[Post-radiotherapy brachial plexopathy and cervical myelopathy].

The case of a 58-year-old male who, after 47 months of treatment with cervical radiotherapy for labial carcinoma, developed brachial plexopathy and six months later cervical myelopathy is herewith presented. Involvement of both the brachial plexus and the spinal cord in the same patient secondary to radiotherapy is exceptional.

Unilateral cervical nerve injuries: brachial plexopathies.

Brachial plexus injuries are frequently encountered in sports. Classically, the symptoms include transient burning, pain, and occasionally weakness of an isolated upper extremity resulting from a peripheral nerve injury of varying degrees. Persistent symptoms or recurrent injuries may necessitate imaging such as electromyography. Medical personnel should be familiar with brachial plexus anatomy and the common nerve injury mechanisms of compression or traction. On-field assessment includes evaluation for potentially more serious neurologic injuries and possible return to play when symptoms have

Extraplexual neurotization of brachial plexus.

The authors reviewed 62 neurotizations of the brachial plexus in 71 patients performed between 1974 and 1989. The nerves used were the accessory, the motor or sensory branches of the cervical plexus, and the intercostals. Twenty-five suprascapular nerves, 19 musculocutaneous, 4 medial roots of the median nerve, and 12 lateral roots of the median nerve were neurotized. The authors concluded that useful results can be achieved using extraplexual neurotizations.

Rhabdomyolysis resulting in concurrent Horner's syndrome and brachial plexopathy: a case report.

This case report describes a 29-year-old male who presented with immediate onset of Horner's syndrome and ipsilateral brachial plexopathy after sleeping with his arm dangling outside a car window for 8 h. Outside workup and imaging revealed rhabdomyolysis of the left neck musculature. Subsequent electrodiagnostic testing and high-resolution brachial plexus magnetic resonance imaging at the authors' institution attributed the Horner's syndrome and concurrent brachial plexopathy to rhabdomyolysis of the longus colli and scalene musculature, which had compressed-and consequently scar tethered-the

Locating the cervical motor branch of the facial nerve: anatomy and clinical application.

BACKGROUND: Platysma innervation has various clinical applications. Although some patients have unsightly platysmal bands, others suffer from hyperkinetic motility disorders of the platysma that constitute both an aesthetic and functional impairment. Recently, the cervical motor branch has been used to reconstruct brachial plexus injuries. Dissection in this area can be precarious. The authors describe surface landmarks to help predict the location of the cervical motor branch of the facial nerve to more efficiently and safely operate in this area. METHODS: Sixteen fresh heminecks were dissec

Spinal root and plexus hypertrophy in chronic inflammatory demyelinating polyneuropathy.

MRI was performed on the spinal roots, brachial and lumbar plexuses of 14 patients with chronic inflammatory demyelinating polyneuropathy (CIDP). Hypertrophy of cervical roots and brachial plexus was demonstrated in eight cases, six of whom also had hypertrophy of the lumbar plexus. Of 11 patients who received gadolinium, five of six cases with hypertrophy and one of five without hypertrophy demonstrated enhancement. All patients with hypertrophy had a relapsing-remitting course and a significantly longer disease duration. Gross onion-bulb formations were seen in a biopsy of nerve from the bra

Documentation of brachial plexus compression (in the thoracic inlet) utilizing provocative neurosensory and muscular testing.

Diagnosis and documentation of brachial plexus compression in the thoracic inlet, "thoracic outlet syndrome", remains difficult because the syndrome complex overlaps that of patients with cervical disc disease, intrinsic shoulder pathology, and peripheral nerve compression. While traditional electrodiagnostic testing can identify cervical radiculopathy and the rare isolated lower trunk compression, it cannot identify brachial plexus compression in the thoracic inlet. In 2000, neurosensory testing with the Pressure-Specified Sensory Device (PSSD) was applied to this diagnostic dilemma, demonstr

Angiographically proven cervical venous engorgement: a possible concurrent cause in the pathophysiology of Hirayama's myelopathy.

The objective of this study is to discuss the possible role of cervical posterior epidural plexus engorgement during cervical flexion in the pathogenesis of Hirayama myelopathy. In Hirayama disease, MRI during neck flexion often shows that the posterior dura detaches from the posterior arches compressing the spinal cord. Autopsies demonstrated asymmetric changes in the anterior horns consistent with chronic ischemic damage, attributed to arterial insufficiency during flexion or to microcirculatory changes due to compression by the tight dura. In a 15-year-old patient with 5-year history of dis

Nerves and nerve plexuses of the human vertebral column.

The origin, distribution, and termination pattern of nerves supplying the vertebral column and its associated structures have been studied in the human fetus by means of an acetylcholinesterase whole-mount method. The vertebral column is surrounded by ventral and dorsal nerve plexuses which are interconnected. The ventral nerve plexus consists of the nerve plexus associated with the anterior longitudinal ligament. This longitudinally oriented nerve plexus has a bilateral supply from many small branches of the sympathetic trunk, rami communicantes, and perivascular nerve plexuses of segmental a

Overshunting-Associated Myelopathy (OSAM) in a Patient with Bilateral Jugular Vein Occlusion.

