Adult respiratory distress syndrome associated with adult epiglottitis.

Adult respiratory distress syndrome (ARDS) is an increased permeability pulmonary edema syndrome known to be associated with a variety of direct and indirect pulmonary insults. ARDS occurred in a 37-year-old woman during an episode of epiglottitis. The patient was treated with endotracheal intubation and mechanical ventilation and recovered, to be discharged from the hospital 1 week after presentation. There are only two cases of ARDS reported in this setting, and in each case, pulmonary edema followed cardiac arrest and the patients had a fatal outcome.

Cardiac failure following inadvertent administration of high-dose epinephrine subcutaneously.

Our aim is to report the consequences of epinephrine toxicity leading to cardiac failure in a child and the successful management with dopamine and milrinone. A previously healthy 13-year-old girl undergoing a left tympanomastoidectomy was inadvertently administered 10 mL of 1:1000 epinephrine subcutaneously (0.175 mg/kg) on the left post auricular region in lieu of lidocaine. She developed sudden supraventricular tachycardia, hypertension and flash pulmonary edema. She was initially treated with propofol, nitrogycerin and increased peak end-expiratory pressure. Within 4 h, she remained tachyc

Pulmonary complications following tricyclic antidepressant overdose in an adolescent.

OBJECTIVE: To report a case of pulmonary edema following a tricyclic antidepressant (TCA) overdose in an adolescent. CASE SUMMARY: A 14-year-old girl with a history of prior suicide attempts ingested 54 50-mg desipramine hydrochloride tablets (45 mg/kg ingestion). The patient developed a cardiac dysrhythmia and hypotension, which were successfully treated. She subsequently developed pulmonary edema and a clinical picture suggestive of adult respiratory distress syndrome (ARDS). She was successfully managed with fluid restriction, tracheal intubation, application of positive end-expiratory pre

Ruptured Pericardial Hydatid Cyst Causing Mitral Annulus Distortion With Severe Mitral Regurgitation and Tamponade-like Symptoms: A Case Report.

BACKGROUND Hydatid disease, a parasitic zoonotic infection caused by Echinococcus granulosus, is characterized by the formation of cysts in various organs. Transmission occurs via the fecal-oral route. The disease is endemic in regions where livestock farming is common, including rural areas of the Mediterranean, Africa, the Middle East, and South America. The liver and lungs are the most commonly affected organs, with reported involvement rates of 50% to 70% and 20% to 30%, respectively. In rare cases, cardiac involvement occurs (0.5%-2%), most frequently affecting the left ventricle due to i

Raised leg exercises for leg edema in the elderly.

UNLABELLED: Leg edema is a common problem in the elderly and requires further evaluation and management. METHOD: From October 1990 to July 1992, 245 patients presented to the Cleveland Clinic Florida with leg edema. All patients were counseled about the benefits of twenty-minute, three-times-a-day raised-leg exercises. Fifty seven (57) of the 245 patients were not compliant with this regimen (nonexercise group). Although not true controls, they formed a comparison group for those who performed the exercise regimen. The exercise group was composed of 188 patients with a mean age of 73 +/- 6.8

Transient visual impairment following cardiac arrest.

This report presents an unusual case of visual impairment following cardiac arrest during general anesthesia. The presumptive etiology was macular capillary insufficiency secondary to vascular stagnation which occurred during cardiac standstill. Ophthalmic examination confirmed areas of macular edema corresponding to localized perifoveal regional infarcts. Isolated macular infarction is not a unique condition and has been reported in hemoglobulinopathies, diabetes, and other systemic diseases.

Effects of Low-Dose Sacubitril/Valsartan on Different Stages of Cardiac Hypertrophy in Salt-Loaded Hypertensive Rats.

BACKGROUND: Sacubitril/valsartan was shown to attenuate the development of cardiac hypertrophy with enhanced blood pressure reduction compared with valsartan alone in animal models. We investigated whether a low-dose sacubitril/valsartan has blood pressure-independent effects on cardiac hypertrophy and pulmonary edema using a rat model of hypertension and obesity. METHODS AND RESULTS: In plan 1, male SHR/NDmcr-cp rats fed normal or phase-increased high salt were treated with vehicle, 6-mg/kg sacubitril/valsartan or 3-mg/kg valsartan, for 6 months. In plan 2, after high-salt loading for 6 mont

Cardiac Magnetic Resonance Studies in a Large Animal Model that Simulates the Cardiac Abnormalities of Human Septic Shock.

BACKGROUND: Septic shock, in humans and in our well-established animal model, is associated with increases in biventricular end diastolic volume (EDV) and decreases in ejection fraction (EF). These abnormalities occur over 2 days and reverse within 10 days. Septic non-survivors do not develop an increase in EDV. The mechanism for this cardiac dysfunction and EDV differences is unknown. METHODS: Purpose-bred beagles randomized to receive intrabronchial Staphylococcus aureus (n=27) or saline (n=6) were provided standard ICU care including sedation, mechanical ventilation, and fluid resuscitatio

Water Lung: An Interesting Case of Non-cardiogenic Pulmonary Edema in a Heart Failure Patient.

Contrast-induced pulmonary edema is a rare but life-threatening condition often missed in heart failure patients. We present a case of a 65-year-old female with a past medical history of coronary artery disease, diastolic heart failure, and chronic kidney disease who presented with chest pain. She received low osmolar intravenous (IV) contrast for cardiac catheterization. Within 24 hours of receiving the contrast, the patient developed respiratory distress, which was found to be secondary to pulmonary edema. Pulmonary edema was considered to be related to cardiogenic at first; however, the pat

Case Report: A Case of Severe Cerebral Malaria Managed with Therapeutic Hypothermia and Other Modalities for Brain Edema.

