How can we identify patients with delirium in the emergency department?: A review of available screening and diagnostic tools.

Delirium is a widespread and serious but under-recognized problem. Increasing evidence argues that emergency health care providers need to assess the mental status of the patient as the "sixth vital sign". A simple, sensitive, time-efficient, and cost-effective tool is needed to identify delirium in patients in the emergency department (ED); however, a stand-alone measurement has not yet been established despite previous studies partly because the differential diagnosis of dementia and delirium superimposed on dementia (DSD) is too difficult to achieve using a single indicator. To fill up the

The effect of deep versus awake removal of the laryngeal mask airway on the incidence of emergence delirium in paediatric tonsillectomy: A randomised controlled trial.

Emergence delirium, characterised by inconsolable crying, perceptual disturbances and thrashing, occurs in young children during the recovery phase from general anaesthesia. Our aim was to determine whether timing of laryngeal mask airway removal (deeply anaesthetised versus awake) influenced the incidence of emergence delirium in children after tonsillectomy. A single-centre, randomised controlled trial was conducted at Albury Wodonga Health, a regional hospital in Australia. Included patients were two to seven years old, American Society of Anesthesiologists physical status classification 1-…

Effectiveness of preoperative intranasal dexmedetomidine, compared with oral midazolam, for the prevention of emergence delirium in the pediatric patient undergoing general anesthesia: a systematic review.

BACKGROUND: Emergence delirium is defined as a cognitive disturbance during emergence from general anesthesia resulting in hallucinations, delusions and confusion manifested by agitation, restlessness, involuntary physical movement and extreme flailing in bed. Postoperative emergence delirium develops in 12% to 18% of all children undergoing general anesthesia for surgery. This post-anesthetic phenomenon changes cognitive and psychomotor behavior, and puts pediatric patients and health care personnel at risk of injury. A newer drug, dexmedetomidine, is a selective alpha-2 agonist, which works …

Risk factors and prediction model of emergence delirium in children after general anesthesia under preoperative sedation with uniform oral midazolam premedication: a prospective observational cohort study.

BACKGROUND: Emergence Delirium (ED) is a frequent complication following general anesthesia in children. It is multifactorial, with no single cause conclusively identified. However, limited research has examined the risk factors for ED in children under such a protocol. Therefore, our goal was to identify independent risk factors for ED in this unified preoperative sedation protocol. METHODS: This prospective observational study collected medical records of children undergoing elective tonsillectomy and adenoidectomy under general anesthesia at Kunming Children’s Hospital between March 2023 an…

The efficacy and safety of dexmedetomidine in preventing emergence delirium in paediatric patients following ophthalmic surgery: a systematic review and meta-analysis of randomised controlled trials.

BACKGROUND: The leading cause of emergence delirium (ED) in children postoperatively is the exposure to inhalational anaesthetics. ED can occur immediately after waking from anaesthesia, making patients generally uncooperative and agitated. Dexmedetomidine has sedative and analgesic effects and helps to reduce agitation and delirium and improve hemodynamic stability and the recovery of respiratory function; in addition to decreasing pain intensity, it is also well known for helping reduce nausea and vomiting. OBJECTIVES: This updated systematic review meta-analysis investigate and summarise c…

A large language model-driven multidisciplinary AI agent system predicts delirium in emergency critically ill patients.

Delirium occurs frequently in emergency departments and is associated with poor outcomes and increased burden. Early delirium risk prediction is crucial for timely prevention and intervention in emergency care, but most existing models focus on intensive care unit (ICU) populations and offer limited interpretability and interactivity. We propose DeLiriuMAgents, a large language model (LLM)-driven multi-agent system for predicting delirium risk in emergency critically ill patients. It simulates multidisciplinary clinical consultation by integrating data-driven, machine learning-based risk predi

Effect of clock rhythm on emergence agitation and early postoperative delirium in older adults undergoing thoracoscopic lung cancer surgery: protocol for a prospective, observational, cohort study.

INTRODUCTION: Surgeries conducted at night can impact patients' prognosis, and the mechanism may be related to circadian rhythm, which influence normal physiological functions and pathophysiological changes. Melatonin is primarily a circadian hormone with hypnotic and chronobiotic effects, thereby affecting disease outcomes through influencing the expression of inflammatory factors and biochemical metabolism. This study aims to observe the effects of circadian rhythms on emergence agitation and early postoperative delirium of older individuals undergoing thoracoscopic lung cancer surgery and e