BACKGROUND: Overshunting-associated myelopathy (OSAM) is a very rare complication of ventricular shunt therapy, and only 11 previous cases have been reported in the literature. We report the successful surgical management of a case of OSAM in a patient with bilateral jugular vein occlusion and congenital hydrocephalus. CASE DESCRIPTION: A 45-year-old patient with shunt-dependent, congenital hydrocephalus presented to our department with an 8-year history of progressive tetraparesis and gait disturbance. The patient was wheelchair-dependent. A new magnetic resonance imaging scan of the head re

[Treatment of deafferentation pain by high-frequency intervention on the dorsal root entry zone].

From August 1980 to May 1981, high-frequency lesions of the dorsal root entry zone of the spinal cord were performed on 35 patients with chronic deafferentiation pains. Among them were 15 patients with traumatic transverse cord lesions, 5 with non-traumatic transverse lesions and 7 with cervical root tears or traumatic brachial plexus lesions, 6 with stump or phantom pain after amputation, and 1 each with sciatic paralysis or spinal arachnopathy. Treatment results were best in complete transverse lesions, cervical root avulsion and brachial plexus lesion, less so for stump or phantom pain of t

Pseudarthrosis of cervical rib: an unusual cause of thoracic outlet syndrome.

Thoracic outlet syndrome is uncommon in adolescence. Cervical rib fracture is an extremely rare cause of thoracic outlet syndrome in this age group. We report an unusual case of thoracic outlet syndrome in a 14-year-old girl caused by pseudarthrosis of the cervical rib. A magnetic resonance imaging scan showed significant compression of the brachial plexus by the pseudarthrosis mass. Excision of the cervical rib through a supraclavicular approach gave excellent results in this case.

Anatomic variations in origin and coursing of the transverse cervical artery: morphogenesis and relation to the high arch-shaped subclavian artery.

This case report presents an atypical transverse cervical artery with its detailed anatomy, morphogenesis, and association with the high arch-shaped subclavian artery. The atypical arteries, related arteries, and adjacent cervical and brachial plexuses were macroscopically examined in a 98-year-old Japanese female cadaver donated to The Nippon Dental University for medical education and research. The atypical deep branch of the transverse cervical artery originated from the internal thoracic artery and passed through between the C5 and C6 roots, in close contact with the C5 and C6 junction, to

Management guidelines for athletic injuries to the cervical spine.

The potential or actual involvement of the nervous system in injuries to the cervical spine requires that management must proceed with particular care. Prevention of further injury is the main objective. Proper management must begin with the removal of the athlete from the field, and the correct procedures for doing this are reviewed. Treatment procedures for the following injuries are discussed: (1) nerve root and brachial plexus neurapraxia; (2) acute cervical sprain syndrome; (3) cervical vertebral subluxation without fracture; (4) cervical fractures and dislocations; (5) cervical spinal co

Ganglia of the cervical part of the sympathetic trunk in the white rat. Sources of blood supply and angioarchitectonics.

Each ganglion of the cervical part of the sympathetic trunk in white rat is supplied by several arteries of diameter of 15--30 micrometers. The arteries arise from three sources: 1) large vessels adjacent to the ganglia, 2) vascular net of connective and muscular tissues surrounding the ganglion, 3) vessels of nerves and interganglionic rami. The vascular network of the cervical sympathetic ganglia was connected with the vascular plexus of the vagus nerve and cervical as well as brachial plexuses.

Delayed Onset and Prolonged Horner Syndrome in Two Children After Single-Shot UltrasoundGuided Infraclavicular and Subclavian Perivascular Brachial Plexus Blocks for Upper Extremity Surgery: Case Reports.

Horner syndrome is a known complication of cervical approaches to brachial plexus blocks due to local anesthetic-induced oculosympathetic paresis. It has rarely been described in relation to ultrasound-guided brachial plexus blocks in children. This syndrome is usually self-limiting but may cause apprehension to the child, parents, and the treating physicians until its resolution. We report delayed manifestations of Horner syndrome in 2 children following ultrasound-guided infraclavicular and subclavian perivascular blocks.

[Possibilities for reconstruction in brachial plexus paralysis: neurotization].

Reinnervation by nerves other than the brachial plexus are necessary if avulsion of the roots prevents neural reconstruction of the brachial plexus itself, or if functional recovery is unsatisfactory and further reconstruction is necessary for improvement. The foreign nerves mostly used today for these neurotizations are the accessory nerve, the deep cervical plexus, the intercostal nerves and, more and more, the contralateral C7 spinal nerve. Our own experimental studies have demonstrated future concepts for reconstructions in very desperate cases to improve the final functional outcome in th

A case of connected superior and middle trunks of the brachial plexus or with abnormal upper trunk.

During treatment of tumors of nerve sheaths, such as schwannomas and neurofibromas, neurosurgeons should be aware of variations and aberrant formations of brachial plexus. Variations of the branches of brachial plexus are common, but its variations in the level of the roots and trunks are rare. Variations regarding lower trunk were founded more frequent in previous studies. An unusual variant of the brachial plexus was found unilaterally during routine dissection of a 75-year-old male cadaver. It was observed that middle trunk was connected to superior trunk.
