Baseball has an elbow injury problem: biomechanics may have the solution.

Elbow injuries-particularly to the ulnar collateral ligament (UCL)-have been a problem in sports medicine for at least half a century. As a pitcher rotates his trunk to face the target and externally rotates his throwing arm, varus torque is produced at the elbow to decelerate arm cocking and initiate arm acceleration. With today's pitchers being larger, throwing harder, and playing more often during their development years, repetition of large varus torque leads to more UCL injuries. Pitch count limits for games, seasons, and per year may provide some prevention of overuse injuries. Varying e…

Trans-olecranon fracture posterior dislocation: a novel type of elbow injury.

BACKGROUND: Based on our experiences, we found that a kind of elbow injury is characterized by an olecranon fracture accompanied by elbow joint posterior dislocation with the proximal radioulnar joint intact. The aim of this study was to better define this kind of severe elbow instability, which has not been previously reported. METHODS: We retrospectively analyzed all patients with olecranon fractures who were treated at our institution from January 2013 to April 2021. Data on these patient demographics, injury characteristics, preoperative and postoperative imaging, surgical management, and

Distal Radius Fracture with Ipsilateral Elbow Dislocation: A Rare but Challenging Injury.

Distal radius fractures are common and account for approximately 14% to 18% of all adult extremity injuries. On rare occasions, ipsilateral elbow dislocation can be observed additionally. However, this can be missed without careful examination, especially in patients experiencing altered mental status. The aim of this study was to analyze the mechanism, level of injury, demographics, and associated injuries in distal radius fracture with ipsilateral elbow dislocation. Between 2012 and 2019, we searched our trauma database for distal radius fracture with ipsilateral elbow dislocation. All patie

Simple medial elbow dislocations: a rare injury at risk for early instability.

PURPOSE: To report the presentation, treatment, and outcomes of a series of simple medial elbow dislocations and to identify features distinguishing this injury from the more common dislocation patterns. METHODS: From 2000 to 2011, 4 cases of simple medial elbow dislocations were treated at a single referral center. Retrospective review was conducted to evaluate presentation, treatment, and outcomes. RESULTS: The mean patient age was 56 years (range, 49-61 y). All dislocations were in the nondominant arm of women after a fall from standing height. Two elbows had immediate closed reduction, a

Association of maximum pitch velocity and elbow injury in professional baseball pitchers.

BACKGROUND: Recent literature has explored the association of upper extremity injury in baseball players with various aspects of the pitchin … lbow injury was then analyzed using an unpaired Student t test. RESULTS: There were 9 players with elbow injuries in the group of pitchers studied, including 4 pitchers with an elbow muscle strain and/or joint inflammation and 5 pitchers with an ulnar collateral ligament sprain or tear. Three of the ulnar collateral ligament injuries required surgery. For the 1 …

Compartment syndrome associated with distal radial fracture and ipsilateral elbow injury.

BACKGROUND: Forearm compartment syndrome is an uncommon sequela of distal radial fractures. This investigation tested the hypothesis that the risk of forearm compartment syndrome associated with an unstable, operatively treated fracture of the distal end of the radius is higher with a concomitant injury of the ipsilateral elbow. METHODS: All patients who sustained an unstable fracture of the distal end of the radius and/or injury to the elbow (a fracture of the proximal end of the radius and/or ulna, simple elbow dislocation, elbow fracture-dislocation, or distal humeral fracture) and were op

Posterior elbow dislocation with associated vascular injury after blunt trauma.

BACKGROUND: Injury of the brachial artery is a rare (5-13%) but serious complication after closed elbow dislocation without associated fractures. METHOD: Retrospective analysis of long-term results (mean, 4.1 years) in four patients. RESULTS: All patients underwent emergency repair of the arterial injury within 2.5 hours. In three patients, a reversed saphenous vein graft was used; in one patient the artery was sutured. This latter patient needed another operation with interposition of a reversed saphenous graft, because the primary anastomosis occluded. The capsule and the collateral ligame

Ulnar nerve injury at the elbow after steroid injection for medial epicondylitis.

We describe an accidental injury to the ulnar nerve at the elbow following steroid injection for medial epicondylitis in a patient with undetected recurrent dislocation of the nerve. The chalky substance found on exploration to intermingle with the nerve fascicles was removed and the nerve was placed under the flexor-pronator mass. After surgery, complete resolution of the epicondylar pain was noted, along with significant improvement in two-point discrimination of the ulnar digits. We recommend clinical assessment of the ulnar nerve location prior to injection to avoid injury. Should recurren

Surgical Options for the Restoration of Elbow Extension After Brachial Plexus Injury.