Malarial infections are uncommon in the United States and almost all reported cases stem from recent travelers coming from endemic countries. Cerebral malaria (CM) is a severe form of the disease usually affecting children and individuals with limited immunity. Despite proper management, mortality from CM can reach up to 25%, especially when it is associated with brain edema. Inefficient management of the edema may result in brain herniation and death. Uniform guidelines for management of CM-associated brain edema are lacking. In this report, we present a case of CM with associated severe brai

Bumetanide: a new diuretic. Results of clinical efficacy and safety in patients with congestive heart failure.

A clinical study of bumetanide, a new diuretic, was conducted in outpatients of a cardiology clinic to determine its long-term effectiveness and safety in the treatment of peripheral edema due to congestive heart failure. The drug was administered for at least 24 weeks on a selected group of 34 patients. The patients were monitored for clinical response, evidenced by changes in cardiac signs and symptoms, and by laboratory tests, as well as by slit-lamp ophthalmic examination, electrocardiogram, and chest x-ray. Based on the clinical evaluation of effects on body weight, chronic edema, functio

Transient Insulin-Induced Edema Following Rapid Metabolic Correction in Poorly Controlled Pediatric Type 1 Diabetes Mellitus.

Insulin-associated edema is an uncommon but well-documented side effect of insulin therapy. It typically occurs with the initiation or rapid escalation in dosage of insulin, especially in patients who have experienced prolonged uncontrolled hyperglycemia. It is frequently overlooked and may result in unnecessary investigations and management. We describe a 14-year-old girl with long-standing poorly controlled type 1 diabetes mellitus (HbA1c 15.1%) who, the week following recovery from severe diabetic ketoacidosis and a rapid increase in insulin therapy, experienced acute edema and gained seven

Reexpansion pulmonary edema following patent ductus arteriosus ligation in a preterm infant.

Reexpansion pulmonary edema (RPE) is rare and usually follows rapid reexpansion of a collapsed lung. We report on a preterm infant who developed pulmonary edema within an hour of surgical ligation of patent ductus arteriosus (PDA). There were no other cardiac anomalies, fluid overload or airway obstruction to explain the change in clinical status. With supportive treatment the patient's condition became stable and was extubated within 48 hours. Lung retraction for better field exposure is often needed when performing PDA ligation in preterm infants. Reinflation of a retracted lung is thought t

[Recurrent unilateral pulmonary edema due to left heart failure].

Unilateral pulmonary edema is uncommon and is usually associated with prolonged surgical procedures or rapid evacuation of a hydro- or pneumothorax. Unilateral pulmonary edema due to left heart failure in the absence of known lung disease is rare. It is therefore not readily recognized and is often confused with other unilateral alveolar or interstitial infiltrates. We describe 2 patients, a 69-year-old man and a 78-year-old woman, who had repeated episodes of unilateral pulmonary edema due to left heart failure. In both cases several other diagnoses were considered, but the cardiac origin of

Upper Sternal Cleft With a Complex Congenital Heart Defect: Repair in a Single Stage.

Sternal clefts are extremely rare in association with complex congenital heart defects, and their management can be challenging. Complete surgical repair in early infancy, of both anomalies in a single stage, is advocated. Different surgical techniques exist for isolated sternal cleft repair, but they do not consider the difficulties of combining sternal defect closure and a cardiac operation with the unavoidable postoperative cardiac edema. We describe a successful surgical technique to repair a sternal cleft in a 3-month-old infant in a concomitant cardiac operation using an absorbable polyg

Cardiac lesions in sheep and goats with spontaneous Clostridium perfringens type D enterotoxemia.

A 12-year retrospective search identified 106 sheep and goats diagnosed with Clostridium perfringens type D enterotoxemia, and in which the heart was histologically examined. Twenty cases (20/106, 19%), including 10 sheep and 10 goats, had cardiac lesions that were presumed to be associated with enterotoxemia. The lesions included myocardial degeneration and/or necrosis (n = 16, 80%), hemorrhage (n = 17, 85%), and proteinaceous interstitial edema (n = 6, 30%). Myocardial degeneration and/or necrosis was more frequent in goats (10/10, 100%) compared with sheep (6/10, 60%). Hemorrhage was more f

Navigating Hypoxemic Respiratory Failure in Critically Ill Cardiac Patients.

Acute respiratory failure is a common reason for admission to cardiac intensive care units and the prevalence of respiratory failure in this cohort is increasing over time. Hypoxemia can occur due to a variety of mechanisms; the most common cause in the cardiac intensive care units remains cardiogenic pulmonary edema, but other etiologies, such as pneumonia and acute respiratory distress syndrome, are also common. This article provides an update on mechanisms of hypoxemia among patients with cardiac critical illness, heart lung interactions during spontaneous and positive pressure ventilation,

Angiopoietin-2 is associated with capillary leak and predicts complications after cardiac surgery.

BACKGROUND: Patients undergoing cardiac surgery are prone to numerous complications. Increased vascular permeability may be associated with morbidity and mortality due to hemodynamic instability, fluid overload, and edema formation. We hypothesized that markers of endothelial injury and inflammation are associated with capillary leak, ultimately increasing the risk of postoperative complications. METHODS: In this prospective, observational, multidisciplinary cohort study at our tertiary academic medical center, we recruited 405 cardiac surgery patients. Patients were assessed daily using body
