Fifty-five outpatients with chronic duodenal erosions and no previous ulcer history were treated, in a double-blind fashion, with either pirenzepine (50 mg twice daily) or ranitidine (150 mg twice daily) for 6 weeks. The drugs were equally effective in inducing symptomatic relief. At endoscopic control, 70.4% of subjects in the pirenzepine group and 39.3% of ranitidine-treated patients showed complete healing (p less than 0.05). Primary upper gastro-intestinal tuberculosis is a very rare disease. We report the case of a 25-year-old man who presented with a febrile illness and non-specific gastro-intestinal symptoms. The upper gastro-intestinal X-rays showed a normal esophagus and non-specific changes of the stomach and duodenum. Endoscopy revealed diffuse esophagogastro-duodenitis of tuberculous origin. The patient recovered completely after treatment.

A case of chronic cholecystitis and duodenojejunitis secondary to Giardia lamblia is presented. Though well documented as a cause of duodenitis, this condition is not a common finding in the United States and hence the diagnosis is not made routinely. This case illustrates an instance where giarditic duodenojejunitis may have been masquerading as a chronic cholecystitis.

We report a case of Strongyloides stercoralis infection in an Ethiopian female refugee with advanced, untreated HIV who presented with weight loss, dysphagia, gastrointestinal bleeding, and pancytopenia. Duodenal and colonic biopsies confirmed the diagnosis, and the patient was successfully treated with oral ivermectin and initiation of antiretroviral therapy (ART), with full symptom resolution at follow-up. A 44-year-old male had persistent hypoalbuminemia and ascites after liver transplantation. Imaging of the liver and gastrointestinal system was normal. Urine examination was negative for proteinuria. A diagnosis of protein-losing enteropathy was suspected, and a duodenal biopsy was done. Duodenal biopsy was positive for cytomegalovirus (CMV). The patient improved with CMV treatment.

We describe a cohort of 33 patients with eosinophilic esophagitis (EoE) and incidental duodenal bulb inflammation, termed bulbar duodenitis (BD). We conducted a single-center retrospective cohort study and recorded demographics, clinical presentation, endoscopic, and histological findings. BD was observed at the initial endoscopy in 12 cases (36%) and at a subsequent endoscopy in the remainder. Bulbar histology was usually a mix of chronic and eosinophilic inflammation.
