[Indications for psychotherapeutic treatment of functional dysphonias].

Functional voice disorders require adequate psychiatric screening and treatment due to the complex etiologies of the psychic elements involved. While the diagnosis of dysphonia increasingly takes into account various emotional factors, considerations of indications for psychotherapy and for differing forms of psychotherapy are missing in the literature. To fill this gap, the aims and principles of a number of psychoanalytically based therapeutic methods are presented. Criteria for general indications for treatment and with respect to patients with functional voice disorders are outlined. Case …

Dysphonia in the aging: physiology versus disease.

A chart review from 151 dysphonic patients over the age of 60 was done to define aging related voice disorders. Overwhelmingly, patients suffered from dysphonia due to disease processes associated with aging rather than to physiologic aging alone. These include: 1. central neurological disorders affecting laryngeal function (e.g., stroke, Parkinson's disease, essential tremor, Alzheimer's disease); 2. benign vocal fold lesions (e.g., Reinke's edema, benign and dysplastic epithelial lesions); 3. inflammatory disorders (e.g., laryngitis sicca, medication effect); 4. laryngeal neoplasia; and 5. l…

[Computed tomography of the larynx in the diagnosis of dysphonia].

By comparing CT scans of the larynx with anatomical sections in the horizontal plane as well as by examining patients, a method of investigation especially aimed at phoniatric questions has been worked out: CT scans are performed every 4 mm parallel to the vocal cord plane using a center of 50 HE and a window of 350 HE. The examination is done during regular respiration and if required can also be performed during phonation when producing the vowel /u/. Using this technique the laryngeal structures of clinical relevance can be demonstrated with the help of at least 10 scans. It requires little

Defining the psychiatric role in spastic dysphonia.

The authors evaluated 11 surgically-treated patients with spastic dysphonia, a phonation disorder of unclear etiology. The results indicate that the illness does not appear to be a somatoform disorder, but that stress may play a role in its expression, and that there may be secondary depression and anxiety. The experience of spastic dysphonics suggests that psychiatric treatments may be inappropriately applied to an illness without clear organic etiology, whereas, conversely, a proper psychiatric role may be rejected when effective medical or surgical treatment is available. The authors recomm…

Dysphonia: an unusual presentation of spontaneous pneumomediastinum.

Spontaneous pneumomediastinum, an unusual entity in children, is rarely associated with dysphonia. Muffling of the voice is more commonly associated with such pediatric illnesses as retropharyngeal or peritonsillar abscesses and epiglottitis. We report a 14-year-old female with spontaneous pneumomediastinum who presented to the emergency department with dysphonia as the primary complaint. Pneumomediastinum should be included in the differential diagnosis of dysphonia.

Treatment of spastic dysphonia by EMG biofedback.

Two middle-aged subjects, a male and female, with spastic dysphonia (hoarseness, stammering) were treated with both frontalis and throat muscle electromyographic (EMG) biofeedback. Both subjects reported no noticeable improvement in their voice qualities. Results of a battery of psychological tests provided some insight into the inability of EMG treatment, as well as other therapeutic approaches, to alleviate the pathology.

[Dysphonia and dysphagia after orotracheal intubation].

In this paper, we explain the importance of suspecting foreign bodies in the aerodigestive tract in previously healthy children who present with acute or chronic symptoms, with or without a history of aspiration/ingestion of a foreign body or airway instrumentation.

Laryngeal Sensory Symptoms in Spasmodic Dysphonia.

OBJECTIVE: The purpose of this research was to determine whether an association between laryngeal hypersensitivity (LH) and spasmodic dysphonia (SD) exists using the LH Questionnaire (LHQ). This study also explored the prevalence of self-reported upper respiratory infection (URI) at the time of SD onset across SD phenotypes. METHODS: Individuals with and without SD were recruited to complete an online survey measure. All respondents provided demographic information and completed the LHQ. Participants with SD were also asked to provide information about their diagnosed SD subtype and pattern o…
