Clinical and autopsy findings are described in 12 patients who had fatal aortic dissection with the entrance tear in the transverse aorta. The 12 patients represent 7% of 182 autopsies of spontaneous aortic dissection studied by us. The ages of the 12 patients at death ranged from 37 to 87 years (mean, 67 years). Eight were men; 8 had a history of systemic hypertension, and 10 had hearts of increased weight. Diagnosis of aortic dissection was made during life in only 4 of the 12 patients. All 12 patients died of rupture of the false channel within 2 weeks of onset of signs or symptoms compatib

A 58-year-old woman experienced a sudden onset of severe chest and back pain and thus visited our center in October 1999. Contrast-enhanced computed tomography (CT) revealed a Stanford type A acute aortic dissection. The CT also demonstrated a 50 mm ascending aorta and dissection from the ascending aorta via the abdominal aorta to the level of the left renal artery. The perioperative transesophageal echocardiogram showed an intimal tear in the ascending aorta without valvular abnormality. Therefore, we performed graft replacement of the ascending aorta. On the first postoperative day, she deve

Angiography with a pre-diagnosis of acute coronary syndrome was performed in a 76-year-old female patient presenting to another hospital with symptoms of chest pain and syncope. Upon determination of type III aortic dissection, the patient was referred to our clinic. On CT angiography, the ascending aortic diameter was 57 mm and no dissection flap was observed. There was a filling defect suggestive of intimo-intimal intussusception at the level of the aortic arch, occlusion of the left arteria carotid communis, and a double-channel aorta extending from the left subclavian artery to the iliac a

BACKGROUND: The purpose of this study was to investigate the cause of ascending aorta and aortic arch reoperations and to identify determinants of early and late outcome.
METHODS: Between January 1991 and March 2003 we repaired aneurysms of the proximal aorta in 597 patients. Of these patients, 104 had reoperations for replacement of the ascending aorta, aortic root, or transverse aortic arch. Previous surgery was defined as any previous cardiac or proximal aortic repair. Median age was 60 years, and 29 of the patients (28%) were female. Indications for reoperation and replacement of the prox

We used gelatin-resorcin-form-aldehyde (GRF) glue to fuse the false lumen of type A acute aortic dissection in four patients. All were operated on within 3-24 hours after onset, and gluing of the two cylinders of the dissecting aorta could be done safely in a short time. Initial intimal tears were located in the transverse aorta in three patients and in the proximal descending aorta in one. Simple transection and end-to-end anastomosis of the ascending aorta was done for the first two cases. But in the last two patients, we resected the intimal tear in the transverse aorta and applied GRF glue

Iatrogenic aortic dissection is rare but remains a lethal complication in cardiac surgery despite advances in surgical technology. The crisis associated with iatrogenic aortic dissection highlights the importance of using extreme caution. We experienced an iatrogenic aortic dissection during an elective ascending aortic replacement procedure for an ascending aortic aneurysm. After a cardiac surgery fellow placed the aortic cannula in the ascending aorta, pulsatility in the cardiopulmonary bypass circuit was confirmed. Cardiopulmonary bypass was initiated following the placement of the venous c

BACKGROUND: Aortic coarctation (CoAo) may be discovered only when complicated by acute type A aortic dissection (ATAAD). We present a case with a one-stage repair of this pathologic association and review the relevant literature focusing on the surgical choices.
CASE REPORT: A 43-year-old man presented with acute thoracic pain. Computed tomography and echocardiography demonstrated CoAo, ATAAD type II, an ascending aorta aneurysm, and moderate regurgitation of a bicuspid aortic valve. Emergency surgery was performed. A clamshell incision, cardiopulmonary bypass with dual arterial cannulation (

Enlargement due to residual dissection after surgical ascending aortic replacement is technically challenging to treat. Endovascular entry closure using an Amplatzer vascular plug 2 (AVP2) in the sub-acute phase has not been reported. A 75-year-old female underwent emergent ascending aorta replacement for Stanford type A aortic dissection. Postoperative computed tomography angiography (CTA) showed a distal aortic arch diameter of 41 mm and a patent false lumen. CTA one month after the operation revealed distal aortic arch enlargement to 52 mm, which is considered high-risk for rupture. Repeat

