Thrombophilia testing has limited usefulness in clinical decision-making and should be used selectively.

Management of venous thromboembolism (VTE) includes evaluation for hypercoagulable state, especially if the VTE occurs in young patients, is recurrent, or is associated with a positive family history. These laboratory tests are costly, and surprisingly, there is little evidence showing that testing leads to improved clinical outcomes. Evidence based on observational prospective studies suggests that optimal duration of anticoagulation should be based on clinical risks resulting in VTE, such as transient, permanent, and idiopathic or unprovoked risks, and less on abnormal thrombophilia values.

[Clinical thinking and decision making in practice. A severely ill elderly man with icterus].

A 74-year-old man was admitted to hospital because of jaundice and malaise of several weeks' duration. Five years earlier he had sustained a stroke from which he had recovered almost completely. On physical examination he was overweight and had an enlarged liver. Laboratory values were consistent with cholestasis and hepatitis. An abdominal ultrasound showed multiple nodular abnormalities in the liver consistent with a malignancy. Rapidly developing abnormalities in blood coagulation were thought to be a contraindication to hepatic biopsy. The patient deteriorated and sustained a new stroke. T

Development and Evaluation of a Shared Decision-Making Patient Decision Aid for Dental Clinic Care: A Mixed-Methods Study.

BACKGROUND AND AIMS: Shared decision-making (SDM) is increasingly recognized as a core element of patient-centered dental care, promoting collaboration between clinicians and patients to ensure that treatment decisions reflect both clinical evidence and individual preferences. However, few patient decision aids (PDAs) tailored to dental contexts have been systematically developed or evaluated. This study aimed to develop a dental-specific PDA and explore patients' perceptions of SDM following PDA-assisted consultations. METHODS: A mixed-methods design was employed in Taiwan, combining a quant

Individualized Prediction of Patient-Defined Treatment Success to Support Patient-Centered Clinical Decision-Making.

OBJECTIVE: Clinical prediction models often target generic thresholds for treatment success, such as the Minimal Important Change. The Personal Meaningful Gain was introduced as a personalized threshold, capturing the improvement an individual seeks to achieve in a self-chosen domain to become satisfied with the treatment results. This study developed and internally validated a treatment-conditional prediction model estimating the probability of achieving the PMG within 3 months in patients with common hand and wrist conditions. METHODS: This study used real-world data from 25 specialized han

Semaglutide modulates explore/exploit decision-making in major depressive disorder: A randomized clinical trial.

BACKGROUND: Explore/exploit decision-making is a crucial component of motivated behavior. Explore/exploit behaviors are related to disparate cognitive and reward-related functions and have been implicated in multiple neuropsychiatric conditions, including major depressive disorder (MDD). Several mediators of explore/exploit decision-making have been proposed; metabolic mechanisms have been theorized to potentially affect its underlying processes. Here, we examined the effects of a GLP-1 receptor agonist (GLP-1RA), semaglutide, on explore/exploit behavior in patients with MDD, using a four-arme

PSMA PET/CT improves precision diagnosis, treatment, and clinical decision-making in prostate cancer.

Prostate cancer is one of the most prevalent malignancies in males worldwide. Its diagnosis and treatment have long faced challenges, including inaccurate disease staging, difficulty in localizing recurrence sites, and a lack of evidence for individualized decision-making. Recently, prostate-specific membrane antigen (PSMA) positron emission tomography (PET)/computed tomography (CT) technology has emerged as a major breakthrough in addressing these limitations. This review systematically summarizes the technical principles and clinical applications of PSMA PET/CT, and evaluates its impact on d

Timing of Implant Placement in the Esthetic Area: Diagnostic Algorithm for Clinical Decision-Making.

