Gastric electrical stimulation for the treatment of diabetic gastroparesis.

Diabetic gastroparesis is a component of autonomic neuropathy, and is the most common manifestation of gastrointestinal neuropathy. Diabetes is responsible for about one quarter of gastroparesis. The upper gastrointestinal symptoms are often non-specific and dominated by nausea, vomiting, early satiety, fullness, bloating. We also have to look for diabetic gastroparesis in case of metabolic instability, such as postprandial hypoglycaemia. The pathophysiology of diabetic gastroparesis is complex, partly due to a vagus nerve damage, but also to changes in secretion of hormones such as motilin an

Wireless distributed functional electrical stimulation system.

BACKGROUND: The control of movement in humans is hierarchical and distributed and uses feedback. An assistive system could be best integrated into the therapy of a human with a central nervous system lesion if the system is controlled in a similar manner. Here, we present a novel wireless architecture and routing protocol for a distributed functional electrical stimulation system that enables control of movement. METHODS: The new system comprises a set of miniature battery-powered devices with stimulating and sensing functionality mounted on the body of the subject. The devices communicate wi

Gastric electrical stimulation on gastric motility in dogs.

AIMS: The aim of this study was to systematically evaluate and compare the effects of synchronized dual-pulse gastric electrical stimulation (SDPS) and conventional gastric electrical stimulation (CGES) on antral contractions, gastric tone, and autonomic functions. METHODS: Seven female hound dogs implanted with four pairs of electrodes on gastric serosa were studied in two separate experiments. The first experiment was designed to investigate the effects of SDPS/CGES on gastric tone and heart rate variations. The second experiment was performed to study the effects of SDPS/CGES on antral con

Neural responses elicited by electrical stimulation of the retina.

PURPOSE: To study electrically elicited responses (EERs) that are produced by epiretinal stimulation of normal and degenerated retina. METHODS: Three biological models of retinal degeneration are compared: normal and rd1 mouse, normal and RCD1 dog, and human with retinitis pigmentosa. In mouse, epiretinal stimulation was accomplished by means of a wire inserted in the vitreous cavity, and single-unit activity was recorded in visual cortex. In dog and human, an implantable retinal stimulator was used to stimulate the retina, and evoked potentials were recorded from the cortical surface (dog) o

Outcomes of electrically stimulated gracilis neosphincter surgery.

OBJECTIVES: To examine patient quality of life (QoL) and long-term costs of electrically stimulated gracilis neosphincter surgery (ESGNS). DESIGN: Independently conducted prospective case-comparison study of patients at the Royal London Hospital (RLH), plus a cross-sectional study of outcomes of ESGNS performed at three other UK centres. PARTICIPANTS: Cases were patients who underwent ESGNS at the participating hospitals during a 5-year period from 1977. Comparisons were made with two groups of people with similar bowel disorders who did not undergo ESGNS. INTERVENTION: ESGNS is a procedure

Overactive bladder: magnetic versus electrical stimulation.

PURPOSE OF REVIEW: To review recent literature on the electrical and magnetic stimulation of the sacral nerve roots and pelvic floor for the treatment of overactive bladder. RECENT FINDINGS: Overactive bladder is a common condition affecting millions of women worldwide, with a significant effect on quality of life. Electrical stimulation and neuromodulation of the sacral nerve roots have provided a useful alternative for these patients with satisfactory outcomes. The use of the procedures has been limited, however, mainly due to local discomfort/pain or invasiveness of the surgical procedure.

Synchronized electrical stimulation in treating pharyngeal dysphagia.

OBJECTIVE/HYPOTHESIS: The objectives were to test the hypothesis that synchronous contraction of the thyrohyoid muscle by electrical stimulation during swallowing would improve dysphagia resulting from reduced laryngeal elevation and to evaluate the effectiveness of the synchronous electrical stimulator. STUDY DESIGN: Prospective study. METHODS: Eleven male and 12 female patients (age range, 35-87 y) with moderate to severe degree of dysphagia resulting from reduced laryngeal elevation who did not respond to medical treatment were treated by stimulating synchronous contraction of the thyrohy

Electrical stimulation of wrist extensors in poststroke hemiplegia.