Dissection of the ascending aorta (type A) following later after aortic valve replacement has been described with increasing frequency. This study analyzes the role of aortic valve replacement for the evolution of late dissection. In a series of 80 consecutive patients with type A dissection, a previous aortic valve replacement had been performed in 12 cases (15%). In addition to arterial hypertension (p < 0.001) and Marfan syndrome (p < 0.01), multivariate analysis identified previous aortic valve replacement (p < 0.01) as an independent predisposing factor for type A dissection. Dissection o

BACKGROUND: Bicuspid aortic valve-associated ascending thoracic aortic aneurysms (BAV-aTAAs) carry a risk of acute type A dissection. Biomechanically, dissection may occur when wall stress exceeds wall strength. Our aim was to develop patient-specific computational models of BAV-aTAAs to determine magnitudes of wall stress by anatomic regions.
METHODS: Patients with BAV-aTAA diameter greater than 4.5 cm (n = 41) underwent electrocardiogram-gated computed tomography angiography. Three-dimensional aneurysm geometries were reconstructed after accounting for prestress and loaded to systemic press

From April 2011 to March 2020, 87 patients with type A intramural haematoma and acute aortic dissection with thrombosed false lumen of the ascending aorta were treated at Kitasato University Hospital. The initial watch-and-wait strategy without emergency aortic repair was taken in 52 cases in which the maximum aortic diameter was ≤50 mm, pain score on arrival at our hospital was ≤3/10 on the numerical rating scale and there was no ulcer-like projection (ULP) in the ascending aorta. Eleven patients who fulfilled the criteria but developed cardiac tamponade underwent emergency pericardial draina

An 80-year-old woman was referred to our hospital following a syncope. Contrast-enhanced computed tomography revealed an acute type A aortic dissection with a bovine aortic arch and an enlarged innominate artery. The dissection affected only the ascending aorta and not the common trunk, which is composed of the innominate and left common carotid arteries. Cardiopulmonary bypass was established using common trunk perfusion and vena cava drainage. Following a thorough evaluation, a surgical intervention involving the replacement of the ascending aorta and partial arch, accompanied by the excisio

A 39-year-old man presented with sudden onset of chest pain. The initial computed tomography angiography (CTA) showed a small amount of pericardial effusion and a soft tissue density localized in the aortic root, but no intimal flap was identified in the aorta. Aortic dissection was suspected, and the patient was hospitalized at the intensive care unit for close monitoring. On the day of admission, repeated non-contrast computed tomography (CT) showed no significant changes in the aorta or pericardial effusion. Electrocardiography-gated, thin-slice contrast-enhanced CTA revealed an intimal fla

OBJECTIVES: Aim of this study was to assess vascular morphology and dimension of the entire aorta and branch vessels in MFS using thoracoabdominal MR angiography (MRA).
BACKGROUND: Evaluation of vascular involvement in Marfan Syndrome (MFS) is focused to the ascending aorta, which has the major risk of dissection/rupture.
METHODS: From March 2006 to January 2011, 64 patients (35 ± 13 years old) with MFS underwent echocardiography and MRA of thoracic and abdominal aorta. Measurements of ascending aorta, aortic arch, descending thoracic and abdominal aorta, and aortic branch vessels were asses

The authors present their initial experience with the use of the intraluminal double-ring prosthesis in the surgical treatment of thoracic aorta aneurysms. To date, they have performed this procedure in a total of 7 patients. Five had dissecting aneurysms of the ascending and descending aorta, one had a false aneurysm of the aortic arch, and one was operated on for a traumatic aneurysm of the descending aorta. Five patients recovered without any severe complications. Two patients died. Of these, one underwent surgery on an emergency basis for ruptured dissecting aneurysm of the ascending aorta

Conotruncal anomalies such as tetralogy of Fallot, double outlet right ventricle, truncus arteriosus, and transposition of the great arteries are a group of congenital heart defects with abnormalities of the outflow tracts and great vessels. It is common for the ascending aorta and aortic root to be significantly dilated following initial repair of the conotruncal anomaly, and little information is available on the management of this increasing problem. Although there are few case reports of aortic dissection and rupture in the literature, it appears to be rare in the setting of a conotruncal
