Syphilis is a systemic sexually transmitted infection caused by the invasive bacterium Treponema pallidum. The diagnosis remains challenging since the disease is labeled as "the great imitator" due to its broad clinical and histopathological appearances. Due to a significant rise in its incidence rates, syphilis should always be considered as a differential diagnosis of any painless, newly developed, oropharyngeal ulcerated lesion. This case demonstrates the rare clinical appearance of a chancre on the tongue as the primary clinical sign of syphilis.

African trypanosomiasis of the Rhodesian variety occurred in an American who recently traveled to Tanzania. Skin findings included a fluctuant, indurated, tsetse-fly bite site (chancre) and a fleeting, erythematous, macular eruption. The diagnosis was confirmed by the presence of the organism in peripheral blood smears. The patient's condition responded to intravenous suramin. African trypanosomiasis should be included in the differential diagnosis of patients who have visited areas where this disease is endemic, and in whom the appropriate skin findings are present.

Syphilis has increased in Denmark and this "great imitator" disease is easily overlooked. Two case stories are presented; one case illustrates how a patient with classical symptoms of secondary syphilis was overlooked by doctors of several specialities for one year, and the other case is of a patient with a lesion on the penis presenting clinically, serologically and histologically as a syphilitic chancre, but it proved to be a rare chronic inflammatory condition known as a pseudotumor. It is emphasized that doctors in all specialities should increase their awareness of this disease.

Chancroid is caused by the bacterium Haemophilus ducreyi. It is a sexually transmitted disease causing a soft chancre with a necrotic base and purulent exudate. The incidence of this illness is very low in Denmark and is probably underestimated. The bacterium is very fragile in transport, and culture is often negative. The chance of demonstrating the bacterium is greatly enhanced by the use of molecular techniques. In this case, we report on a specific PCR test for H. ducreyi that was used to establish the diagnosis in a 40-year-old male.

Syphilis is a sexually transmitted infection by Treponema pallidum. Without antibiotic treatment syphilis lasts for several decades and may develop up to 4 different clinical stages. Usually, the disease begins with a distinct painless and indurated ulcer at the contact site: the primary chancre. An indolent regional lymph node swelling is usually associated with the syphilitic chancre. After spontaneous healing of the primary lesion and several weeks of latency, the clinical symptoms of secondary syphilis occur. Treponema pallidum bacteremia leads to common symptoms like fever and malaise, bu…

Syphilis is well known as a great mimic, however it is not recognized as a cause of cervical lymphadenopathy. We report a case of a 21-year-old man who presented with marked unilateral cervical lymphadenopathy. He had no evidence of oropharyngeal chancres, skin or genital lesions. Computed tomography (CT) and magnetic resonance (MR) images showed multiple cervical lymphadenopathy, and serologic tests for syphilis were positive. Syphilis should still be a factor for consideration in the differential diagnosis of cervical lymphadenopathy.

Non-gonococcal sexually transmitted infections (STIs) include chlamydia, syphilis, and chancroids. Chlamydia is the most common STI caused b … inflammation and tender inguinal lymphadenopathy. Chancroids can be differentiated from syphilitic chancres based on their appearance. In contrast to painless chancres, chancroids are painful. Ureaplasma urealyticum, Ureaplasma parvum, and Mycoplasma hominis are considered symbiotic bacteria. Infections caused by these bacteria are usually not considered …

A homosexual male, seropositive to HIV and with previously documented syphilitic infections, developed a slowly growing nodule on his left wrist (possibly a chancre) followed, after 2 months, by a few scattered, large papular lesions. Serological evidence of active syphilis was obtained and treponema were identified in the initial lesion by means of immunofluorescent staining. Treatment with both aqueous crystalline penicillin G and benzathine penicillin G healed the lesions. This represents a further case of an atypical presentation of early syphilis in an immunocompromised host.

Syphilitic balanitis is an uncommon rather underreported manifestation of primary syphilis initially described by Eugene Follmann and subsequently named after him. It is characterized by balanitis with or without a primary chancre and inguinal lymphadenopathy. Given its rarity, Follmann balanitis may be misdiagnosed with other causes of balanitis. Therefore, a strong clinical suspicion and awareness are crucial, particularly in the current resurgence of syphilis. With this background, we hereby report a case of Follmann balanitis in secondary syphilis, diagnosed based on clinical features, pos…