BACKGROUND AND PURPOSE: It has been suggested that cyclic neuromuscular electrical stimulation (ES) may enhance motor recovery after stroke. We have investigated the effects of ES of the wrist extensors on impairment of wrist function and on upper-limb disability in patients being rehabilitated after acute stroke. METHODS: We recruited 60 hemiparetic patients (mean age, 68 years) 2 to 4 weeks after stroke into a randomized, controlled, parallel-group study comparing standard rehabilitation treatment with standard treatment plus ES of wrist extensors (3 times 30 minutes daily for 8 weeks). The electrically stimulated neoanal sphincter and colonic conduit.

Faecal incontinence affects about 2.2% of the population. Non-surgical interventions include dietary manipulation or the use of enemas and drug regimens. Surgical procedures such as anterior sphincter repair may improve function for the majority of patients, but for some a permanent stoma may be the only therapeutic option. A technique pioneered at the Royal London Hospital uses gracilis muscle augmentation combined with electrical stimulation to improve sphincter control. However, some patients experience residual evacuatory problems. Antegrade irrigation may be incorporated into the procedur

Application of tilt sensors in functional electrical stimulation.

Tilt sensors, or inclinometers have been investigated for the control of Functional Electrical Stimulation (FES) to improve the gait of persons who had a stroke or incomplete spinal cord injury (SCI). Different types of tilt sensors were studied for their characteristics and their performance in measuring the angular displacement of leg segments during gait. Signal patterns of the lower leg with inertial tilt sensors were identified with control subjects and subjects with footdrop who are being stimulated during level walking. To minimize acceleration responses when the foot swings or hits the

Transcranial electrical stimulation in patients with apallic syndrome.

Muscle responses (MEPs) to transcranial electrical stimulation were studied in 7 patients with apallic syndrome. All the patients showed clinical signs of upper motor neurone impairment in the upper and lower limbs. MEPs were absent or markedly delayed in 4 of the 7 patients. Since patients with apallic syndrome show only minimal voluntary movement, transcranial stimulation is the only way to demonstrate abnormalities of fast corticospinal axons in these patients. Even though these patients often look similar clinically, with tetraplegia and decorticate or decerebrate posture, only some cases

Bipolar surface electrical stimulation of the vertebrate retina.

BACKGROUND: Retinitis pigmentosa with attendant photoreceptor loss can cause a profound visual handicap. We have postulated that an intraocular prosthesis that could electrically stimulate the inner retina might provide vision to some of these patients. For such a prosthesis to be feasible, electrical stimulation of the inner retina must elicit a focal retinal response. The stimulating current densities required to elicit such a response must not result in irreversible toxic reactions at the electrode-tissue interface. METHODS: To test the feasibility of this approach, we used bipolar platinu

Modeling axon membranes for functional electrical stimulation.

Four models are discussed which can be used to predict the behavior of warm-blooded axons, when excited by electric fields. Up to now, most results were obtained with the Frankenhaeuser-Huxley model, but nearly all of them are wrong in time scale and the cathodic block phenomenon was not observable because the temperature dependence of the gating mechanism has been neglected. However, in the corrected form this model reacts with similar excitability as the two other myelinated nerve models which consider that the potassium current is negligible in mammalian axon membranes. Electrical vestibular stimulation and space motion sickness.

Electrical vestibular stimulation (EVS) in dynamic balance condition was studied in order to search for a new provocative test of space motion sickness (SMS). SMS is usually attributed to a sensory conflict caused by exposure to microgravity. Vestibular information is conflicting but also unusual and insignificant. EVS is in accordance with this feature because it is not the adequate stimulus of the vestibular receptors. EVS was achieved by means of binaural electrical stimulation. Effects of EVS were potentiated by compelling the subject to maintain dynamic balance on a seesaw. The quantifica

Application of transcutaneous electrical nerve stimulation in dentistry.

TENS is extremely useful in some dental procedures, such as TMJ syndrome and tooth extraction; however, its use is not practical in all situations. The dentist must remember that TENS is an adjunctive form of treatment. It is not a panacea for all types of pain, nor should it be used as a last resort. When applied correctly and with care, TENS is useful in the management of pain in the head and face.
