Differences in coronary plaque composition with aging measured by coronary computed tomography angiography.

BACKGROUND: Little is known about the independent impact of aging on coronary plaque morphology and composition in the era of cardiac computed tomography angiography (CCTA). METHODS: We studied 1015 consecutive asymptomatic South Korean subjects (49 ± 10 years, 64% men) who underwent 64-slice CCTA during routine health evaluation. Coronary plaque characteristics were analyzed on a per-segment basis according to the modified AHA classification. Plaques with >50% calcified tissue were classified as calcified (CAP), plaques with <50% calcified tissue were classified as mixed (MCAP), and plaques

Coronary computed tomography angiography---a promising imaging modality in diagnosing coronary artery disease.

BACKGROUND: Traditionally, information on coronary artery lesions is obtained from invasive coronary angiography (CAG). The clinical applicability and diagnostic performance of the newly developed 64-slice multislice computed tomography (MSCT) scanner in coronary angiographic evaluation is not well evaluated. METHODS: Coronary computed tomography angiography (CCTA) was performed in 345 patients (119 women, 226 men; mean age, 59.64 +/- 11.67 years). Concomitant CAG was performed in 53 patients. The diagnostic performance of CCTA for detecting significant lesions was compared with that of CAG b…

[How I investigate... The coronary arteries in 2007: contributions of CT coronary angiography].

Cardiac imaging has always been a challenge because of the continuous movement of the heart. Cardiac computed tomography (CT) has undergone an accelerated progression over the past decade, due to the combination of the high-speed rotation of the X-ray tube, the ECG-gating technique and the infra-millimeter spatial resolution. Multidetector CT allows visualisation of the coronary artery lumen and the detection of coronary stenosis after intravenous injection of contrast medium. Studies have demonstrated a high negative predictive value of CT coronary angiography (CTCA). CTCA may be reasonably u…

Detection of malignant right coronary artery anomaly by multi-slice CT coronary angiography.

Coronary artery anomalies occur in 0.3-0.8% of the population and infer a high risk for sudden cardiac death in young adults. Diagnosis is usually established during coronary angiography, which is hampered by poor spatial visualization. Magnetic resonance imaging is an alternative, but it is not feasible in the presence of metal objects or claustrophobia. In this report, a 15-year-old boy experienced ventricular fibrillation and was successfully resuscitated. Cardiac catheterization was inconclusive, and pacemaker implantation prohibited the use of MR imaging. Multi-slice CT coronary angiograp…

Abnormal epicardial coronary resistance in patients with diffuse atherosclerosis but "Normal" coronary angiography.

BACKGROUND: Coronary arteries without focal stenosis at angiography are generally considered non-flow-limiting. However, atherosclerosis is a diffuse process that often remains invisible at angiography. Accordingly, we hypothesized that in patients with coronary artery disease, nonstenotic coronary arteries induce a decrease in pressure along their length due to diffuse coronary atherosclerosis. METHODS AND RESULTS: Coronary pressure and fractional flow reserve (FFR), as indices of coronary conductance, were obtained from 37 arteries in 10 individuals without atherosclerosis (group I) and fro

Cardiac catheterization and selective coronary angiography with tortuous aorta and anomalous coronary artery.

The authors report a case of selective coronary angiography by use of a modified technique in a patient with tortuous iliac artery, distorted thoracic descending aorta, and anomalous left coronary artery. This approach to such anatomy can decrease the fluoroscopy time and reduce the likelihood of complications.

Acute coronary occlusion during coronary angiography in two cases. Treatment by transluminal disobliteration.

Acute coronary obstruction occurred in two patients during coronary angiography. In one case the obstruction was in the left main coronary artery; in the other it was close to the origin of the left anterior descending artery. In both cases acute cardiac ischaemia ensued, with electromechanical dissociation and collapse, which was not reversible by resuscitation. Rapid disobliteration of the occluded coronary artery was done with a guide-wire pushed through the obstruction via the coronary catheter. The recanalisation was completed by an intracoronary perfusion of streptokinase in one case. In…

Comparison of Invasive Coronary Angiography Versus Computed Tomography Angiography to Assess Mehran Classification of In-Stent Restenosis in Bifurcation Percutaneous Coronary Intervention.

