Intra-articular distal radius fractures are a heterogeneous group of injuries with different fracture patterns. The existing classification systems are helpful for describing the fractures but not for assessing their stability or for deciding which surgical approach to use. Patients who have a fracture with at least 1.0 mm of displacement of the articular surface may benefit from open surgical treatment. Improved diagnostic imaging with CT is helpful for fracture classification and surgical planning. The options for surgical treatment include limited open reduction and internal fixation, arthr

PURPOSE: The aim of this study was to compare the functional and radiographic outcomes of dorsally displaced distal radius fractures treated by closed reduction plaster cast fixation (CRPCF) and external fixation (EF) in patients 65 years and older.
METHODS: This retrospective and nonrandomized study comprised 46 consecutive patients older than 65 years who had distal radial fractures. Patients were divided into 2 groups according to treatment: a CRPCF group and an EF group. All the fractures were dorsally displaced and AO/ASIF type A or C, without articular stepoff or gap. Cases were evaluat

OBJECTIVES: To determine the effect of intravenous regional guanethidine on post-traumatic algodystrophy.
METHODS: Ten of 20 consecutive patients with algodystrophy after Colles' fracture were treated with serial intravenous regional guanethidine blockades and the other 10 were treated with physiotherapy alone. Patients were assessed before and after each block and then monthly over a six month period.
RESULTS: The treated patients had subjective and objective improvements in the assessed features of algodystrophy; treatment resulted in a significant reduction in finger tenderness at five an…

Posttraumatic malunion usually results from incorrect treatment of Colles or Smith fracture. Chronic pains, limited motion and impaired power can be avoided and cosmetic appearance of the hand improved by restoration of the functional anatomy of the wrist. The malalignment is corrected by open wedge osteotomy of the distal end of the radius. A small iliae cortico-cancellus bone graft is inserted at the osteotomy site. By using this technique it is usually possible to avoid additional resection of the distal ulna. The surgical approach dorsal or volar is determined by the type of dislocation. T

Carpal bones are rarely affected by avascular necrosis (AVN) in the absence of fractures. The lunate is the most frequently affected carpal bone, followed by the scaphoid and the capitate. The triquetrum is rarely affected by AVN. We report a case of multiple cystic changes in the triquetrum in a patient with a history of trauma. He was treated by below elbow Colles plaster cast for 3 months, with no improvement. Cystic changes resulted from irreversible AVN of the triquetrum. This is the first case to be reported in the literature with cystic AVN changes in the triquetrum.

The Kapandji-Sauvé operation consists in the arthrodesis of the distal radioulnar joint surmounted with a segmentary resection of the lower ulna. This technique may be used not only in rheumatoid dislocations of the distal radioulnar joint instead of the resection of the distal end of the ulna (Moore-Darrach) but also in traumatic diseases such as dislocations, sprains, chronical instabilities of this joint and stiffness secondary to Colles fractures. Two techniques are described, following the original one proposed in 1936. The first one (Technique I) indicated to chronical instabilities seco

OBJECTIVE: To observe the curative effect of self-made eccentric traction belt as a manipulative reduction tool in the treatment of the distal radial fracture.
METHODS: From February 2011 to June 2012,62 patients with distal radial closed fractures were treated by manipulative reduction with self-made wristlet-eccentric traction belt and combined with the small splint and plaster external fixation. Among them, 59 cases were Colles fractures, 2 were Smith fractures, 1 was Barton fracture. After the reduction, lateral X-ray imaging was used to evaluate curative effect immediately.
RESULTS: All

BACKGROUND AND PURPOSE: During physiotherapy rehabilitation, judgements about activity limitation are often based on measurements of impairment. The present paper reports an investigation into the relationship between measurements of impairment and activity limitation in people with a fracture of the distal radius.
METHOD: Twenty people with a fracture of the distal radius were referred to physiotherapy after conservative management of their fractures. Measurements of impairment (grip strength and range of wrist extension) and activity limitation (Jebsen Test of Hand Function) were taken at t

