Protective role of tauroursodeoxycholate during harvesting and cold storage of human liver: a pilot study in transplant recipients.

BACKGROUND: Ischemia-reperfusion injury is a major cause of early graft dysfunction after liver transplantation. Tauroursodeoxycholic acid (TUDCA), a natural amidated hydrophilic bile salt, protects from cholestasis and hepatocellular damage in a variety of experimental models, as well as from ischemia-reperfusion injury. We investigated in the human liver transplantation setting the effect of the addition of TUDCA at time of liver harvesting and cold storage on the intra- and postoperative enzyme release and liver histopathology at the end of cold storage, at reperfusion, and 7 days after tra

Ischemia considerations for the development of an organ and tissue donor derived bone marrow bank.

BACKGROUND: Deceased organ donors represent an untapped source of therapeutic bone marrow (BM) that can be recovered in 3-5 times the volume of that obtained from living donors, tested for quality, cryopreserved, and banked indefinitely for future on-demand use. A challenge for a future BM banking system will be to manage the prolonged ischemia times that are inevitable when bones procured at geographically-dispersed locations are shipped to distant facilities for processing. Our objectives were to: (a) quantify, under realistic field conditions, the relationship between ischemia time and the

Cold blood cardioplegia reduces the increase in cardiac enzyme levels compared with cold crystalloid cardioplegia in patients undergoing aortic valve replacement for isolated aortic stenosis.

OBJECTIVES: Cardiac arrest during cardiac surgery is most commonly induced by cold blood or cold crystalloid cardioplegia. The results from clinical studies are divergent regarding which of the 2 solutions provides better myocardial protection. This might be explained by several factors. Both heterogeneity in disease for the included patients and the fact that most studies are retrospective in design and that patients with coronary artery disease with different degrees of myocardial ischemia are included might explain these findings. To circumvent these potentially confounding factors, we incl

Uterine Transplant Optimization From a Preclinical Donor Model With Controlled Cardiocirculatory Arrest.

BACKGROUND: Uterus transplantation from deceased donors offers a promising solution to the organ shortage, but optimal preservation methods are crucial for successful outcomes. Our primary objective is to conduct an initial assessment of the contribution of oxygenated hypothermic perfusion in uterine transplantation. METHODS: We performed a preclinical study on a porcine model of controlled donation after circulatory death (60 min warm ischemia). Ten uterus grafts were preserved for 12 h using static cold storage or hypothermic machine perfusion (VitaSmart device, perfusion pressure at 15 mm

Raynaud's phenomenon in a healthy Turkish population.

Raynaud's phenomenon (RP) is a vasospastic disease and is characterized by ischemia of the digits, nose, and ears. The vasospasm can be triggered by cold weather, cold water, or emotional stress and is followed by triphasic color changes. First white color (ischemia), then blue color (congestion and cyanosis), and finally red color change (reactive hyperemia) can be observed. The prevalence is reported to be between 0.5 and 20% in different studies and is dependent on genetic, occupational, and environmental factors. The purpose of our study was to determine the prevalence of RP in an eastern

Influence of sympathetic activity, temperature, ischemia and diazepam on thermal and vibration thresholds.

In healthy volunteers thermal specific, thermal pain and vibration thresholds were assessed and correlated to each other and to sympathetic nervous system parameters. Additionally, different factors such as temperature, diazepam and ischemia affecting perception thresholds were evaluated. The thresholds assessed did not correlate significantly to each other and to sympathetic skin response latency or amplitude. Diazepam influenced the thermal specific, thermal pain and vibration thresholds assessed. The preingestion and postingestion (after 30, 60 and 90 minutes) thresholds did not differ sign

5-Aminolevulinate improves metabolic recovery and cell survival of the liver following cold preservation.

