Severe fetal ventriculomegaly: Fetal morbidity and mortality, caesarean delivery rates and obstetrical challenges in a large prospective cohort.

INTRODUCTION: Severe fetal ventriculomegaly (VM) is defined as an enlargement of the atria of the lateral cerebral ventricles (Vp) of greater than 15 mm. While it is well established that it confers significant risk of morbidity and mortality to the neonate, there is limited information pertaining to the caesarean delivery rates and the obstetric management of these complex cases. The aim of this study was twofold: firstly, to determine survival rates in fetuses with severe VM, and secondly to determine the caesarean delivery rates in continuing pregnancies. We explore the obstetric challenges

Experience and confidence in vaginal breech and twin deliveries among obstetric trainees and new specialists in Australia and New Zealand.

BACKGROUND: The number of vaginal breech and twin deliveries may be insufficient for adequate training. OBJECTIVE: To determine whether advanced trainees and new Fellows in obstetrics are gaining adequate experience and confidence in vaginal breech and twin deliveries. METHODS: An online survey was emailed to registered Royal Australia and New Zealand College of Obstetrics and Gynaecology (RANZCOG) advanced trainees and new Fellows (Years 1-5). This survey asked about their experience, confidence and whether they intend to perform vaginal breech and twin deliveries as specialists. RESULTS:

The influence of mode of delivery, obstetric analgesia and anaesthesia on the response of isolated human chorionic plate arteries to angiotensin II.

A six point concentration: response curved of the contractile effect of angiotensin II (AII) on helically-cut strips of human chorionic plate artery strips were established at final concentrations of between 10(-14) to 10(-9) M. The tissues were obtained from the placentae of primigravid patients who had normal pregnancy, and also from those with pregnancy induced hypertension (PIH). The tissue response were then related to mode of delivery, obstetric analgesia, and anaesthesia. A total of 36 chorionic plate arteries from 12 primigravid patients were studied. i) The overall initial EC50s of th…

Prolonged intrapartum refusal of cesarean delivery and eventual consent: A case report on the ethical boundaries of maternal autonomy in obstetrics.

This case from Saudi Arabia describes a 27-year-old woman at 39 weeks and 6 days of gestation in spontaneous labor, who had had two prior cesarean deliveries and no previous vaginal births, but declined cesarean delivery despite worsening fetal heart tracings and labor dystocia. Repeated counseling was provided, and an ethics consultation was done as the intrapartum refusal persisted. After approximately 12 hours of labor and no cervical changes, she consented to cesarean delivery. The neonate was born with metabolic acidosis related to three tight nuchal cords. This case illustrates the ethic

From Delivery to Depression: Exploring the Role of Perceived Partner Support in the Link Between Obstetric Violence and Postpartum Depression.

Obstetric violence is linked to postpartum depression, yet the mechanisms underlying this relationship remain underexplored. This study examined perceived partner support as a mediating mechanism among 535 women who had given birth within the past 18 months. Results indicated that obstetric violence was associated with higher postpartum depression and lower perceived partner support. Perceived partner support was negatively related to depression. Mediation analyses showed that perceived partner support partially mediated the association between obstetric abuse and violence and postpartum depre

Latency to delivery and incidence of adverse obstetric and perinatal outcomes in preterm premature rupture of membranes before 32 weeks.

PURPOSE: The aim of this study was to evaluate the average latency to delivery, obstetric outcomes and neonatal survival in pregnancies complicated by preterm premature rupture of membranes (PPROM) before 32 weeks. METHODS: A retrospective study was conducted on pregnant women admitted for PPROM before 32 weeks. Patients were categorized into three groups based on gestational age (GA) at PPROM (< 24, 24 to 28, 28 to 32 weeks). Latency to delivery, obstetric outcomes and neonatal survival were analyzed. RESULTS: 86 women who had PPROM before 32 weeks were identified. The mean GA at PPROM was …

Is sonographic measurement of head-perineum distance useful to predict obstetrical anal sphincter injury in case of vacuum delivery?

OBJECTIVE: Determine if head-perineum distance (HPD) measurement before vacuum extraction (VE) was predictive of an obstetric anal sphincter injury (OASIS) occurrence. METHODS: Retrospective, bicentric (Lille and Poissy, France) cohort study conducted from January 2019 to June 2020. All VE in singleton pregnancies of ≥34 weeks were included. HPD measurement was performed without compression of the tissues before each VE. The judgment criterion was the occurrence of an OASIS. RESULTS: Of 12 568 deliveries, VE was performed in 1093 (8.6%). Among these 1093 women undergoing VE, 675 (61.7%) with

Incidence and Risk Factors of Obstetric Anal Sphincter Injuries after Various Modes of Vaginal Deliveries in Chinese Women.

