Swallowing and Communication in Cockayne Syndrome: Clinical Characteristics and Management.

Cockayne syndrome (CS) is an ultrarare genetic disorder associated with genes encoding proteins involved in DNA repair. The clinical course of CS involves neurodevelopmental and neurodegenerative features, including swallowing and communication impairments. The aim of this article is to provide an overview of the literature relating to deglutition and communication in individuals with CS to guide management of this complex disease and highlight clinical knowledge gaps. Pertinent articles and case reports on CS, published between 1990 and 2024, were reviewed in three domains: etiology, dysphagi

Sensor Technologies for Measuring Tongue Biomechanics Relevant to Swallowing: A Narrative Review.

Tongue biomechanics are central to swallowing, yet commonly used clinical assessments provide limited insight into the forces and coordination underlying bolus propulsion. Sensor technologies have emerged to address this gap, but the literature remains fragmented across device classes, calibration approaches, and outcome definitions. This narrative review synthesizes sensor modalities used to characterize tongue biomechanics in dysphagia assessment and rehabilitation. A structured search of biomedical databases identified studies describing pneumatic, piezoelectric, strain gauge, capacitive, o

Spontaneous swallowing frequency in the evaluation of swallowing function.

INTRODUCTION: Oropharyngeal dysphagia (OD) is a common but underdiagnosed condition associated with serious complications such as malnutrition, aspiration pneumonia, and increased mortality. Spontaneous swallowing frequency (SSF) has recently emerged as a potential noninvasive biomarker for dysphagia screening. AREAS COVERED: This narrative review explores the current state of knowledge on SSF in both healthy individuals and patients with OD, including stroke, Parkinson's disease, head and neck cancer, and cerebral palsy. The review discusses the physiological and neurological bases of SSF, s

A Letter to the Editor on "Effect of Visual Biofeedback Obtained Using the Iowa Oral Performance Instrument on the Suprahyoid Muscle Activation Level During Effortful Swallowing Maneuver".

The author has provided an alternative biofeedback method to help maintain the effortful swallowing (ES) maneuver target effort to utilize a constantly high swallowing pressure. The author has identified ES with visual feedback (i.e. the Iowa Oral Performance Instrument (IOPI)) produced higher EMG activation levels than without. However, the author accepted higher activation levels as higher muscle force production, attributed the lower EMG activation levels in 4th set to fatigue, and did not consider other possibilities. As the author stated in the discussion "his/her method caused participan

Oral and Pharyngeal Dysphagia in Adults.

Patients with oral and pharyngeal dysphagia have difficulty forming a cohesive bolus and/or transferring food from the mouth into the pharynx and esophagus to initiate the involuntary swallowing process. This may be accompanied by nasopharyngeal regurgitation, aspiration, and a sensation of residual food remaining in the pharynx. Abnormalities affecting the upper esophageal sphincter, pharynx, larynx, or tongue, in isolation or combination, result in oropharyngeal dysphagia affecting either or both transit and airway protection. These issues can be addressed with a combination of management of

Revealing Goal-Directed Neural Control of the Pharyngeal Phase of Swallowing.

Swallowing is considered a three-phase mechanism involving the oral, pharyngeal, and esophageal phases. The pharyngeal phase relies on highly coordinated movements in the pharynx and larynx to move food through the aerodigestive crossing. While the brainstem has been identified as the primary control center for the pharyngeal phase of swallowing, existing evidence suggests that the higher brain regions can contribute to controlling the pharyngeal phase of swallowing to match the motor response to the current context and task at hand. This suggests that the pharyngeal phase of swallowing cannot

Swallowing function in patients with spinal muscular atrophy before and after the introduction of new gene-based therapies: what has changed?

OBJECTIVE: Individuals diagnosed with Spinal Muscular Atrophy (SMA), particularly those presenting with the most severe phenotypes, have long contended with significant swallowing dysfunction. The recent emergence of efficacious advanced therapy has fundamentally altered the landscape of SMA management. By encompassing both the pre and post gene-based therapy eras within our analysis, we endeavour to elucidate the potential impact of these novel therapeutic interventions on this function. METHODS: Following the established methodology outlined by the Joanna Briggs Institute, a scoping review

A Scoping Review on the Intersection Between Voice and Swallowing Measures in Healthy and Disordered Populations.

PURPOSE: Voice and swallowing are distinct functions that share anatomical and physiological properties; however, research investigating their intersection is limited. The purpose of this scoping review was to explore the literature surrounding the relationship between voice and swallowing measures in healthy adults and those with non-degenerative disorders. Specifically, we aimed to elucidate whether objective voice measures could be used as correlates of swallowing function. METHOD: We systematically searched four databases (Embase, PubMed, CINAHL, and Web of Science) for relevant literatur

Patient exposure during videofluoroscopic swallowing studies performed by speech-language pathologists.

Videofluoroscopic swallowing studies (VFSSs) are fluoroscopic examinations performed by speech and language pathologists (SLPs), for the evaluation of the oral and pharyngeal phases of swallowing, in patients who are diagnosed with symptoms like dysphagia and speech impairment. The study was focused on the evaluation of the patient doses from VFSS performed at Hamad Medical Corporation hospitals. Data on the patient exposure and examination parameters were extracted from the Radiation Dose Monitoring system, statistically analysed and compared with literature. For adult patients, the mean (med

Early and accurate dysphagia diagnosis is essential for reducing the risk of associated co-morbidities and mortalities. Barriers to current evaluation methods may alter the effectiveness of identifying at-risk patients. This preliminary study evaluates the feasibility of using iPhone X-captured videos of swallowing as a non-contact dysphagia screening tool. Video recordings of the anterior and lateral necks were captured simultaneously with videofluoroscopy in dysphagic patients. Videos were analyzed using an image registration algorithm (phase-based Savitzky-Golay gradient correlation (P-SG-G
