION: Rarely in the anterior circulation, an anastomosis of the carotid and anterior cerebral arteries occurs when an anomalous branch arises from the ophthalmic segment of the internal carotid artery and anastomoses with the A1-A2 junction of the anterior communicating artery. Right-side predominance is known. To our knowledge, the incidence of carotid-anterior cerebral artery anastomosis has not been reported, so we researched cases in our institution records to determine incidence and investigated characteristic features of the condition on magnetic resonance (MR) angiography.

Therapeutic results with respect to lesion size were analyzed and compared in patients with hyperacute cerebral infarction with and without major artery lesions on magnetic resonance angiography (MRA) and in those who did and did not receive intravenous (IV) tissue plasminogen activator (t-PA). Of the patients with cerebral infarction who visited the hospital within 3 hours of onset between April 2007 and September 2009, 127 patients with cerebral infarction in the anterior circulation region in whom head magnetic resonance imaging (diffusion-weighted imaging [DWI]) or MRA was performed (81 me

BACKGROUND: In acute infarction involving the anterior circulation, the ipsilateral posterior cerebral artery (PCA) often appears to be longer than the contralateral PCA on magnetic resonance angiography. We described this finding as dominant ipsilateral PCA (DIPCA). We sought to find the frequency of DIPCA and its predictors.
METHODS: We recruited 164 consecutive individuals with acute infarct involving the anterior circulation, and analyzed their radiological and clinical features.
RESULTS: DIPCA was noted in 27 patients (16.5%). It was more frequent in patients with than in patients witho

BACKGROUND AND PURPOSE: Cerebral veins show wide variation, and unexpected veins occasionally limit surgical procedures for clipping of cerebral aneurysms. This study was undertaken to assess the utility of three-dimensional CT angiography (3D-CTA) in the evaluation of venous anatomy before surgical clipping of cerebral aneurysms.
METHODS: 3D-CTA was performed in 19 patients before pterional craniotomy. The venous anatomy visualized by use of 3D-CTA was compared retrospectively with intraoperative findings.
RESULTS: 3D-CTA clearly depicted the superficial sylvian veins (SSVs) in 17 of 19 cas

There has been no report on the use of spiral computed tomography angiography (CTA) in the investigation of intracranial cerebral artery stenosis. A prospective pilot study was conducted to investigate the feasibility of CTA in the diagnosis of intracranial occlusive disease and its correlation with transcranial Doppler. With transcranial Doppler, 10 patients with acute ischaemic stroke with middle cerebral artery stenosis or occlusion were identified. There were seven middle cerebral artery stenoses and five middle cerebral artery occlusions. The CTA confirmed all diagnoses by transcranial Do

100 patients undergoing digital subtraction cerebral angiography were randomized to have contrast delivered by either hand or mechanical injection. We compared the two modes of injection for image quality, the presence of vertebral reflux, possible complications and radiation exposure to the radiologist. There was no statistical difference between the two modes of contrast delivery for image quality and vertebral reflux. Only one definite temporary neurological complication was recorded. However, radiation exposure to the radiologist's hand and body were reduced by up to 70% by using a mechani…

BACKGROUND AND PURPOSE: Cerebral vasoreactivity is an important indicator of the reserve capacity of the cerebral circulation. To make a quantitative analysis of cerebral vasoreactivity in individual major arterial territories, we evaluated the response to acetazolamide using three-dimensional time-of-flight magnetic resonance angiography.
METHODS: We examined 10 healthy volunteers and 6 patients with unilateral stenosis of the middle cerebral artery by a 1.5-T superconducting magnetic resonance imaging system. After a baseline vascular image was obtained, each subject received 17 mg/kg IV of

A report is given on pathomorphic local findings in the cervical vessels and cerebral complications after direct (percutaneous) carotid angiography (d.c.a.). The studies were based on 1345 d.c.a. in 1199 patients of our establishment carried out in the period from July 1966 to the end of 1981. 112 d.c.a. of 102 decreased and dissected patients were pathomorphically evaluated. Of these, 12 cases showed massive angiographically produced findings in the cervical vessels, five of them with verified cerebral complications. In the cervical vessels, both A. carotis communis and V. jugularis may be af

BACKGROUND: Spinal cord injury (CIS) is an exceedingly rare complication of cerebral angiography, with only nine cases since 1962. Pathogenesis remains poorly understood.
CASE PRESENTATION: A 67-year-old diabetic woman presented with dizziness and normal neurological examination. During cerebral angiography, repeated injections of ioversol contrast medium (total 100 mL; approximately 20 min at a single site) were performed into the right subclavian artery for vertebral artery imaging due to vascular tortuosity. The angiography showed normal cerebrovascular morphology, without emboli, stenosis…

BACKGROUND AND PURPOSE: Endovascular thrombectomy is now the standard of care for large vessel occlusion acute ischemic stroke. However, acute stroke due to medium-vessel occlusions often result in unfavorable outcomes, and guidelines for thrombectomy are lacking. Moreover, nearly all clinical data and thrombectomy trials are based on biplane angiography systems. This study aims to compare the safety and efficacy of stroke thrombectomy procedures performed on single-plane versus biplane angiography systems in patients presenting with medium-vessel occlusions of the middle cerebral artery.

PURPOSE: The thalamo-mesencephalic (TM) branches of the posterior cerebral artery (PCA) supply critical structures. Previous descriptions of these vessels are inconsistent and almost exclusively rely on cadaver studies. We aimed to provide a neuroradiological description of TM vessels in vivo based on routine 3D rotational angiographies (3D-RA).
METHODS: We analyzed 3D-RAs of 58 patients with pathologies remote from the PCA. PCA-origins were considered. Delineation, origin and number of branches of the collicular artery (CA), the accessory CA (ACA), the posterior thalamoperforating artery (PT

BACKGROUND: Angiogenesis and nascent blood vessel formation is thought to be important in cerebral arteriovenous malformation (cAVM) development and maintenance, of which little is known. Digital subtraction angiogram (DSA) features of angiogenesis in cAVMs are poorly described and the aim of this study was to describe the frequency of angiogenesis in patients who had a DSA showing a cAVM. We also sought to evaluate the intra- and inter-observer agreement of a diagnosis of angiogenesis and explore which angioarchitectural features were associated with angiogenesis.
METHOD: Patients that under
