Anxiety and Depression in Liver Disease: Pathways, Prevalence, and Implications for Care - a Review.

Anxiety and depressive symptoms and disorders are highly prevalent among individuals living with chronic liver disease, yet remain insufficiently recognised and addressed in hepatology care. Evidence supports a bidirectional relationship between liver and mental health: psychiatric conditions may contribute to behavioural and metabolic risk factors for liver disease, while chronic liver disease increases vulnerability to anxiety and depression through biological mechanisms, symptom burden, and stigma. These interactions adversely affect continuity of care, health-related behaviours, and diseas

Evidence-based pain medicine for primary care physicians.

The last several decades have seen a marked increase in both the recognition and treatment of chronic pain. Unfortunately, patients frequently misunderstand both the nature of pain and the best practices for its treatment. Because primary care physicians treat the majority of chronic pain, they are ideally situated to provide evidence-based pain care. The majority of the medical evidence supports a biopsychosocial model of pain that integrates physical, emotional, social, and cultural variables. The goal of this primer is to assist primary care physicians in their understanding of pain, evalua

Reimagining Territorial Rehabilitation: A Modern Framework for Physiotherapy Practice in Chronic Disease Management.

Background Italy is undergoing a profound transformation in the management of chronic diseases, driven by demographic aging, increasing multimorbidity, and the territorial reorganization introduced by Ministerial Decree 77/2022. Despite these developments, rehabilitation pathways remain constrained by regulations that require physician‑led access and repeated formal transitions between outpatient, community, and home‑based settings. This structure limits continuity, delays treatment initiation, and hinders the integration of physiotherapists, professionals whose role in chronic care is strongl

Increasing the capacity of community managed organisations to provide chronic disease preventive care to people with a mental health condition: protocol for a cluster-randomised controlled trial.

BACKGROUND: Comprehensive and routine provision of preventive care for modifiable risk factors within mental health community managed organisations (CMOs) is variable. METHODS: This protocol describes a planned cluster-randomised controlled trial to support the delivery of such care within a large Australian CMO. A model of care and implementation support strategies will be co-designed and delivered across six months, with 12 CMO sites randomised to intervention or control groups. The model of care referred to as 'The Live Well and Flourish Framework' will guide staff in intervention sites th

A cost analysis of a simplified model for HCV screening and treatment at a tertiary hospital in Zimbabwe.

BACKGROUND: The treatment of chronic hepatitis C virus (HCV) infection using directly acting antivirals was recently adopted in the treatment guidelines of Zimbabwe. The objectives of this study were to design a simplified model of HCV care and estimate the cost of screening and treatment of hepatitis C infection at a tertiary hospital in Zimbabwe. METHODS: We developed a model of care for HCV using WHO 2018 guidelines for the treatment of HCV infection and expert opinion. We then performed a micro-costing to estimate the costs of implementing the model of care from the healthcare sector pers

Delirium and Neuropsychological Recovery among Emergency General Surgery Survivors (DANE): study protocol for a randomized controlled trial and collaborative care  intervention.

Background: Delirium is a complex neuropsychiatric syndrome which consists of acute and varying changes in cognition and consciousness. Patients who develop delirium are at increased risk for a constellation of physical, cognitive, and psychological disability long after the delirium has ended. Collaborative care models integrating primary and specialty care in order to address patients with complex biopsychosocial needs has been demonstrated to improve outcomes in patients with chronic diseases. The purpose of this study is to evaluate the ability of a collaborative care model on the neuropsy

MRI-based machine-learning radiomics of the liver to predict liver-related events in hepatitis B virus-associated fibrosis.

BACKGROUND: The onset of liver-related events (LREs) in fibrosis indicates a poor prognosis and worsens patients' quality of life, making the prediction and early detection of LREs crucial. The aim of this study was to develop a radiomics model using liver magnetic resonance imaging (MRI) to predict LRE risk in patients undergoing antiviral treatment for chronic fibrosis caused by hepatitis B virus (HBV). METHODS: Patients with HBV-associated liver fibrosis and liver stiffness measurements ≥ 10 kPa were included. Feature selection and dimensionality reduction techniques identified discriminat

Transforming Care Models in Cystic Fibrosis: A Review.

Cystic fibrosis (CF) is a multisystemic, chronic disease that requires a large multidisciplinary team for effective treatment. Over the past 20 years, the landscape of cystic fibrosis care has evolved from an almost exclusively pediatric disease to both a pediatric and adult condition. The median age of cystic fibrosis patients is rising, and the number of adults with CF is also increasing. With new developments in cystic fibrosis care, patients' health and needs have changed, and therefore the care model of the cystic fibrosis team has also changed. The introduction of highly effective CFTR m

Psychosocial Underpinnings of Pain and Sleep Disturbance in Safety-Net Primary Care Patients.

OBJECTIVE: The aim of this study was to uncover possible psychosocial underpinnings of pain and sleep disturbance in a safety-net primary care sample. METHODS: Patients (n = 210) awaiting care in a safety-net primary care clinic waiting room completed measures of cynical hostility, social support, mental health, sleep disturbance, and pain. This study was cross-sectional and observational. RESULTS: A structural equation model suggested that higher cynical hostility was associated with lower social support, which in turn was associated with poorer mental health, which then corresponded with h

Predictive Modeling of Lapses in Care for People Living with HIV in Chicago: Algorithm Development and Interpretation.

BACKGROUND: Reducing care lapses for people living with HIV is critical to ending the HIV epidemic and beneficial for their health. Predictive modeling can identify clinical factors associated with HIV care lapses. Previous studies have identified these factors within a single clinic or using a national network of clinics, but public health strategies to improve retention in care in the United States often occur within a regional jurisdiction (eg, a city or county). OBJECTIVE: We sought to build predictive models of HIV care lapses using a large, multisite, noncurated database of electronic h

The Role of Psychology in Pediatric Concussion.

Postconcussive symptoms in children and adolescents may include cognitive, psychological, and behavioral changes. When symptoms become chronic they are often not able to be identified with standard medical evaluations. Physicians may find that these chronic symptoms are also resistant to traditional medical treatments. Postconcussive symptoms may be associated with the injury itself and/or secondary to the resulting psychological issues or stressors/changes following a concussion. It is important to conduct an extensive evaluation of psychological and nonorganic factors that may be contributin

Pharmacist-Led Full-Course Pharmaceutical Care Model in Allergic Bronchopulmonary Aspergillosis: A Case Report-Based Approach to Optimizing Multidisciplinary Management.

INTRODUCTION: The management of allergic bronchopulmonary aspergillosis (ABPA) is complicated by prolonged, multi-drug regimens that carry a high risk of multi-system adverse drug reactions (ADRs). This complexity underscores the need for integrated, continuous pharmaceutical care to optimize safety and efficacy, yet standardized models for such care are lacking. CASE PRESENTATION: We report the case of a patient with ABPA and comorbid chronic hepatitis B, who had a history of failed ABPA therapy. During the current therapeutic course, the patient not only sequentially developed steroid-induc

Reframing Chronicity in Research and Care: A Patient-Informed Framework of Pain Adaptation.

Chronic pain is a complex and heterogeneous condition shaped by interactions among biological processes, psychological responses, behaviors, environmental contexts, cultural and structural constraints. Yet many prevailing models remain static, insufficiently capturing how patient experiences evolve over time, particularly during the transition from acute to chronic pain. This conceptual paper advances a patient-informed conceptual framework developed through qualitative research conducted within a National Institutes of Health-funded study on patient preferences and appropriateness of care.
