Infectious Aortitis Following Type B Dissection.

BACKGROUND: Infectious aortitis of the native thoracic aorta is rare and life threatening, typically presenting with nonspecific symptoms. Diagnosis relies on advanced imaging and microbiological confirmation. Management involves targeted antibiotics with surgical intervention. CASE SUMMARY: We report a patient with Loeys-Dietz syndrome and a Stanford type B aortic dissection who developed Staphylococcus aureus bacteremia from a peripheral intravenous line. Despite targeted antibiotics, an infection involving the dissected thoracic aorta developed without involvement of an abdominal endograft

Topologic distributions of vasa vasorum and lymphatic vasa vasorum in the aortic adventitia--Implications for the prevalence of aortic diseases.

BACKGROUND AND AIMS: Vasa vasorum (VV) and lymphatic vasa vasorum (LVV) form their own networks in the adventitia. VV supply the aorta with nutrition and oxygen; however, the distribution and role of LVV remains to be determined. The purpose of this study was to investigate differences in the distribution of VV and LVV along the aorta. METHODS: Aortic samples were obtained from 22 autopsy cases without medical history of aortic diseases. Aortic segments were classified as arch (Ar), descending thoracic (De), suprarenal abdominal (S-Ab), and infrarenal abdominal (I-Ab). Adventitial VV and LVV

Genetics of Thoracic and Thoracoabdominal Aortic Dissections and Aneurysms.

Acute aortic dissection is a life-threatening event that requires immediate medical attention and surgical intervention. Aortic dissections affect 4 to 5 per 10,000 individuals in the USA. These emergency events can have as high as a 50% mortality risk. Twenty percent of patients who develop a thoracic aortic dissection have an underlying genetic cause for their increased risk for aortic aneurysm and dissection, which can be identified using genetic testing. Genes associated with abdominal aortic aneurysm and dissection have been identified but have not been reported for clinical use or manage

Long-Term Longitudinal Computational Study of a Marfan Syndrome Patient After Hybrid Repair of Aortic Arch Dissection With Parallel Stent-Grafts.

Hybrid repair is a valuable alternative treatment for aortic arch disease in Marfan syndrome patients after proximal aorta replacement. This study aimed to investigate the long-term durability of this technique with the use of parallel stent-grafts and evaluate strategies to prevent abdominal aortic dilation. One Marfan syndrome patient who underwent hybrid aortic repair with parallel stent-grafts for arch dissection after the Bentall procedure was admitted. Five patient-specific three-dimensional models were reconstructed based on preoperative and follow-up computed tomography angiography sca

Pathobiology of Aortic Aneurysms and Dissections: Synthesis of Recent Investigations and Evolving Insights.

The pathobiology of aortic disease is linked to aortic region: atherosclerosis for abdominal aorta, primary medial degeneration or aortitis for ascending thoracic aorta, and all causes for descending thoracic aorta and thoracoabdominal lesions. The pathogenesis of aortic dissection involves damage of the outer media from impaired perfusion from dysfunctional vasa vasorum, formation of discrete foci of disrupted vascular smooth muscle cell-elastic fiber extension-contractile units, and imbalance of radial sheer stress across the aortic wall, thereby creating an intimal tear and linear dissectio

[Thoracoabdominal aorta replacement for penetrating atherosclerotic ulcers of suprarenal abdominal aorta].

A 58-year-old man with a complaint of a feeling of fullness and constipation was admitted to our hospital. Enhanced computed tomography (CT) images demonstrated sacral aneurysm with multiple penetrating atherosclerotic ulcer (PAU) at the abdominal aorta above the renal artery. The aneurysm was expanded for 2 weeks progressively. An urgent thracoabdominal aorta replacement was performed. Pathological findings showed that the media of aorta was destroyed and dissected, and intramural hematoma was found. The postoperative course was good. He has been from any aortic events 12 months after surgery

Aortic dissection manifested as leg ischemia.

The ischemic leg as the only clinical manifestation of an aortic dissection is rare. Without the usual symptoms of an aortic dissection, this diagnosis may easily by overlooked during routine peripheral angiography for the ischemic limb. Three cases are reviewed in which the patient was thought to have a spontaneous peripheral vascular occlusion, but was later found to have an aortic dissection without the usual thoracic or abdominal symptoms. The radiologist should observe carefully for signs of dissection such as fusiform narrowing of a vessel, nonfilling of branches of the aorta, or displac

Livedo Reticularis as a Manifestation of Abdominal Aortic Aneurysm.

