Three-dimensional reconstruction model in the diagnosis of Morgagni's hernia.

Morgagni's hernia (MH) can be diagnosed by different utilities, but all these methods are not always 100% accurate. Three-dimensional (3D) reconstruction model could be helpful in better understanding the important anatomical structures. We report a case of MH who was once misdiagnosed as diaphragmatic eventration at the other institution and we offered laparoscopic repair according to the 3D reconstruction model. Our case highlights that 3D reconstruction model could be a useful supplementary tool in the diagnosis and preoperative assessment for patients with MH especially when it is confused…

Successful Repair of Hypoplastic Left Heart Syndrome With Intact Atrial Septum, Congenital Diaphragm Hernia, and Anomalous Origin of Coronary Artery: Defying the Odds.

We report a case of an infant who was postnatally diagnosed with hypoplastic left heart syndrome and an intact atrial septum who underwent emergent atrial decompression followed by the Norwood operation. She was also found to have a congenital diaphragmatic hernia on the left side and a congenital eventration of the right diaphragm, both requiring surgical repair. She was later found to have an anomalous origin of the left circumflex coronary artery from the right pulmonary artery that was ligated at the time of the bilateral bidirectional Glenn operation. She is currently thriving at home, de

Colonic displacement. Proposed treatment for gastric remnant volvulus due to eventration of the diaphragm.

Displacement of the gastric remnant in patients with left diaphragmatic eventration may lead to gastric remnant volvulus requiring surgical correction. This results from the strong negative intrathoracic pulling force created by the potential subphrenic space an paradoxical movement of th diaphragm. Unless this strong negative force is first eliminated, gastropexy alone is likely to result in recurrence. Obliteration of the subphrenic space by colonic displacement is an effective and simple way of abolishing this negative subdiaphragmatic pulling force. Once this is achieved, any form of gastr…

Congenital partial eventration of the left diaphragm.

Two unusual cases of congenital partial eventration of the left diaphragm in young infants are described. In both cases the entity appeared as a space occupying lesion in the left anterior chest and was initially thought to represent pericardial cyst, mediastinal tumor, or cardiac enlargement. The left lobe of the liver occupied the eventration in both cases; one was proven at surgery, and the other was documented with a radionuclide liver scan. The radiographic features of eventration are discussed and the role of radionuclide liver scanning in infants with chest masses is emphasized.

Fetal tissue engineering: diaphragmatic replacement.

BACKGROUND/PURPOSE: Prosthetic repair of congenital diaphragmatic hernia has been associated with high complication rates. This study was aimed at applying fetal tissue engineering to diaphragmatic replacement. METHODS: Fetal lambs underwent harvest of skeletal muscle specimens. Once expanded in vitro, fetal myoblasts were suspended in a collagen hydrogel submitted to controlled radial tension. The construct was then placed in a bioreactor. After birth, all animals underwent creation of 2 diaphragmatic defects. One defect was repaired with the autologous-engineered construct placed in between

Genomic alterations that contribute to the development of isolated and non-isolated congenital diaphragmatic hernia.

BACKGROUND: Congenital diaphragmatic hernia (CDH) is a life threatening birth defect. Most of the genetic factors that contribute to the development of CDH remain unidentified. OBJECTIVE: To identify genomic alterations that contribute to the development of diaphragmatic defects. METHODS: A cohort of 45 unrelated patients with CDH or diaphragmatic eventrations was screened for genomic alterations by array comparative genomic hybridisation or single nucleotide polymorphism based copy number analysis. RESULTS: Genomic alterations that were likely to have contributed to the development of CDH …

Diaphragmatic overlay reconstruction using a bovine pericardial patch (Peri-Guard®): feasibility, safety, and radiographic outcomes.

BACKGROUND: Surgical treatment of symptomatic diaphragmatic paralysis or eventration classically relies on suture plication of the elevated hemidiaphragm. We describe an alternative technique-overlay reconstruction of the diaphragm using a bovine pericardial patch (Peri-Guard®) without formal plication sutures-and report its feasibility, safety, and radiographic outcomes. METHODS: We retrospectively reviewed consecutive patients undergoing diaphragmatic overlay reconstruction with a Peri-Guard® patch at Hannover Medical School between January 2020 and May 2025. Demographic, perioperative, and

[Diseases of the diaphragm].

The diaphragm is the main muscle involved in ventilation and is supplied exclusively by the phrenic nerves. Congenital defects of the diaphragm muscle or phrenic nerve injury cause diaphragmatic paralysis-eventration. Prognosis and treatment depend on whether involvement is unilateral or bilateral and on the patient's previous clinical status. In addition, the diaphragm is an anatomical barrier between the thoracic and abdominal cavities and is traversed by the esophagus and important vascular and nerve structures. Abnormal dilation of the natural orifices of the diaphragm or loss of its conti

[Computed tomography in the diagnosis of pathological states of the lumbar diaphragm. 2: diseases of the diaphragm].

CT potentialities in the diagnosis of diaphragmatic diseases were considered. CT provides valuable information in relaxations of the diaphragm and diaphragmitides, permitting the detection of infiltration of the diaphragm by a tumor, retroperitoneal fibrosis, diaphragmatic cysts, Bochdalek's hernia, and hernias of the esophageal opening. An algorithm for investigation of patients with suspected diaphragmatic pathological conditions was proposed.

Eventration of diaphragm with ectopic thoracic kidney: A cause of chronic debilitating flank pain, managed via robot-assisted approach.

Usually, flank pain signifies the presence of a renal/ureteric pathology. Here, we highlight an unusual cause of flank pain, which can be an important differential to be borne in mind while evaluating the causes of flank pain. Anatomical variants of the kidneys are very common, but rarely do they cause flank pain unless associated with an ongoing pathological process within them. We describe a case wherein an anatomical variant of the kidney, without any pathology, led to unremitting flank pain, purely due to its location and associated "vascular drag." Correction of the underlying anatomical

Stapled Diaphragmatic Plication: Is It Better Than Suture Plication?

Diaphragm dysfunction, which can be due to eventration or an abnormally high-positioned diaphragm, can cause respiratory compromise. Eventration is most commonly due to unilateral diaphragm paralysis, which reduces ventilatory function in adults by about 25%. Since the 1920s, this condition has been treated with diaphragm plication, which itself has evolved to include the use of several different techniques and materials. This review explores the relevant literature on diaphragm plication using sutures or staples for the treatment of eventration to determine if either provides a clinical benef

Diverticulum of the midthoracic oesophagus and left diaphragmatic relaxation.

The case of a 58-year-old man with a large midthoracic oesophageal diverticulum and a left diaphragmatic relaxation who presented with night regurgitations, abdominal bloating, epigastric burning and a sensation of fullness after meals is reported. The patient underwent a successful thoracotomic diverticulectomy with left diaphragmatic plicature. The postoperative course was uneventful. To our knowledge this is the first reported case of an association between midthoracic oesophageal diverticulum and left diaphragmatic relaxation. Moreover, we hypothesised that the diverticulum was caused by a