The Mehran classification is used to determine the presence of in-stent restenosis (ISR) and characterization of its subtypes in invasive coronary angiography (ICA). The utility of computed tomography angiography (CTA) for the assessment of Mehran classification is unknown. We aimed to compare the agreement and reproducibility of Mehran classification between ICA and CTA and evaluate the sensitivity and specificity of both imaging methods. Consecutive patients who had ISR on ICA preceded with CTA before intervention were enrolled in our study. Modified Mehran's classification was employed by C…

Procedural characteristics of coronary angiography in patients with anomalous aortic origin of a coronary artery.

BACKGROUND: Anomalous coronary arteries (ACA) are seen in 1-5 % of patients undergoing coronary angiography, and their presence may increase procedural difficulty. We aimed to compare procedural outcomes of coronary angiography in ACA patients by anatomy and prior knowledge of the ACA. METHODS: This was a single-center review of ACA patients undergoing coronary angiography between October 2013 and February 2022. Primary endpoints were contrast volume, fluoroscopy dose, time, and dose-area product (DAP). Comparisons were made between groups based on ACA anatomy and based on prior knowledge of …

Efficacy and safety of coronary computed tomography angiography in diagnosing coronary lesions in children.

INTRODUCTION: Identification of paediatric coronary artery abnormalities is challenging. We studied whether coronary artery CT angiography can be performed safely and reliably in children. MATERIALS: Retrospective analysis of consecutive coronary CT angiography scans was performed for image quality and estimated radiation dose. Both factors were assessed for correlation with electrocardiographic-gating technique that was protocoled on a case-by-case basis, radiation exposure parameters, image noise artefact parameters, heart rate, and heart rate variability. RESULTS: Sixty scans were evaluat

Impact of timing to coronary angiography in acute coronary syndrome on contemporary clinical practice.

Recent studies appear to suggest a correlation between timing to coronary angiography and clinical outcome among patients with acute coronary syndrome (ACS). We aim to study 12-month outcomes of ACS patients who are stratified according to early (≤24 hours), intermediate (>24 to <48 hours) and delayed (≥48 hours) coronary angiography. This is a prospective observational study of patients with ACS defined as either unstable angina pectoris or non-ST elevation myocardial infarction (MI) admitted between October 2008 and July 2009. Baseline clinical characteristics of age, gender, cardiovascular …

Evaluation of anatomy, variation and anomalies of the coronary arteries with coronary computed tomography angiography.

Recent technical advances in computed tomography (CT) have improved image quality, diagnostic performance and accuracy of coronary CT angiography (CCTA). Latest dose-reduction strategies reduce radiation dose to an acceptable level even lower than that from conventional coronary angiography. CCTA is a noninvasive imaging modality which can effectively show complex coronary artery anatomy, variations and congenital anomalies of the coronary arteries. Congenital coronary artery anomalies are rare entities, but sometimes have a potential of producing fatal consequences. CCTA is now the primary im…

Use of multislice computed tomographic coronary angiography for the diagnosis of anomalous coronary arteries.

The accurate diagnosis of anomalous coronary arteries by invasive angiography is limited by the inability to define the anatomic course in relation to surrounding structures. Computed tomographic coronary angiography has recently emerged as a noninvasive method to visualize the coronary arteries. Multislice computed tomography with up to 64 detector arrays, along with 3-dimensional rendering, has further improved the temporal and spatial resolution of noninvasive coronary imaging. In this series of cases, the investigators describe their institution's experience with computed tomographic coron…

Clinical role of coronary magnetic resonance angiography in the diagnosis of anomalous coronary arteries.

Though rare, anomalous coronary artery disease is a well-known cause of myocardial ischemia and sudden death among children and young adults. The projectional nature of conventional x-ray angiography often leads to difficulty in the definition of anomalous vessels. Studies have now documented the high accuracy of coronary magnetic resonance angiography (MRA) for the noninvasive detection and definition of anomalous coronary arteries among patients with suspected anomalous coronary arteries of congenital conditions associated with anomalous coronary arteries. With increasing clinical experience
