Two features of the atlas vertebra simulating fractures by tomography.

The bipartite superior joint surface and the pseudonotch of the atlas, although well described in anatomic and radiologic literature, may be mistaken for fractures by tomography. The first may appear as a trough on the superior articular surface of the lateral mass of the atlas. The second, the normal tubercle for insertion of the transverse ligament of the atlas, may look like a separate ossicle or a chip fracture.

Management of acute traumatic atlas fractures.

OBJECTIVE: A prospective review of a clinical series was performed. The treatment features of atlas fractures with and without associated axis injuries were investigated. METHODS: Twenty-nine patients were investigated. RESULTS: No displaced fractures were treated with a cervical orthosis. Patients with displaced fractures were managed with a halo vest immobilization; 96.4% patients had a solid fusion at their last follow-up evaluations. CONCLUSIONS: Isolated not displaced or combined with not displaced axis fractures atlas fractures can be treated effectively with a rigid cervical collar a

CT morphometric analysis of the cervical spinal canal with special reference to the correlation with the vertebral level.

BACKGROUND: Narrowness of the spinal canal is associated with the development of cervical myelopathy. While studies have addressed the sagittal diameter of the cervical spinal canal, few evaluated the correlation between the size of the spinal canal and the vertebral level. We addressed this issue. METHODS: Our retrospective study included 102 patients with cranial or spinal disorders. We examined the correlation between the cervical spinal canal diameter (SCD) at C1 to C7 and the inner anteroposterior diameter (IAPD) of the atlas on CT images. RESULTS: At C1, the SCD was largest, at C4 it w

Atlas Fractures: Diagnosis, Current Treatment Recommendations, and Implications for Elderly Patients.

Fractures of the C1 vertebrae (atlas) are commonly the result of falls and other trauma, which cause hyperextension, or axial compression of the cervical spine. Although historically thought as a benign injury with lower neurological risks, current data suggests that this may not hold true for geriatric patients (aged 65 y and older) who may be predisposed to these fractures even after lower-energy trauma such as ground-level falls. Advancements in orthopedic trauma care has increased our diagnostic abilities to identify and manage patients with C1 fractures and other upper cervical spine trau

Possible Further Evidence of Low Genetic Diversity in the El Sidrón (Asturias, Spain) Neandertal Group: Congenital Clefts of the Atlas.

We present here the first cases in Neandertals of congenital clefts of the arch of the atlas. Two atlases from El Sidrón, northern Spain, present respectively a defect of the posterior (frequency in extant modern human populations ranging from 0.73% to 3.84%), and anterior (frequency in extant modern human populations ranging from 0.087% to 0.1%) arch, a condition in most cases not associated with any clinical manifestation. The fact that two out of three observable atlases present a low frequency congenital condition, together with previously reported evidence of retained deciduous mandibular

Biomechanical comparison of the pullout strengths of C1 lateral mass screws and C1 posterior arch screws.

BACKGROUND CONTEXT: Conditions of the atlantoaxial complex requiring internal stabilization can result from trauma, malignancy, inflammatory diseases, and congenital malformation. Several techniques have been used for stabilization and fusion. Posterior wiring is biomechanically inferior to screw fixation. C1 lateral mass screws and C1 posterior arch screws are used for instrumentation of the atlas. Previous studies have shown that unicortical C1 lateral mass screws are biomechanically stable for fixation. No study has evaluated the biomechanical stability of C1 posterior arch screws or compar

Aneurysmal bone cyst of the atlas: successful treatment through selective arterial embolization: case report.

OBJECTIVE AND IMPORTANCE: Aneurysmal bone cysts (ABCs) are benign and expansile osteolytic lesions that can occur in any location in the spine, including the craniovertebral junction. Aggressive resection followed by bone grafting has been the mainstay of treatment, with selective arterial embolization as a presurgical adjunct. Complete excision of these lesions at the craniovertebral junction is associated with high surgical morbidity. We report a case of successful treatment of an ABC of the atlas in a child with selective arterial embolization alone. CLINICAL PRESENTATION: A 10-year-old gi

Fractures of C1 and C2 in an infant gazelle: a model of the human fracture pattern.

Spontaneously occurring fractures of the anterior and posterior synchondroses of C1 and the superior facet of C2 of an immature mammal were analyzed. The posterior C1 injury showed comminution, with involvement of both sides of the synchondrosis. Just below the anterior C1 fracture was an incomplete, comminuted fracture of the C2 superior facet. These fractures, especially the C1 injury, appear similar to the radiographic injury patterns in skeletally immature humans. Presumably the histologic failure patterns in the child would also be similar.

Experimental study of atlas injuries. I. Biomechanical analysis of their mechanisms and fracture patterns.

Understanding injury mechanisms is important for the prevention, diagnosis, and treatment of spinal injuries. Using 10 fresh cadaveric human spine specimens of occiput to C3, clinically similar injuries of the atlas (C1) were produced with high-speed (4.4 m/sec) axial compression. The traumatic event was biomechanically monitored. The resulting injuries were studied with radiography, computed tomography, and a multidirectional instability test. The average compressive failure force was 3,050 N for specimens impacted in neutral posture (n = 437) and 2,100 N for those in extended posture (n = 28

Injury to the upper cervical spine: fracture versus congenital anomaly.

As skull and cervical spinal radiographs are being obtained routinely on head and neck injuries, more incidental anomalies of the neck region are being detected. The differentiation of congenital anomaly from fracture can be difficult. The importance of this, however, cannot be underestimated because of the dire consequences of a missed cervical fracture. A case of such an anomaly and its presentation following injury is presented.

Treatment of Jefferson fracture with a halo apparatus. Report of two cases.

Two patients with bursting fractures of the atlas vertebra are presented. The use of a halo apparatus as an effective alternative to bedrest and cervical traction in these patients is discussed. Polytomography was helpful in establishing an accurate diagnosis.