Rationale: Hibernating thirteen-lined ground squirrels (GS; Ictidomys tridecemlineatus) are naturally adapted to prolonged periods of ultraprofound hypothermia (body temperature < 5 ºC) during torpor, and drastic oscillations of body temperature and ischemia/reperfusion-like stress during their short euthermic interbout arousals. Thus, their superior adaptability may hold tremendous promise for the advancement of donor organ cold preservation and subsequent organ transplantation. However, bridging hibernation research and translational medicine has been impeded by a dearth of in vitro research

Multivisceral Transplantation as curative Treatment for patients with Diffuse Splanchnic Arterial Ischemia: a case report.

BACKGROUND: Criteria for multivisceral transplantation (MVTx) have broadened. However, diffuse splanchnic arterial ischemia secondary to visceral arterial thrombosis has only been described once. CASE REPORT: A 46-year-old woman with acute-on-chronic diffuse splanchnic ischemia received a MVTx from a 14-year-old braindead donor. Cold ischemia time was 6h29, warm ischemia time 0h36. Pre-operative embolization of inferior mesenteric artery was performed and the patient was placed on continuous veno-venous hemofiltration. An aortic conduit was constructed. Non-vascularized-rectus-fascia transpla…

Using thermal stress to model aspects of disease states.

Exposure to acute heat or cold stress elicits numerous physiological responses aimed at maintaining body temperatures. Interestingly, many of the physiological responses, mediated by the cardiovascular and autonomic nervous systems, resemble aspects of, or responses to, certain disease states. The purpose of this Perspective is to highlight some of these areas in order to explore how they may help us better understand the pathophysiology underlying aspects of certain disease states. The benefits of using this human thermal stress approach are that (1) no adjustments for inherent comparative di

A porcine ex vivo lung perfusion model with maximal argon exposure to attenuate ischemia-reperfusion injury.

Argon (Ar) is a noble gas with known organoprotective effects in rodents and in vitro models. In a previous study we failed to find a postconditioning effect of Ar during ex vivo lung perfusion (EVLP) on warm-ischemic injury in a porcine model. In this study, we further investigated a prolonged exposure to Ar to decrease cold ischemia-reperfusion injury after lung transplantation in a porcine model with EVLP assessment. Domestic pigs (n = 6/group) were pre-conditioned for 6 hours with 21% O2 and 79% N2 (CONTR) or 79% Ar (ARG). Subsequently, lungs were cold flushed and stored inflated on ice fo…

Infusion of Cold Saline into the Carotid Artery Does Not Affect Outcome After Intrastriatal Hemorrhage.

Localized brain hypothermia (HYPO) can be achieved by infusing cold saline into the carotid artery of animals and patients. Studies suggest that HYPO improves behavioral and histological outcomes in focal ischemia models. Given that ischemic stroke and intracerebral hemorrhage (ICH) share pathophysiological overlap, we tested whether cold saline infusion is safe and neuroprotective when given during collagenase-induced ICH. Eighty-five adult male Sprague-Dawley rats were used. Experiment 1 investigated brain and body temperature changes associated with a cold saline infusion paradigm that was

Liver Cold Storage and Transplantation in the Cold-Adaptive Daurian Ground Squirrels.

In liver transplantation, the functional recovery of donor grafts following extended cold storage is a major challenge. The same obstacle applies to standard rodent models (e.g., mice and rats) for liver transplantation despite their well-established surgical protocols, thus restricting research on key topics such as post-transplantation functional recovery and host-graft immunocompatibility. Here, we successfully established a novel liver transplantation model using an obligate hibernator, the Daurian ground squirrel (DGS; Spermophilus dauricus), which demonstrates intrinsic adaptation to ult

Metabolic alterations of uterine grafts after extended cold ischemic storage: experimental study in ewes.

Uterine transplantation from a deceased donor could become an available option for widely treating uterine infertility. However, this procedure requires more precise knowledge about the graft's tolerance to extended cold ischemia. Here, we sought to assess the uterine metabolic alterations after extended cold ischemic storage in a model of auto-transplantation in ewe. A total of 14 uterine auto-transplantations were performed, divided into 2 groups: 7 after 3 h of cold ischemia time (CIT) and 7 after 24 h. Venous uterine blood was collected before uterus retrieval and during reperfusion (30, 6…
