Revision Microvascular Decompression for Trigeminal Neuralgia and Hemifacial Spasm: Factors Associated with Surgical Failure.

Chronic orthostatic hypotension (OH), affecting 10 to 30% of the elderly, is associated with falls, and increased morbidity and mortality. Current pharmacologic therapy can cause or worsen hypertension and fluid retention. (r)α lipoic acid (ALA), a powerful natural antioxidant, avoids those complications and may assist management of chronic neurogenic orthostatic hypotension (NOH). The purpose of this study is to demonstrate improvement in the symptoms of orthostatic dysfunction with r-ALA, including improved sympathetic (S) and blood pressure (BP) responses to head-up postural change (standin

Surgical decompression without dural opening for symptomatic Chiari type II malformation in young infants.

OBJECTIVE: Prompt surgical decompression for symptomatic Chiari type II malformation in neonates and young infants has been reported to be effective. However, the optimal operative method has been arguable. The authors present their experiences with surgical decompression without dural opening. METHODS: We retrospectively analyzed surgical results of patients with symptomatic Chiari type II malformation. The charts were reviewed for demographic data, clinical presentation, surgical procedures, and clinical outcomes. RESULTS: Ten consecutive patients with symptomatic Chiari type II malformati

Sacral radiculopathy due to cement leakage from percutaneous sacroplasty, successfully treated with surgical decompression.

Percutaneous sacroplasty is a procedure adapted from vertebroplasty, which is designed to ameliorate the painful morbidity associated with sacral insufficiency fractures without the invasiveness of open surgery. Early estimates of efficacy, according to several case reports and small series, appear promising, but the procedure is not without risk. Several cases of radiculopathy due to nerve root compression by extravasated polymethylmethacrylate (PMMA) have been reported. The authors present a case of radiculopathy caused by cement leakage from sacroplasty, treated with surgical decompression

Cauda equina syndrome secondary to an absent inferior vena cava managed with surgical decompression.

The authors report on the case of a 24-year-old man who presented with back pain and radiculopathy due to epidural venous engorgement in the setting of a congenitally absent inferior vena cava. Despite initial improvement after steroid administration, the patient's health ultimately declined over a period of weeks, and signs and symptoms of cauda equina syndrome manifested. Lumbar decompression was performed and involved coagulation and resection of the compressive epidural veins. No complications occurred, and the patient made a full neurological recovery.

Micro-decompression for lumbar spinal stenosis: the early outcome using a modified surgical technique.

We describe a modified technique of micro-decompression of the lumbar spine involving the use of an operating microscope, a malleable retractor and a high-speed burr, which allows decompression to be performed on both sides of the spine through a unilateral, hemi-laminectomy approach. The first 100 patients to be treated with this technique have been evaluated prospectively using a visual analogue score for sciatica and back pain, the MacNab criteria for patient satisfaction, and functional assessment with the Oswestry Disability Index. After a period of follow-up from 12 months to six years a

Neurological recovery after surgical decompression in patients with cervical spondylotic myelopathy - a prospective study.

Cervical spondylotic myelopathy is a common clinical problem. No study has examined the pattern of neurological recovery after surgical decompression. We conducted a prospective study on the pattern of neurological recovery after surgical decompression in patients with cervical spondylotic myelopathy. Patients suffering from cervical spondylotic myelopathy and requiring surgical decompression from January 1995 to December 2000 were prospectively included. Upper limbs, lower limbs and sphincter functions were assessed using the Japanese Orthopaedic Association (JOA) score. Assessment was done b

Multimodal intraoperative monitoring (MIOM) during surgical decompression of thoracic spinal stenosis in 36 patients.

A prospective study of 36 patients who received multimodal intraoperative monitoring (MIOM) during decompression of thoracic spinal stenosis between March 2000 and December 2005 was chosen as the study design. The objective was to determine the sensitivity and specificity of MIOM techniques used for monitoring spinal cord during surgical thoracic decompression. The background data revealed that the surgical decompression for thoracic spinal stenosis is less frequent than in other regions of the spine. However, due to the relative narrow spinal canal, neurological complications could be severe.

Results of degenerative spondylolisthesis treated with posterior decompression alone via a new surgical approach.

OBJECT: The purpose of this study was to assess radiologically demonstrated results and clinical outcomes in patients with degenerative spondylolisthesis who underwent posterior decompressive surgery via a new (unilateral) approach. This approach allows surgeons to perform central and bilateral decompression while only stripping the muscles unilaterally, thus preserving the posterior osteoligamentous complexes. METHODS: The authors evaluated 51 consecutive patients in whom surgery was performed between 1987 and 1996. The mean follow-up period was 4.7 years. There was no statistically signific

Surgical technique for cranio-cervical decompression in syringomyelia associated with Chiari type I malformation.

