Lidocaine in the early phase of acute myocardial infarction: the controversy over prophylactic or selective use.

In acute myocardial infarction, lidocaine is considered the drug of choice for the treatment of malignant ventricular arrhythmias. While initially a so-called "selective" treatment strategy prevailed, in which lidocaine was administered only after the onset of certain "warning arrhythmias," the prophylactic use of lidocaine in acute myocardial infarction has been gaining wider usage in intravenous and intramuscular application in recent years. Both therapeutic applications have been found to be problematic of late, which has led to increasingly restrictive use of lidocaine. While in selective

Calcium-sensing receptor activator cinacalcet for treatment of cyclic nucleotide-mediated secretory diarrheas.

Cyclic nucleotide elevation in intestinal epithelial cells is the key pathology causing intestinal fluid loss in secretory diarrheas such as cholera. Current secretory diarrhea treatment is primarily supportive, and oral rehydration solution is the mainstay of cholera treatment. There is an unmet need for safe, simple and effective diarrhea treatments. By promoting cAMP hydrolysis, extracellular calcium-sensing receptor (CaSR) is a regulator of intestinal fluid transport. We studied the antidiarrheal mechanisms of FDA-approved CaSR activator cinacalcet and tested its efficacy in clinically rel

Comparison of Spinal Cord Stimulation Waveforms for Treating Chronic Low Back Pain: Systematic Review and Meta-Analysis.

BACKGROUND: The treatment of chronic refractory low back pain (LBP) is challenging. Conservative and pharmacologic options have demonstrated limited efficacy. Spinal cord stimulation (SCS) has been shown to be effective in reducing chronic LBP in various contexts. With emerging SCS technologies, the collective evidence of novel waveforms relative to traditional tonic stimulation for treating chronic LBP has yet to be clearly characterized. OBJECTIVES: To provide evidence for various SCS waveforms-tonic, burst, and high frequency (HF)-relative to each other for treating chronic LBP. STUDY DES

Protective efficacy of a 'pan-fungal' vaccination strategy against experimental Pneumocystis infection in drug-immunosuppressed macaques.

INTRODUCTION: Pneumocystis jirovecii causes life-threatening fungal pneumonia (PJP) and other serious pulmonary sequelae in HIV infected individuals and other immunocompromised populations. In recent years, while the frequency of PJP has declined in HIV infected individuals treated with anti-retroviral therapies, the incidence has increased among non-HIV populations due to the expanding use of corticosteroids and other immunomodulatory agents to treat immune-mediated inflammatory diseases and hematologic and solid malignancies. Despite the success of trimethoprim-sulfamethoxazole (TMP-SMX) pro

ALG1-mediated PDL1 glycosylation drives macrophage m2 polarization to promote bladder cancer progression.

Asparagine-linked glycosylation protein 1 homolog (ALG1), a β-1,4-mannosyltransferase, catalyzes the initial committed step of the N-glycosylation pathway. While its role in congenital glycosylation disorders is well established, the contribution of ALG1 to bladder cancer (BC) progression, particularly its impact on the tumor immune microenvironment, remains poorly understood. In this multi-omics study, we delineated the pathological significance and molecular mechanisms of ALG1 in BC. We demonstrated that ALG1 directly interacted with programmed death-ligand 1 (PDL1) and catalyzed its glycosy

Revealing the secrets of quorum sensing: a comprehensive review from signal mechanisms to practical applications.

Quorum sensing (QS), a sophisticated cell-cell communication system in microorganisms, governs collective behaviors such as virulence, biofilm formation, and antibiotic resistance in a population-density-dependent manner. Targeting QS with quorum sensing inhibitors (QSIs) has emerged as a promising anti-virulence strategy to combat bacterial pathogens without exerting lethal selective pressure, thereby potentially curbing the rise of antimicrobial resistance. This review provides an integrated overview of the molecular mechanisms of QS signaling networks in Gram-negative and Gram-positive bact

The potential of utilizing Local Field Potential (LFP) in diagnosis and management of Parkinsonism's hallmark symptoms: a comprehensive review.

Parkinson’s disease (PD) is a prevalent neurodegenerative disorder that significantly impacts motor functions. Despite advances in diagnostic imaging, accurately diagnosing and understanding PD remains challenging. Local field potentials (LFPs), which represent the collective electrical activity of neuron populations, can provide insights into the pathophysiology of PD, particularly in symptoms such as tremors, dystonia, gait disturbances, sleep disorders, and cognitive dysfunction. Tremors, a hallmark symptom of PD, have been closely studied through LFPs recorded from the subthalamic nucleus