PURPOSE OF THE STUDY: The triangular fibrocartilaginous complex (TFCC) can be injured either due to trauma or by chronic strain. Based on these findings, Palmer devised a classification system distinguishing traumatic (I) and degenerative (II) TFCC lesions. Traumatic TFCC injury may be single or involved in a combined injury of the distal radius. The aim of this study was to evaluate the results of surgical treatment in patients with traumatic TFCC injury at six-month follow-up.
MATERIAL AND METHODS: In the years 2000 to 2004, 23 patients with injury to the wrist were treated. The group compr

We have investigated the use of dolorimetry in the assessment of algodystrophy of the hand. Findings in 12 affected patients following Colles' fracture were compared with age- and sex-matched normal controls. Algodystrophy was associated with a significant increase in tenderness of both the finger joints and bones. Tenderness of the whole hand was quantified by summing the dolorimetry readings at each site within the hand and the ratio of the summed readings for the two hands was defined as the dolorimetry ratio. This ratio was highly reproducible and independent of external variables. It was …

Manual reduction and antegrade intramedullary K-wire fixation were performed for 29 selected Colles-type distal radial fractures in 29 patients. Severely comminuted intra-articular fractures and Barton's fracture were not included in this study. The tips of the K-wires supported an articular surface of the distal radius. Except for 1 case of skin irritation in the forearm, there were no complications related to tendon or nerve injuries or reflex dystrophy. There was no pin loosening, but protrusion of sharp tips of K-wires into the wrist joint occurred in 3 patients. Average volar tilt, radial

The development of osteoporosis is a multifactorial process. Part of contributing factors are not to be influenced, others may be avoided or prevented. The achievement of an optimal peak bone mass as well as its maintenance require sufficient sex hormones in connection with optimal calcium supply with the nourishment and sufficient physical exercise. During the preclinical phase of osteoporosis only the analysis of risk factors is possible. There are no clinical symptoms of the condition itself. Manifestation of osteoporosis is documented by crush fractures or discontinuing fractures. In osteo

A 60-year-old man, who did administrative work, consulted for evaluation of the presence of osteoporosis. He smoked ten cigarettes a day and drank alcohol occasionally. Eight years ago he suffered a Colle's fracture in his right arm after an incidental fall, which resolved without complications. His mother had a hip fracture when she was 78 years old. The patient weighed 89.4 kg and his height was 165 cm (BMI 38 kg/m(2)). The DXA showed a T-score -2.4 at lumbar spine and -1.9 at femoral neck. He had suffered a myocardial infarction one year ago and is presently taking statin, a beta-blocker an

BACKGROUND: Distal radius fractures are common osteoporotic injuries in elderly patients. Dermatoporosis, an age-related cutaneous insufficiency often associated with corticosteroid use and malnutrition, is frequently overlooked in these cases despite its clinical relevance during fracture management.
MATERIALS AND METHODS: A 72-year-old underweight woman with chronic obstructive pulmonary disease and long-term corticosteroid use presented with an extra-articular Colles-type distal radius fracture after a trivial fall. Closed reduction under hematoma block was attempted. During disimpaction,

We determined broadband ultrasound attenuation (BUA) of the calcaneus, and bone mineral density (BMD) of the spine, proximal femur and radius in 137 healthy subjects (79 women and 58 men) and in 56 women with Colles' fracture. The repeated measurements on 9 healthy subjects indicated a short-term reproducibility (coefficient of variation) of 4 percent for BUA. There was a small (7 percent) but significant difference in BUA between normal men and women. The age-dependence in normal subjects was weak. When all the study subjects were pooled, modest correlations between BUA of the calcaneus and B…

Based on previously published reports on fractures of the distal radius and on the analysis of 400 fresh fractures treated in two accident hospitals affiliated with our university hospital, we propose a new classification as shown in Table 2. This new classification includes a type of combined Chauffeur-Barton fracture which has never been described before. Each type of fracture of the distal radius requires a specific treatment as shown in Fig. 1.
