The clinical presentation and imaging studies of central nervous system tuberculosis are often indistinguishable from those of patients with malignant neoplasms or metastatic disease. Biopsies may be necessary to distinguish tuberculomas from other intracranial lesions such as pyogenic abscesses or necrotic tumors.

Vertigo/dizziness and syncope are among the most frequent clinical entities encountered in neurology. In patients with presumed syncope, it is important to distinguish it from neurological and psychiatric diseases causing a transient loss of consciousness due to another etiology. Moreover, central nervous disorders of autonomic blood pressure regulation as well as affections of the peripheral autonomic nerves can be responsible for the onset of real syncope. This is particularly relevant in recurrent syncope. Vertigo occurs in the context of temporary disorders, relatively harmless diseases as…

Dizziness with or without associated neurologic symptoms is the most common symptom of posterior circulation transient ischemic attack (TIA) and can be more frequent before posterior circulation strokes. This entity carries a high risk of recurrent events and should be considered as a potential cause of spontaneous episodic vestibular syndrome. Diagnostic evaluation should include intracranial and extracranial imaging of the vertebral arteries and basilar artery. Aggressive medical management with antiplatelet therapy, statin use, and risk factor modification is the mainstay of treatment. This…

INTRODUCTION: Although cerebral small vessel disease is a significant contributor to the development of imbalance and falls in the elderly, whether it causes dizziness is not known.
METHODS: A retrospective case analysis was conducted for 122 dizzy patients referred to two neuro-otology tertiary centres in London and Pisa. Patients were divided into 'explained' causes of dizziness (e.g. benign positional vertigo, vestibular neuritis, orthostatic hypotension, cerebellar ataxias) and 'unexplained' dizziness. White matter hyperintensities (WMH) in MRI (T2 weighted and FLAIR sequences) were blind…

BACKGROUND: Chronic rocking dizziness, often described as the feeling of being on a boat, is classically triggered by prolonged exposure to passive motion. Patients with this motion-triggered sensation of rocking, which is also known as MAL DE DEBARQUEMENT SYNDROME , often develop new onset headaches along with the dizziness. Chronic rocking dizziness has also been noted in vestibular migraine, occurring without a motion trigger. We sought to clarify the association between both motion-triggered (MT) and non-motion-triggered (non-MT) chronic rocking dizziness and headache history.
METHODS: Ou…

OBJECTIVE: We report an unusual case of dizziness caused by carbon monoxide poisoning.
CASE REPORT: A 55-year-old man was referred to an ENT surgeon with dizziness. The patient described vague, non-specific symptoms not consistent with a diagnosis of benign paroxysmal positional vertigo, labyrinthitis or Ménière's disease. It emerged later that the patient had been suffering from carbon monoxide poisoning from a leaky gas hot water boiler in his house. After having the boiler fixed, the patient's symptoms completely resolved.
CONCLUSION: When the more common causes of dizziness cannot be fou…

Superficial cerebral hemosiderosis is characterized by sensorineural hearing loss, gait ataxia and pyramidal signs with irreversible myelopathy. It is caused by chronic hemorrhage into the subarachnoid space with hemosiderin deposition in the subpial, leptomeningeal and subependymal layers. Imaging of the entire neuroaxis is indicated to localize a source of bleeding, including cerebral and spinal angiography when necessary. Taking into account clinical signs and symptoms the interpretation of T2*-weighted images allows the radiologist to set the course for the optimal therapeutic regimen.

To investigate the clinical significance of vertigo and dizziness in diagnosing cerebral cortical lesion, we analyzed 12 patients with supratentorial lesions whose chief complaint was rotational vertigo or non-rotational dizziness. There were 8 patients with right, and 4 with left hemispheric lesions. The lesion was localized in the vicinity of the central sulcus in 3 patients, temporal and/or parietal regions in 6, and occipital region in 3. Vertigo was seen in three patients, two of them with lesions in the vicinity of the central sulcus, and one in the temporo-parietal junction. The other 9…

BACKGROUND: In previous studies of dizziness, the prevalence of specific causes has varied widely and either no or multiple causes have been identified. Dizziness might be better considered a geriatric syndrome that results from impairment or disease in multiple systems.
OBJECTIVE: To determine the predisposing characteristics and situational factors associated with dizziness.
DESIGN: Population-based, cross-sectional study.
SETTING: Community.
PARTICIPANTS: Probability sample of 1087 community-living persons in New Haven, Connecticut, who were at least 72 years of age.

A 62-year-old woman entered a cardiac rehabilitation program for help with weight loss and fitness, and also for monitoring of episodic atrial fibrillation, which had begun in 1992. She was able to exercise without triggering atrial fibrillation. Additional past medical problems included glucose intolerance (treated with diet), hyperlipidemia, hypertension, migraine headaches, degenerative disease of the spine, and a duodenal ulcer.

A growing body of evidence supports the idea that dizziness that persists for months and even years can be caused by an unsuspected perilymphatic fistula. Perilymphatic fistulas are abnormal ruptures that allow perilymph to leak out of the inner ear into the middle ear space. Most commonly, these ruptures occur secondary to a traumatic event. The term postconcussive syndrome has been used to describe a myriad of symptoms following head trauma. Some of these symptoms, such as cognitive changes, tinnitus, neck stiffness, and dizziness, are also commonly caused by active perilymphatic fistulas. T…

Dizziness is a common but often nonspecific symptom. Through careful history taking and physical examination, primary care physicians can usually determine the type of sensation (vertigo or light-headedness) and its source. Although most causes of dizziness are benign, disorders of the central nervous system tend to be ominous and require immediate referral to a specialist.

A previously undescribed syndrome with profound sensorineural hearing loss since early infancy, episodes of dizziness, and hypodontia developed in a brother and sister. The presence of two unaffected children and unaffected parents suggests autosomal recessive inheritance.

Different causes of dizziness or vertigo can only be recognized by thorough anamnestic explorations. Following a classification in vestibular and nonvestibular causes for vertigo, a further differentiation is possible by defining different characteristic qualities of the symptoms involved. In addition to the classical vestibular forms of vertigo seen, dizziness currently results from drug overdosages, hypertension, polyneuropathy and--less commonly, but equally important--brief epileptic seizures. Psychosomatic and neurotic symptoms may also lead to unsteady gait, dizziness or vertigo, all of …

BackgroundChronic dizziness is a multidimensional condition often influenced by psychological factors. Among these, personality traits such as neuroticism have been linked to heightened symptom perception and disability. However, few studies have systematically examined the relationship between neuroticism, functional burden, and diagnostic characteristics in a population with clinical dizziness.ObjectiveTo investigate the relationship between perceived dizziness-related handicap and neuroticism as a personality trait among patients referred to a multidisciplinary dizziness and balance clinic.…
