Guideline on the diagnosis and treatment of primary idiopathic clubfoot.

- A delegation of 6 pediatric orthopedic surgeons from the Dutch Orthopedic Association (NOV) and 2 members of the board of the Dutch Parents' Association for children with clubfoot created the guideline "The diagnosis and treatment of primary idiopathic clubfeet" between April 2011 and February 2014. The development of the guideline was supported by a professional methodologist from the Dutch Knowledge Institute of Medical Specialists. This evidence-based guideline process was new and unique, in the sense that the process was initiated by a parents' association. This is the first official gui

Management of idiopathic clubfoot in toddlers by Ponseti's method.

The Ponseti method has been reported to have successful results in clubfoot patients less than 6 months of age but the literature on its efficacy in older clubfoot patients still remains sparse. In our study, we prospectively evaluated 55 clubfeet (37 patients) to determine clinically whether the Ponseti method is effective in the management of clubfoot in older children between the age of 12 and 36 months (mean: 24.8 months). All the patients belonged to moderate or severe grades of deformity as per the Pirani scoring. Painless, supple, plantigrade and cosmetically acceptable feet were achiev

Differential distraction for relapsed clubfoot deformity in children.

PURPOSE: To evaluate the long-term outcome of clubfoot correction following differential distraction using the Joshi external stabilisation system (JESS). METHODS: 13 male and 9 female patients aged 1.5 to 5 years with 28 idiopathic clubfeet underwent differential distraction using JESS for relapsed deformity after posteromedial soft-tissue release. They were available for review after a mean of 12 years. Deformities of the feet were rigid and graded as Dimeglio type III or more. Pain, function, and satisfaction were evaluated using a questionnaire. Alignments of the forefoot and hindfoot, su

Correction of arthrogrypotic clubfoot with a modified Ponseti technique.

Surgical releases for arthrogrypotic clubfeet have high recurrence rates, require further surgery, and result in short, painful feet. We asked whether a modified Ponseti technique could achieve plantigrade, braceable feet. Ten patients (mean age, 16.2 months; range, 3-40 months), with 19 arthrogrypotic clubfeet, underwent an initial percutaneous Achilles tenotomy to unlock the calcaneus from the posterior tibia followed by weekly Ponseti-style casts. A second percutaneous Achilles tenotomy was performed in 53%. Mean number of casts was 7.7 (range, 4-12). From pretreatment to completion of init

Recurrent clubfoot--approach and treatment with external fixation.

A simplified standard setting of the circular external fixator allows correction of all the complex deformities of recurrent clubfoot with minimal surgical intervention and no major complications. In those cases where additional corrective arthrodesis is necessary, it is performed with minimal bone resection because the severe deformities of the foot and ankle have already been corrected.

Ankle valgus and subtalar varus in treated clubfoot.

UNLABELLED: The purpose of this study was to clarify whether residual subtalar varus in treated clubfoot affects the configuration of the ankle joint in the coronal plane. Radiographic characteristics of the ankle and subtalar joint in congenital clubfoot, after a minimum of 10 years of treatment, were analyzed and presented. Weight-bearing radiographs of the ankles and feet, as well as computerized tomographic images of the hindfoot, were obtained for 30 patients (41 feet), at a mean of 15.2 +/- 6.1 years after initial treatment. Subtalar varus was measured in terms of the tilting angle of th

Congenital clubfoot associated with an accessory soleus muscle.

A 16-month-old male with previously untreated bilateral clubfeet was admitted to S.B. Izmir Tepecik Education and Research Hospital, Izmir, Turkey. Both feet underwent surgical treatment. During surgery, an accessory soleus muscle was detected on the right side. The accessory soleus muscle had a distinct distal insertion at the superior anteromedial border of the calcaneus and also anterior and medial to the Achilles tendon. He was treated by bilateral complete subtalar release with Cincinnati incision, and the accessory soleus was also cut and the distal part resected. At the final follow-up

A model for clubfoot based on micro-CT data.