Our purpose is to present our results with the surgical treatment of syringomyelia associated with Chiari type I malformation. Between October 1989 and October 1995, twenty-eight patients underwent a sub-occipital craniotomy and a C1 laminectomy. After dura mater opening the cerebellar tonsils were mobilised. Neither catheter, nor plugging of the obex, nor tonsillar tissue removal was performed. The dura mater was enlarged by means of a wide graft to create a new cisterna magna of adequate size. Postoperative MRI scans showed an ascent of the cerebellum of 4.3 +/- 4.8 mm (measured by the fasti

Microvascular decompression of cochlear nerve for tinnitus incapacity: pre-surgical data, surgical analyses and long-term follow-up of 15 patients.

The level of success of neurovascular decompression in ponto-cerebellar angle for hemifacial spasm and trigeminal neuralgia has already established the reality of the pathology to explain such symptoms. However, cochlear nerve compression syndrome by vascular loop is still a controversial topic. We have performed a retrospective cases review with long-term follow-up (5-7 years) concerning the results of microvascular decompression surgery of the cochlear nerve via an endoscopy assisted retrosigmoid approach on 15 patients suffering from unilateral incapacitating tinnitus with abnormal auditory

Hybrid Microvascular Decompression for Cranial Neurovascular Compression Syndromes: A Retrospective Case Series and Surgical Nuances.

BACKGROUND: Microvascular decompression (MVD) remains the gold standard for the treatment of neurovascular compression syndromes (NVCS). Adjunctive visualization tools have been introduced to improve visualization beyond the direct microscopic line-of-sight. This study evaluated the technical feasibility, safety, and short-to mid-term clinical outcomes of QEVO-assisted microscopic MVD. METHODS: This retrospective series included 43 patients who underwent QEVO-assisted MVD for trigeminal neuralgia (TN, 31), hemifacial spasm (HFS, 11), and cochleo-vestibular nerve compression syndrome (1) betwe

Surgical Outcomes of Full-Endoscopic Degenerative Lumbar Lateral Recess Stenosis Decompression Through an Interlaminar Approach.

OBJECTIVE: The conventional open decompression surgery for degenerative lumbar lateral recess stenosis (DLLRS) yields definitive therapeutic outcomes; however, it confronts numerous challenges, including extensive surgical trauma and iatrogenic spinal instability. The purpose of this study is to investigate the surgical outcomes of full-endoscopic DLLRS decompression by an interlaminar approach. METHODS: A consecutive cohort of 275 patients, including 148 males and 127 females, with an average age of 64.62 (55-82) years, with DLLRS between July 2021 and December 2022, was reviewed in this ret

Orbital Apex Syndrome Resulting From Trauma: Recovery Outcomes Following Surgical Decompression of the Orbital Wall.

While orbital apex syndrome is considered a rare condition, it has significant neuro-ophthalmic implications for diagnosis and management. It is characterized by the involvement of the following cranial nerves: the optic nerve, the oculomotor nerve, the trochlear nerve, the abducens nerve, and the first division of the trigeminal nerve, manifesting as clinical symptoms upon their impairment. Although various factors can be causal, trauma can also lead to the development of orbital apex syndrome. Regardless of the cause, treatment often involves inducing decompression of the orbital space using

Early versus Delayed Surgical Decompression in Spinal Cord Injury: A Systematic Review and Meta-Analysis.

Spinal cord injury is a devastating clinical condition that causes secondary damage, which can be prevented with some treatments. Early surgical decompression may have a beneficial effect and lead to a better neurological outcome. This study aims to demonstrate the effectiveness of early compared with delayed surgical decompression to evaluate neurological improvement in patients with traumatic spinal cord injury. A systematic review and meta-analysis were conducted following the PRISMA-2020 (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. Until April 2024, PubM

Patient-Reported Outcomes and Complications of Simultaneous versus Staged Surgical Decompression for Tandem Spinal Stenosis.

Introduction  Tandem spinal stenosis (TSS) refers to the narrowing of the spinal canal at two distinct anatomic areas. Symptoms can present due to either cervical myelopathy or lumbar stenosis. Consequently, determining the symptomatic anatomical levels requiring surgery can pose a challenge. We sought to identify the surgical approach associated with better patient-reported outcomes. Materials and Methods  The Information Management System was queried using the International Classification of Diseases Ninth and Tenth Edition codes to identify patients who underwent simultaneous or staged deco

Degenerative Spinal Stenosis and Ipsi-Contralateral Decompression: Presentation of a Surgical Technique and Clinical Cases.

Lumbar spinal stenosis is a widespread condition that significantly affects the quality of life in elderly individuals. Conservative therapy has a positive effect on patients whose primary symptom is pain. However, in severe cases with the presence of hypesthesia and paresis, surgical treatment comes into consideration. The aim of surgery is to decompress the neurovascular elements compressed by the narrowed spinal canal while preserving spinal stability. Conventional laminectomy, with or without fusion, has been considered effective for the treatment of this pathology, but its drawbacks are s