BACKGROUND: Obstetric anal sphincter injuries (OASIS) can cause an adverse impact on women's physical and mental health. There was lack of published data in Chinese population particularly on studying the risk of OASIS for nonrotational outlet forceps. This study was to determine the incidence and risk factors of OASIS. METHODS: This is a retrospective cohort study carried out in a tertiary referral hospital in Hong Kong. The control group was selected randomly. Univariate and multivariate logistic regression analysis was performed to evaluate the influence of potential risk factors on OASIS.

Assessing quality of obstetric care for low-risk deliveries; methodological problems in the use of population based mortality data.

BACKGROUND: Studies evaluating safety of different birth settings for low-risk deliveries are often difficult to interpret because of great methodological problems. OBJECTIVE: To assess potential bias in comparisons of mortality between maternity institutions with different size and level of care, particularly when using various definitions of low-risk delivery and when studying stillbirth rates. DESIGN: Population-based study. POPULATION: The population of 1.74 million births in Norway from 1967 to 1996 recorded in The Medical Birth Registry of Norway. METHODS: First we explored the probl

[Analysis of breech deliveries from the Clinic of Obstetrics and Gynecology, National Institute for Mother and Child in a 12 month period].

An analysis of 130 breech deliveries was carried out. The material was divided into two groups: vaginal delivery and delivery by cesarean section. In both groups maternal age, parity, weeks of pregnancy, newborn weight. Apgar score in the 1st and the 5th minute and perinatal mortality rate of newborns were compared. Frequency of breech presentation in our material was about 4.1%. Cesarean section was performed in 66.2% of cases being 22.7% of all cesarean sections performed in this period. In primiparae (58.5% of breech presentation) cesarean sections were performed in 75% and in 93.3% of the

[Effect of pregnancy and delivery on the function of the cervico-urethral closure. II. Retrospective study on late consequences of obstetrical trauma].

Adding together the obstetric histories and the urodynamic investigations carried out on 178 incontinent women has made it possible for the authors to work out the role that obstetric trauma can play in urinary symptoms. They are able to demonstrate the effects of repeated normal deliveries and also the results of traumata over and above those that occur physiologically and are sometimes undergone by the perineum.

Abnormal obstetric shock index and associated factors among immediate postpartum women following vaginal delivery at a tertiary hospital in southwestern Uganda.

BACKGROUND: Early recognition of haemodynamic instability after birth and prompt interventions are necessary to reduce adverse maternal outcomes due to postpartum haemorrhage. Obstetric shock Index (OSI) has been recommended as a simple, accurate, reliable, and low-cost early diagnostic measure that identifies hemodynamically unstable women. OBJECTIVES: We determined the prevalence of abnormal obstetric shock index and associated factors among women in the immediate postpartum period following vaginal delivery at Mbarara Regional Referral Hospital (MRRH) in southwestern Uganda. METHODS: We c

Intrapartum Ultrasound Guidance to Make Safer Any Obstetric Intervention: Fetal Head Rotation, Assisted Vaginal Birth, Breech Delivery of the Second Twin.

Intrapartum ultrasound (US) is more reliable than clinical assessment in determining parameters of crucial importance to optimize the management of labor including the position and station of the presenting part. Evidence from the literature supports the role of intrapartum US in predicting the outcome of labor in women diagnosed with slow progress during the first and second stage of labor, and randomized data have demonstrated that transabdominal US is far more accurate than digital examination in assessing fetal position before performing an instrumental delivery. Intrapartum US has also be

Increased risk of obstetric anal sphincter injury in women undergoing vaginal delivery after caesarean section: A systematic review and meta-analysis.

BACKGROUND: There is increased focus on obstetric anal sphincter injury (OASI) and there are several well-established risk factors such as birthweight, instrumental delivery and median episiotomy. Some studies have found increased risk of OASI in women delivering vaginally after a previous caesarean section (VBAC). OBJECTIVE: To evaluate whether there is increased prevalence of OASI in VBAC compared with primiparous women. SEARCH STRATEGY: Literature search using PubMed, Embase and Cochrane databases. SELECTION CRITERIA: All studies with data on both primiparous women and women undergoing V

Effect of subsequent vaginal delivery on bowel symptoms and anorectal function in women who sustained a previous obstetric anal sphincter injury.

INTRODUCTION AND HYPOTHESIS: Our primary objective was to prospectively evaluate anorectal symptoms, anal manometry and endoanal ultrasound (EAUS) in women who followed the recommended mode of subsequent delivery following index obstetric anal sphincter injuries (OASIs) using our unit's standardised protocol. Our secondary objectives were to evaluate the role of internal anal sphincter defects and also to compare outcomes in a subgroup of symptomatic women with normal anorectal physiology. METHODS: This is a prospective follow-up study of pregnant women with previous OASIs who were counselled