Immediate implant placement in the esthetic area is a challenging surgical procedure. In the anterior maxilla, the esthetic outcome and the tissue stability over time are highly emphasized. A thorough preoperative analysis of the local site anatomy is essential to choosing the most suitable surgical and prosthetic protocol. Due to the heterogenous data from the literature, a diagnostic algorithm was developed to assist the clinician in the decision-making process for single-tooth implant rehabilitation in the anterior maxilla. The treatment options were based on the predictability of implant p

Intraoperative Transesophageal Echocardiography in Cardiac Surgery: Clinical Applications, Decision-Making, and Impact on Outcomes.

Intraoperative transesophageal echocardiography (TEE) has become a central component of modern cardiac surgery by providing real-time, high-resolution assessment of cardiac anatomy and physiology throughout the perioperative period. Beyond its traditional diagnostic role, intraoperative TEE directly influences surgical and anesthetic decision-making by enabling immediate detection of unexpected pathology, residual lesions, and hemodynamic instability. Accumulating evidence demonstrates that TEE-guided intraoperative assessment frequently leads to modification of surgical strategy or anesthetic

Early Intestinal Ultrasound Assessment Predicts Therapy Response: An Easy Tool for Clinical Decision-Making.

BACKGROUND: Transmural healing (TH) has emerged as a therapeutic target in Crohn's disease (CD), providing a more comprehensive indicator of deep remission than mucosal healing alone. Intestinal ultrasound (IUS) is a noninvasive method for assessing TH, but its prognostic value remains insufficiently defined. OBJECTIVE: The aim of this prospective study was to evaluate whether early improvement in IUS parameters during biological therapy could predict TH at 12 months. DESIGN: This is a prospective multicenter study enrolling CD patients initiating biological therapies. IUS and Doppler parame

Plasmablasts as Translational Biomarkers in Autoimmune Diseases: From Cellular Dynamics to Clinical Decision-Making.

B cells are key drivers of immune dysregulation across systemic autoimmune diseases. Among their progeny, plasmablasts occupy a uniquely revealing niche: short-lived, highly proliferative intermediates that mirror real-time B-cell activation. Their appearance in peripheral blood integrates antigenic stimulation, cytokine-driven differentiation, and aberrant germinal-center dynamics, transforming them into sensitive indicators of ongoing immunological activity. This review synthesizes current knowledge on plasmablast biology and highlights disease-specific phenotypes across systemic lupus eryth

Rheumatic Digital Twin: Proposed Machine Learning-Based Multimodal Framework to Inform Clinical Decision-Making.

Rheumatic diseases are chronic, immune-mediated conditions characterized by significant heterogeneity in presentation and disease course. However, current clinical approaches often rely on snapshot-based assessments that fail to capture the complex longitudinal evolution of these conditions. To address these limitations and support the implementation of precision medicine, we present the design for the Rheumatic Digital Twin, a novel, modular conceptual framework intended to integrate heterogeneous multimodal data, ranging from electronic health records and clinical notes to imaging and omics,

Orthopedic Mechanophenotype: A Structured Narrative Review of Multiscale Biomechanics, Mechanotransduction, and Clinical Decision-Making.

Orthopedic disorders are still classified mainly by anatomy or end-stage morphology, yet osteoporosis, osteoarthritis, fracture nonunion, spinal degeneration, and bone-implant interface failure share a common mechanobiological logic: external load is filtered by hierarchical tissue architecture, converted into cell-scale signals, and integrated with inflammation, metabolism, and repair capacity to determine whether adaptation or failure occurs. This structured narrative review synthesizes recent evidence on multiscale biomechanics, finite element analysis (FEA), mechanotransduction biology, ti

Management strategies and determinants of clinical decision-making in oroantral communication among dental practitioners.

INTRODUCTION: Oroantral communication (OAC) represents an abnormal connection between the oral cavity and the maxillary sinus, often arising after posterior maxillary extractions. Unmanaged OACs can lead to chronic sinusitis and fistula formation. Despite various treatment options, standardized guidelines are lacking. This study examined current management strategies, influencing factors, and practitioner confidence in treating OACs among dental professionals in the United States (U.S.). METHODS: A nationwide, cross-sectional online survey was conducted among 193 dental practitioners. The surv
