A female patient aged 28 years suffered from recurrent episodes of diarrhoea. Giardia lamblia was isolated once. By rigorous application of basic clinical skills, such as thorough history taking and physical examination, together with laboratory tests the correct diagnosis was established (in this patient systemic lupus erythematosus). Two important aspects of clinical reasoning, viz. returning to the patient when new information becomes available (cyclic way of working) and pattern recognition ensured a fruitful diagnostic process.

Physician cognition, metacognition and affect may have an impact upon the quality of clinical reasoning. The purpose of this study was to examine the relationship between measures of physician metacognition and affect and patient outcomes in obstetric practice. Reflective coping (RC), proactive coping, need for cognition (NFC), tolerance for ambiguity, state-trait anxiety and metacognitive awareness were assessed for obstetricians (n = 12) who provided intra-partum care to 4,149 women. Outcome measures included delivery mode and intrapartum asphyxia. Analysis was carried out using logistic reg…

PURPOSE: Large language models (LLMs) have demonstrated impressive performance across various medical examinations; however, their competency in subspecialty clinical reasoning, particularly in prenatal diagnosis, remains poorly characterized.
MATERIALS AND METHODS: A cross-sectional comparative study was conducted by using 125 questions from China's nationally standardized Prenatal Ultrasound Screening Qualification Examination. Three LLMs and two human physicians were evaluated. Questions were classified along three independent taxonomic axes: modality, disease system, and question format. …

Emergency Departments (ED) require rapid differentiation of time-sensitive critical conditions. We present a dynamic Clinical Decision Support System (CDSS) utilizing a Time-Aware Retrieval-Augmented Generation (RAG) framework. In a retrospective pilot study of 70 cases (Aortic Dissection, MI, Stroke, and Trauma Controls), the system's Top-1 diagnostic accuracy significantly improved from 69.5% (triage) to 86.7% (discharge). This validates the system's capability to integrate evolving FHIR data with clinical guidelines to reduce the diagnostic uncertainty.

Clinical reasoning is often described through partially independent frameworks, including causal explanation, probabilistic inference, ethical deliberation, and narrative understanding. What remains insufficiently clarified is how these dimensions are structurally integrated in the care of a particular patient. Drawing on Paul Ricoeur, this paper argues that clinical reasoning is best understood as a hermeneutic and practical movement structured by three interrelated arcs: an epistemological arc (from naive understanding, through explanation, to informed understanding), an ethical arc (from te…

OBJECTIVE: To investigate the efficacy of Mind Mapping-Virtual Simulation (MM-VSP) in enhancing clinical reasoning and procedural application competencies among residents.
METHODS: A retrospective study was conducted with second-year residents (n=80) during their obstetrics and gynecology (OB/GYN) rotations (January 2023 - December 2024). Participants were categorized to either the study group (n=40, receiving MM-VSP training) or the control group (n=40, receiving VSP training), all trained by a unified faculty team using identical clinical cases. Primary outcomes-theoretical knowledge, clini…

A 58-year-old man presented with progressive right-hand loss of dexterity and resting tremor over 2 years, accompanied by sensory symptoms in the distal extremities and chronic cough. Neurologic examination revealed asymmetric parkinsonism, distal hypoesthesia, and decreased vibratory sensation. Nerve conduction studies revealed a non-length-dependent axonal polyneuropathy. This case presents a structured approach to both parkinsonism and polyneuropathy, and the clinical reasoning necessary when dealing with multiple system involvement, in which different differential diagnoses should be consi…

A 10-year-old boy developed progressive tremor, insomnia, and autonomic dysfunction after an upper respiratory tract infection. Initial workup, including brain MRI, CSF analysis, and antibody screening, was unremarkable. By the third month of illness, brain MRI revealed multiple hyperintensities on fluid-attenuated inversion recovery sequences. Empiric immunotherapy led to partial clinical and radiologic improvement. However, neurologic deterioration continued despite subsequent immunosuppressive treatment, and follow-up imaging demonstrated progressive cerebral atrophy. 18F-fluorodeoxyglucose

A 38-year-old male patient with no personal or family history of neurologic disease presented with a 1-year history of chronic, progressive gait instability and predominantly right-sided involuntary jerk-like movements. A brain MRI revealed diffuse cerebellar atrophy on T2 fluid-attenuated inversion recovery and T1-weighted sequences. Laboratory and diagnostic investigations focused on identifying acquired etiologies. This case explores the differential diagnosis of myoclonus-ataxia syndrome, emphasizing its progression and the distinct underlying causes. It also presents a structured approach

A 50-year-old man presented with headache. Examination showed left sided ataxic hemiparesis and elevated blood pressure. Brain imaging revealed an acute intracerebral hemorrhage in the right lentiform nucleus, deep and periventricular white matter hyperintensities, and predominantly deep cerebral microbleeds. Fundus examination showed important arteriolar tortuosity involving several blood vessels. In this young patient, we explain the diagnostic approach to intracerebral hemorrhage, the causes of cerebral small vessel disease, and the interpretation of biomolecular tests.

Approaching patients with lumbosacral plexopathies or radiculoplexus neuropathies can be challenging and a well-defined strategy considering specific etiologies is necessary to arrive at the correct diagnosis. In this case, a 61-year-old man presented with a 10-year history of slowly progressive right lower extremity numbness and weakness. His examination was marked by right lower extremity weakness, decreased temperature/pinprick and proprioceptive/vibratory sensations, hypotonia, muscle atrophy, and absent right patellar, and ankle deep tendon reflexes. His workup was notable for electrodiag

Evaluating patients with neuropathy is common, especially in elderly patients with comorbidities. Neuropathy can often be a manifestation of systemic diseases, cancer, or drug-induced toxicity; thus, the differential diagnosis is challenging. The mechanism of nerve damage can vary significantly, affecting the patient's therapeutic management. We describe a 66-year-old woman who presented with subacute and progressively worsening motor weakness of the lower extremities with bilateral numbness and tingling after a recent respiratory tract infection. Her medical history included diabetes mellitus

Meningeal melanocytomas are extremely rare, pigmented tumors of the CNS. They generally carry a favorable prognosis, although recurrence and transformation into the more aggressive malignant melanoma have been reported. We present a case of a patient who reported constipation and abdominal pain around the umbilicus, which progressed into cord compression with lower extremity weakness and gait instability. Spinal MRI revealed a tumor at the level of T11, and she underwent gross total resection of the mass. Pathology demonstrated a meningeal melanocytoma with intermediate features. She received

A 50-year-old man presented with a 9-month history of progressive left arm weakness and dysarthria. Family history showed that his parents are cousins, and one of his siblings died of motor neuron disease. Brain MRI showed T2-weighted white matter hyperintensities along the course of pyramidal tracts. Neurologic examination and EMG revealed upper and lower motor neuron signs involving the bulbar, cervical, thoracic, and lumbosacral segments, which meets the criteria of a definite amyotrophic lateral sclerosis (ALS), according to the revised EI Escorial criteria. Whole-exome genetic sequencing