Brachial plexus injuries (BPIs) are devastating and lead to marked functional disability. Injuries involving the proximal radial nerve, posterior cord, or lower cervical nerve roots can lead to triceps palsy and inability to actively extend the elbow. Although not as functionally important as elbow flexion, active elbow extension is required for some activities of daily living, and discussion of reconstructive options with the patient is warranted. Various nerve transfers have been successfully used and may be available, depending on the extent of injury and other surgeries planned or performe

Terrible triad of the elbow with an ipsilateral Essex-Lopresti injury: case report.

Terrible triad of the elbow and the Essex-Lopresti injury are both rare lesions with a historically poor clinical outcome. We present the case of a unique association of the two injuries with an elbow dislocation, radial and coronoid process fractures and a distal radioulnar joint dislocation due to an interosseous membrane rupture. The case was managed with closed reduction of the elbow dislocation and distal radioulnar joint followed by open reduction and repair of the damaged structures in the elbow and an unloading of the interosseous membrane. A high index of suspicion with a detailed exa

Biepicondylar fracture dislocation of the elbow joint concomitant with ulnar nerve injury.

In this article, we present a case of humeral biepicondylar fracture dislocation concomitant with ulnar nerve injury in a seventeen year-old male patient. Physical examination of our patient in the emergency room revealed a painful, edematous and deformed-looking left elbow joint. Hypoesthesia of the little finger was also diagnosed on the left hand. Radiological assessment ended up with a posterior fracture dislocation of the elbow joint accompanied by intra-articular loose bodies. Open reduction-Internal fixation of the fracture dislocation and ulnar nerve exploration were performed under ge

Orthopedic pitfalls in the ED: neurovascular injury associated with posterior elbow dislocations.

Posterior elbow dislocations are the most common type of elbow dislocation and are usually caused by a fall on an outstretched hand. Although the incidence of elbow dislocation complications is rare, the emergency physician is responsible for evaluation and identification of concomitant neurovascular injuries. Failure to identify neurovascular compromise after elbow dislocation or reduction can potentially lead to severe morbidity with limb ischemia, neurologic changes, compartment syndrome, and potential loss of limb. Cyanosis, pallor, pulselessness, and marked pain should suggest vascular in

Brachial artery injury in closed posterior elbow dislocation case report.

The authors describe a case with a closed posterior elbow dislocation associated with a distal radial fracture and complete transsection of the brachial artery. The patient had a pulseless distal upper extremity and immediate gross swelling of the elbow and forearm. As closed reduction was not possible, open reduction had to be performed through an anteromedial approach to the elbow. End-to-end suture of the brachial artery was successful. After fasciotomy and internal fixation of the distal radial fracture, the elbow was stabilized with an external fixator spanning the elbow joint. After two

Does bone mineral mass affect medial elbow injuries in young baseball players?: evaluation using bioelectric impedance analysis.

BACKGROUND: Medial elbow injuries occur in approximately one-third of all young baseball players and are characterized by injuries to the bone and cartilaginous growth plates. Bone mineral mass, a representative of bone strength, may show individual differences during growth, and the bone strength can temporarily decrease depending on the stage of bone development. However, the relationship between bone mineral mass and medial elbow injuries in young baseball players has not yet been investigated. HYPOTHESIS: The bone mineral mass would be lower in players with medial elbow injuries, causing …

Nerve Transfers for Elbow Reconstruction in Upper and Extended Upper-Type Brachial Plexus Injuries: A Case Series.

BACKGROUND: Nerve transfers for elbow flexion in brachial plexus injuries have been used with increasing frequency because of the higher rate of success and acceptable morbidity. This is especially true in upper and extended upper-type brachial plexus injuries. OBJECTIVE: To present the clinical outcomes of nerve transfers for elbow flexion in patients with upper and extended upper-type brachial plexus injuries. METHODS: A retrospective cohort review was done on all patients with upper and extended upper-type brachial plexus injuries from 2006 to 2017, who underwent nerve transfers for the r

The Effectiveness of a Hinged Elbow Orthosis in Medial Collateral Ligament Injuries: An In Vitro Biomechanical Study.

BACKGROUND: Medial collateral ligament (MCL) injuries are common after elbow trauma and in overhead throwing athletes. A hinged elbow orthosis (HEO) is often used to protect the elbow from valgus stress early after injury and during early return to play. However, there is minimal evidence regarding the efficacy of these orthoses in controlling instability and their influence on long-term clinical outcomes. PURPOSE: (1) To quantify the effect of an HEO on elbow stability after simulated MCL injury. (2) To determine whether arm position, forearm rotation, and muscle activation influence the eff