The pathological anatomy of idiopathic clubfoot has been investigated for more than 180 years using anatomy, computed tomography (CT), histology and microscopy. Seven idiopathic clubfeet and two normal feet of aborted fetuses were dissected in the present study, with special emphasis on the shape of the cartilage and bones. A three-dimensional (3D) micro-CT system, which generates a series of X-ray attenuation measurements, was used to produce computed reconstructed 3D data sets of each of the separated bones. Based on the micro-CT data scans a high-definition 3D colour printing system was use

The principles of the Ponseti method in the treatment of congenital clubfoot.

We present Ignatio Ponseti's approach to idiopathic clubfoot management and its biological basis. The key principles of the Ponseti method of treatment are 1. All deformity components are corrected simultaneously except equinus, which is corrected last, usually by percutaneous Achilles tenotomy. 2. The correction is maintained by external foot rotation around the head of the talus. 3. The excavatus is corrected by supination of the first ray of the foot. 4. The corrections are performed weekly and after 4-5 weeks of treatment a percutaneous Achilles tenotomy is performed in local or general an

Accurate determination of cast weight for neonates with clubfoot.

The purpose of this study was to determine changes in cast weight during the first 48 hours after application so that the true weight of a neonate can be estimated without the need for removing the cast. Five types of cast materials were compared. Cast weight measurements were obtained before and after application and at intervals during 48 hours. Final cast weight averaged 107.5% of dry weight for plaster and 99% of dry weight for synthetic cast materials. For very low birth weight infants, the difference between the initial wet weight of plaster of Paris and its final dry weight may be signi

[Analysis of intraoperative complications during surgery for clubfoot].

The paper presents an analysis of the complications accompanying surgical treatment of congenital club feet. In the year 1966-1993 178 feet in 149 patients aged ranging from 5 to 140 months (mean age 17.5 months. In 57 cases a posterior capsulotomy with lengthening of the Achilles tendon was performed, in 121 cases a total sub-talar release was performed. Complications wee classified as intra-operative and early and late post-operative. A total of 85 complications in 44 patients were observed, which amount to 44.7% of the cases. In 16 cases intra-operative complications were a result of techni

Ossification of the calcaneus in the normal fetal foot and in clubfoot.

The ossification of the fetal calcaneus was studied in a series of plastinated sections of 44 normal feet and three clubfeet. The results documented a precise sequence of perichondral and endochondral ossification in the normal calcaneus characterized by ossification grooves and associated cartilage canals. Within the clubfoot calcaneus, the coordination of perichondral and endochondral ossification is disturbed, ossification grooves and associated cartilage canals are not found in regular positions, and the process of endochondral ossification obviously is disturbed. The findings in the clubf

An independent assessment of two clubfoot-classification systems.

We conducted an independent assessment of two clubfoot-classification systems. In a blinded trial, two orthopaedists scored 55 feet by using the classification systems developed by Pirani et al. and by Dimeglio et al. Thirty-seven of the feet were also scored by a physical therapist. By using the 10-point classification described by Pirani, the two physician examiners tallied total scores that were within one point of one another 89% of the time. The mean difference between the scores assigned by the two examiners was 0.6 points. For the 20-point classification described by Dimeglio et al., to

Onset of ossification of the tarsal bones in congenital clubfoot.

Subjects of this study were 45 patients with 56 idiopathic congenital clubfeet (34 boys with 42 feet and 11 girls with 14 feet) who underwent successful conservative treatment. Radiologic examinations were performed at intervals of 3 months. In anteroposterior radiographs of bilateral feet, the period of appearance of the ossification center was observed for the first, second, and third cuneiform and the tarsal navicular. The ossification of the tarsal bones in the congenital clubfeet occurred later than that in the unaffected feet in boys. The difference between the clubfeet and the unaffecte