Parasitic diseases, once limited to developing countries, may be seen in the United States with increasing frequency as air travel links continents. When cutaneous manifestations of these diseases are a part of the clinical picture, dermatologists are often called upon to contribute to the diagnosis. Amebiasis may result in painful anogenital ulcers; African trypanosomiasis (sleeping sickness) often begins with the trypanosomal chancre at the site of parasitic entry, and American trypanosomiasis (Chagas' disease) most commonly presents as Romaña's sign. Epidemiology, pathophysiology, immunolog…

The study of six cases of MacLeod-Donovan chancre, 2 in Paris, 4 in Brest, in young men coming from the West Indies, showed after 15 days incubation, a balano-preputial lesion consisting of an oval-shaped granuloma, 1 to 4 cm diameter, raised, indurated, ulcerated, reddish-yellow, spontaneously painful, bleeding easily, accompanied in 50 p. 100 of cases by inguinal adenitis, due to secondary infection. Swabs, which should be repeated, showed Donovan bodies within numerous histiocytes. The inactivity of penicillin may be contrasted with the rapidly favourable effect of streptomycin, chloramphen…

Syphilis is a sexually transmitted infection caused by the spirochaete Treponema pallidum. Syphilis re-emerged worldwide in the late 1990s, and hereafter increasing rates of syphilis were seen also in Denmark. Currently, around 700 cases are diagnosed yearly in Denmark, and syphilis is mainly encountered among men, who have sex with men. However, because of the risk of congenital infection screening of pregnant women has been introduced. Syphilis gives rise to highly variable symptoms such as chancre, skin rashes and fever or latent infection. The recommended treatment is intramuscular adminis…

We report the circulating genotypes and the frequency of macrolide-resistance patterns among Treponema pallidum pallidum DNA isolated from syphilitic lesions from patients who attended 2 sexual health clinics in Lima, Peru. We implemented and used a molecular typing scheme to describe local T. pallidum pallidum strains. Among 14 specimens, subtype 14d/f was the most prevalent strain in 7 fully typed T. pallidum DNA specimens obtained from men who have sex with men and transgender women presenting with chancre-like lesions. No macrolide-resistance mutations were found in T. pallidum DNA from 10…

A 30-year-old Swedish woman visiting Gambia in 1975 acquired trypanosomiasis probably of the Gambian type. The first sign of her disease appeared as a trypanosomal chancre on her right thigh, soon followed by lymphadenitis in her right groin. One week later she became febrile and therefore hospitalized after returning to Sweden. The diagnosis was established by demonstrating numerous trypanosomes in a blood smear. 10 pentamidine injections (200 mg once daily intramuscularly cured her disease. The risk of acquiring trypanosomiasis in Gambia seems to be extremely small for tourists, but not quit…

BACKGROUND: Increasing numbers of reported primary and secondary (P&S) syphilis cases in the United States suggest the need for improved surveillance methods. An outbreak detection method using reported syphilis test results, which can be counted before the conclusion of a syphilis case investigation, could lead to timelier outbreak detection.
METHODS: The historical limits comparison method was used to compare the number of positive rapid plasma reagin results reported during 2011-2014 with data for the preceding 3 years. An outbreak alert was generated when the monthly count of positive rap

The acquired immunodeficiency syndrome (AIDS) appears to alter the course of syphilis and particularly neurosyphilis. We report the case of an HIV-positive patient with two CNS lesions due to vascular complications of neurosyphilis of the meningovascular type, 18 months after he had developed a penile chancre. On admission, CT scans were inconsistent with cerebral toxoplasmosis or CNS lymphoma. At necropsy, histopathological examination showed typical Heubner's endarteritis of the basilar and posterior cerebral arteries. In AIDS patients, superadded meningovascular syphilis must be suspected, …

A 20-year-old female patient is described who contracted syphilis in a rape attempt. She consulted a dermatovenereologist twice but the disease was not diagnosed, for despite the fact that the patient had hard chancre fragments on the posterior adhesion of the vagina, polyscleradenitis, abundant roseolar eruption on the trunk skin, horny palmar and plantar papular efflorescence, erythematous syphilitic tonsillitis, she was not examined nor serological tests were carried out. The condition was diagnosed as callosity and acute respiratory viral disease, antihistaminic and antibiotic drugs were p…

Syphilis, a sexually transmitted disease caused by Treponema pallidum, is widespread but most common in large cities. The various stages include primary, secondary, latent, tertiary (late), and congenital syphilis and neurosyphilis. Definitive diagnosis is made by finding spirochetes on a darkfield examination of material from a chancre or moist mucosal lesion. Serologic tests for syphilis in common use in the United States include the VDRL and the FTA-ABS. Penicillin is the drug of choice for all stages of syphilis, with tetracycline or erythromycin indicated in certain patients. Accurate and…