Abdominal aortic aneurysm is defined as the abnormal dilation of the abdominal aorta to a diameter of three centimeters or greater. The risk of rupture increases significantly when the aneurysm is greater than 6 centimeters in diameter. Ruptured abdominal aortic aneurysms carry a mortality rate of up to 83%, with up to a third of these deaths occurring before the patient reaches the hospital. Symptoms of dissection or rupture of the abdominal aorta include a tearing sensation in the back, lower extremity weakness, sudden and severe abdominal pain, and hypotension. Symptoms are often vague, and

A case of giant cell arteritis associated with culture-proven Coxiella burnetii aortitis.

A case of proven Coxiella burnetii aortitis, possibly associated with giant cell arteritis (GCA), is reported. A 72-year-old man, who is a hunter, presented with weight loss, fever, jaw claudication, and hardened temporal arteries associated with a persistent inflammatory syndrome and arteritis of the whole aorta, including the brachiocephalic arteries, as seen on 18F-fluorodeoxyglucose positron emission tomography/computed tomography. The diagnosis of GCA was retained, and treatment with prednisolone was started. Given the aneurysm of the abdominal aorta, the patient underwent replacement of

Midline transperitoneal infrapancreatic approach to the supraceliac aorta.

Surgical exposure of the paravisceral and supraceliac abdominal aorta through traditional anterior transperitoneal and lateral retroperitoneal approaches are limited by fragmented operative fields and restricted access to right-sided vasculature. We report the technique of a midline transperitoneal infrapancreatic approach to the supraceliac aorta in a 73-year-old man requiring transaortic paravisceral thrombectomy and aortobifemoral bypass grafting for lower-extremity lifestyle-limiting claudication secondary to extensive paravisceral and aortoiliofemoral atherosclerotic disease. Ruptured abdominal aneurysm subsequent to cystic medial necrosis: a case report.

The majority of abdominal aortic aneurysms are the result of atherosclerosis. Cystic medial necrosis is a pathologic finding classically associated with thoracic aortic dissection, particularly in patients with Marfan's syndrome. The presence of cystic medial necrosis in other segments of the aorta or its branches is unusual. We report a case of a ruptured abdominal aortic aneurysm in a 38-year-old woman without Marfan's syndrome.

Key success factors for thoracic endovascular aortic repair for non-acute Stanford type B aortic dissection.

OBJECTIVES: We aimed to determine the key factors associated with successful early and late outcomes after thoracic endovascular aortic repair (TEVAR) for non-acute Stanford type B aortic dissection at our institution. METHODS: Inpatient and outpatient records were retrospectively reviewed. Patients operated on within 14 days after the onset of acute aortic dissection and those with rupture or malperfusion were excluded. RESULTS: Forty-five patients (mean age, 55.5 ± 13.1 years; 23-79 years) underwent 53 TEVAR operations for non-acute Stanford type B aortic dissection between 1998 and 2012.

A Timeline From Index Procedure To Complex Reinterventions For Thoracoabdominal Aortic Dissection.

The optimal management of acute type A aortic dissection (ATAAD) remains a controversial subject. While some surgeons opt for a hemiarch approach to minimize bypass and cross-clamping time, others prefer partial or total arch replacement to prevent the need for additional operations. The advent of hybrid approaches offers a variety of options to the aortic surgeon in treating ATAAD. Herein, we present a complex case of ATAAD requiring multistage reconstruction of the aortic arch and its branch vessels utilizing hybrid techniques. A 67-year-old man presented with chest pain and hypertension, le

Peripheral athero-embolism from the aorta complicating transhiatal esophagectomy.

The clinical courses of two patients with moderate atherosclerotic aneurysmal dilation of the upper abdominal aorta suffering significant thrombo-embolic sequelae following transhiatal esophagectomy are presented. Inadvertent dislodgement of debris from the diseased aorta during the course of the hiatal and intrathoracic "blunt" dissection was felt to be responsible for this uncommon postoperative problem. This potential complication should be kept in mind when operating near the esophageal hiatus during the course of esophagectomy in such vasculopathic patients.

Total aortic arch grafting for acute type A dissection: analysis of residual false lumen.

BACKGROUND: In surgery for acute type A dissection, an unresected dissection and residual false lumina are causes of the progression of aneurysms and ruptures. We grafted the ascending aorta and total arch, the maximum grafting possible through a median sternotomy alone, in all patients with type A dissection extending to the descending aorta, wherever initial tears existed in the arch. METHODS: A total of 37 consecutive patients with acute type A dissection underwent ascending and total arch grafting between August 1994 and December 2000. Cerebral protection was achieved by selective cerebra
